Mediating role of perceived social support between treatment burden and long-term condition empowerment in older adults

Objective: To explore the mediating effect of perceived social support on treatment burden and long-term condition empowerment in older adults. Methods: The Perceived Social Support Scale, Treatment Burden Questionnaire, Long-term Condition Empowerment Scale were used to conduct a questionnaire survey among 460 older adults from 6 nursing homes in Qingdao during January 2021 to August 2021. Results: Treatment burden was significantly negatively associated with perceived social support (P<0.001) and negatively associated with long-term condition empowerment (P<0.001). Perceived social support was significantly positively associated with long-term condition empowerment (P<0.001).The results of Bootstrap test showed that the perceived social support partially mediated the relationship between treatment burden and long-term condition empowerment, accounting for 51.98% of the total effect. Conclusion: Treatment burden not only directly affects long-term condition empowerment, but also indirectly affects long-term condition empowerment through perceived social support.

This paper introduced the construction and application of "Internet + Nursing Service" intelligent follow-up system for older adults with chronic diseases. The system included accurate follow-up needs assessment based on patient-reported outcomes, intelligent decision making engine supported by evidence-based knowledge base, and flexible and comprehensive online and offline follow-up services. The application of the intelligent system has brought benefits to patients, constructed a new follow-up practice model to meet the diversified and multi-level follow-up needs of older adults with chronic diseases, increased the enthusiasm of nursing staff, and provided a useful reference and enlightenment for the comprehensive development of "Internet + Nursing Service" in the future.

Current situation and influencing factors of geriatric nursing competence of clinical nurses in 28 medical institutions in Beijing

Objective: To investigate the geriatric nursing competence of clinical nurses in Beijing and analyze its influencing factors, so as to provide theoretical basis for improving the geriatric nursing competence of clinical nurses. Methods: From April to May 2022, a cross-sectional survey was conducted among 3202 clinical nurses from 28 medical institutions in Beijing using the general information questionnaire and Geriatric Nursing Competence Scale for Clinical Nurses. Multiple linear regression was used to analyze the influencing factors of geriatric nursing competence of clinical nurses. Results: A total of 3202 valid questionnaires were collected. The total score of geriatric nursing competence of clinical nurses was 119.85±36.60, and the average score of overall items was 2.78±0.85. Among the three first-level dimensions, the score of professional literacy was the highest, and the score of professional development was the lowest. Among the 10 second-level dimensions, the score of safety management was the highest, followed by critical thinking, and the score of scientific research innovation was the lowest. The results of multiple linear regression analysis showed that age, geriatric nursing work years, being a senior professional nurse or not, likeness of clinical work, and clinical work satisfaction were independent influencing factors of geriatric nursing competence of clinical nurses (P<0.05). Conclusion: The geriatric nursing competence of clinical nurses is not balanced and at a medium level. Attention should be paid to the professional development competence, especially the cultivation of scientific research and innovation competence. The hospital should carry out geriatric nursing related training to improve the geriatric nursing competence of clinical nurses.

Supply and demand matching of nursing services for the disabled elderly in the long-term care insurance system in Fujian province

Objective: To understand the supply and demand of services for the disabled elderly in the long-term care insurance system and the matching gap between supply and demand, so as to provide reference for narrowing the matching gap between supply and demand of long-term care insurance services. Methods: A self-designed questionnaire was used to investigate the service supply and demand of 390 disabled elderly people in the long-term care insurance system in Fujian province from May to July 2021 by using convenient sampling method. Results: The total score of service need and service supply of 390 disabled elderly in long-term care insurance nursing institutions was 57.82±3.47 and 52.94±8.41, respectively. There was a significant difference between the supply and demand of nursing services for the disabled elderly (P<0.001). Conclusion: The matching of supply and demand of nursing services for the disabled elderly in the long-term care insurance system in Fujian province is not optimistic. Long-term care insurance nursing institutions should focus on the needs of the disabled elderly and accurately meet their diverse and multi-level health needs to narrow the gap between supply and demand.

Application of motion sensing game in home-based exercise rehabilitation of elderly stroke patients: a scoping review

Objective: To identify relevant research on the application of motion sensing game in home-based exercise rehabilitation of elderly stroke patients, and to provide guidance for further research. Methods: A literature search was performed in the PubMed, Web of Science, Embase, PsycINFO, CINAHL, Cochrane Library, CNKI, Wanfang Data and VIP. The included literature were summarized and analyzed. Results: Totally 14 studies were included. The results suggested that the application of motion sensing game in the home-based exercise rehabilitation of stroke patients was gradually becoming a trend. The combination with virtual reality technology was the most common form of motion sensing game. The goal of home-based exercise rehabilitation was mainly the sensory and motor function of the upper limb. The implementation quality of the patients' home rehabilitation could be reflected by the completion of the game tasks. The outcome indicators included intervention effect, feasibility and patient satisfaction of motion sensing game. Conclusion: Motion sensing game makes positive contribution to the home rehabilitation of elderly stroke patients, and this intervention method is helpful to improve the the experience and quality of home-based exercise rehabilitation. Future research can design corresponding somatic game materials for different limb rehabilitation goals and constantly enrich the home-based exercise rehabilitation game library for stroke patients.

Cancer risk communication in families with hereditary cancers from the perspectives of probands and their relatives: a qualitative study

Objective: To understand the cancer risk communication among family members from the perspectives of probands and their relatives in the families with hereditary colorectal cancer or breast-ovarian cancer syndrome. Methods: This study was a qualitative phenomelological research. In-depth semi-structured interviews were conducted in 14 probands with hereditary colorectal cancer and 8 probands with hereditary breast-ovarian cancer and their 20 relatives from March 2021 to September 2021. Topic analysis method was used to analyze the interview data. Results: Cancer risk communication in hereditary cancer families included three themes: barriers to information exchange in family history; limited cancer risk communication and disease information was selectively communicated; factors associated with cancer risk communication. Conclusion: Poor communication on cancer risk in hereditary cancer families and lack of effective transmission of necessary information seriously influence the understanding of cancer risk among high-risk family members. The communication regarding cancer risk in these family members need to be improved. We can promote the early diagnosis and treatment of cancer in these high-risk families through effective communication.

Objective: To evaluate the effects of integrative psychological nursing intervention based on MPNFS model on anxiety, family intimacy and social support of adolescent patients with anxiety disorder. Methods: A total of 86 adolescent patients with anxiety disorder in the outpatient department of a tertiary grade A hospital in Jingzhou City from January to April 2022 were included by convenient sampling, and were divided into the control group (n=43) and the observation group (n=43) according to the random number table method. Those in the control group were given medical treatment and psychological knowledge education by psychologists in the outpatient department. Those in the observation group were treated with integrative psychological nursing intervention based on MPNFS model on the basis of the control group. The scores of anxiety, family intimacy and social support were observed and compared between the two groups before and after the intervention. Results: There were no significant differences in the scores of anxiety, family intimacy and social support in the two groups before the intervention (P>0.05). After the intervention, the scores of anxiety, family intimay and social support were better in both group, the anxiety score of the observation group was lower, and the scores of family intimacy and social support were higher than that of the control group (P<0.05). Conclusion: Integrative psychological nursing intervention based on MPNFS model can effectively alleviate the anxiety and improve the family intimacy and social support of adolescent patients with anxiety disorder.

Construction of a prediction model on mental health status for spouses of patients with gynecological malignant tumors

Objective: To construct a mental health prediction model for spouses of patients with gynecological malignant tumors. Methods: A convenient sampling was used to select the spouses of 292 patients with gynecological malignant tumor in a tertiary grade A hospital from February 2019 to January 2022. Based on the total score of the Symptom Checklist 90 exceeding 160, the training set was divided into the mental health group (n=96) and the mental health abnormal group (n=109). R language and Logistic regression were used to construct a predictive model, and 87 patients were included for model validation. Results: Logistic regression analysis showed that the protective factors included social support, level of hope, education level, and the risk factors were medical payment method, birth demand and tumor metastasis. The sensitivity of the model was 0.954, the specificity was 0.844, the C-index of consistency was 0.951 (95%CI: 0.920-0.982), and the validation accuracy was good. Conclusion: The predictive model of mental health status for spouses of patients with gynecological malignant tumor drawn by the above factors has a good correlation with the actual observation results, which is worthy popularization.

Development of the Nurses' Practice Behavior in Caring of the Dying Evaluation Questionnaire and psychometric properties test

Objective: To develop the Nurses' Practice Behavior in Caring of the Dying Evaluation Questionnaire and to test its psychometric properties. Methods: An item pool was development based on the CARES framework. The items were selected and modified by expert assessment, and then totally 210 oncology nurses were investigated with the questionnaire to test its psychometric properties. Through item analysis, factor analysis, internal consistency analysis, test-retest reliability to screen items of the questionnaire and conduct reliability and validity tests. Results: The final questionnaire consisted of 31 items. Exploratory factor analysis identified four principal factors which could explain 72.459% of the variance. Considering the items content, the final questionnaire was adjusted to 5 factors, namely pain management and comfort, airway management, restlessness and delirium management, emotional and spiritual support, and nurse self-care. The I-CVI was 0.80-1.00, and the S-CVI/Ave was 0.98. The Cronbach's α coefficient of the overall questionnaire was 0.945 and the test-retest reliability was 0.785. Conclusion: The Nurses' Practice Behavior in Caring of the Dying Evaluation Questionnaire has good validity and reliability, which can assess nurses' practice behaviors in providing the care for the dying.

Development of a humanistic care program for patients in ICU based on HAVENS model

Objective: To develop a humanistic care program for ICU patients based on HAVENS model. Methods: Based on the HAVENS humanistic care framework constructed in the previous study, we developed the initial humanistic care program for ICU patients by literature review. A total of twenty-one experts were invited to participate in a two-round consultation to finalize the program. Results: The humanistic care program consisted of 6 first-level items (family, activities, visits, environment, nursing and safety), 16 second-level items and 48 third-level items. The expert authority scores of two rounds were 0.791 and 0.813, respectively. The expert coordination coefficients in importance of two rounds were 0.279 and 0.311 (P<0.05). The expert coordination coefficient in operability of two rounds were 0.331 and 0.354 (P<0.05). Conclusion: The humanistic care program is developed with high reliability for ICU patients. It can be used to improve the medical experience of patients and their families.

Objective: To explore the behavioral intention of ECG monitor alarm management of nurses in neonatology department based on the theory of planned behavior, in order to provide clinical basis for exploring effective ECG monitor alarm management strategies in neonatology department. Methods: The interview outline was formulated according to the theoretical framework of planned behavior, a semi-structured interview was conducted in 13 nurses from neonatology department of a tertiary grade A hospital in Shantou from February to March 2022, and the data were analyzed by Colaizzi seven-step analysis method. Results: Based on the theory of planned behavior, 8 themes were extracted, including positive attitude, negative attitude, group pressure, leadership pressure, patient pressure, knowledge and skills, equipment performance, and workload. Conclusion: Most interviewees hold a positive attitude towards ECG monitoring alarm management, and the alarm management behavior was mainly affected by external pressure from group, leaders and the patients, and there were many obstacles in knowledge and skills, equipment performance and workload. Nursing managers might explore alarm management strategies and implementation plans, to promote the quality of neonatal alarm management.

Quality evaluation and content analysis of guidelines for padeatric patients with malignant tumor oral mucositis

Objective: To systematically review the quality of guidelines for the management of oral mucositis in children with malignant tumors, and to provide a basis for constructing an evidence-based practice plan for the management of oral mucositis in children with malignant tumors. Methods: Literature databases, websites of societies and guidelines in Chinese or English were searched from the establishment of the database to January 31, 2021. The Appraisal of Guidelines for Research and Evaluation II (AGREE II) was used to evaluate the quality of the guidelines and summarize the recommended contents. Results: Nine guidelines included, with the overall quality evaluation of Grade B. A total of 22 recommendations for the management of oral mucositis in children with malignant tumors were obtained, including the assessment, prevention, nursing care, photobiological regulation, cryotherapy and health education. Conclusion: The overall methodological quality of the guidelines for the management of oral mucositis in children with malignant tumors is relatively moderate, which needed to be improved in terms of participants domain and application domain. In the future, cultural adaptation of foreign guidelines is needed to guide clinical practice.

Practice evidence on assessment and management of sexual health in breast cancer patients

Objective: To search, evaluate and summarize the evidence of sexual health assessment and management of breast cancer patients. Methods: All guidelines, evidence summaries, recommended practices, expert opinions and systematic reviews on sexual health assessment and management of breast cancer patients in the databases and website were systematically searched, and the retrieval time was from the establishment of the database to February 2022. Two researchers independently screened the articles, used the Australia JBI evidence-based health care Center corresponding evaluation standards (2016 edition) and AGREE II to evaluate the quality of the articles, and combined with professional judgment to extract and summarize the evidence. Results: A total of 21 articles were included, including 5 guidelines, 2 expert opinions and 14 systematic reviews/Meta-analyses. Finally, 20 pieces of evidence were summarized from 5 aspects. Conclusion: The evidence for the evaluation and management of breast cancer patients can provide evidence-based practice for clinical professionals to evaluate and manage the sexual health of breast cancer patients in a scientific way, which is helpful to improve the quality of life of patients.

Status and its influencing factors of the general well-being of nurses in blood collection and supply institutions

Objective: To investigate the general well-being of nurses in blood collection and supply institutions. Methods: In March 2022, the convenient sampling method was adopted to conduct a questionnaire survey among nurses in blood collection and supply institutions in Beijing. The survey was conducted using the General Well-being Scale. A total of 308 valid questionnaires were collected. Results: The General Well-being score of nurses was 83.91±16.11, which was at a high level. Among the dimensions, the scores of happy or depressed mood were the highest, which was 5.39±1.16, and the lowest was life satisfaction and interest, which was 3.43±1.21. The multiple regression analysis showed that length of service, employment methods, education background were the influencing factors of nurses' general well-being. Conclusion: Nurse managers should perform relevant intervention measures in order to enhance the general well-being of nurses in blood collection and supply institutions in the future.

Analysis of the status quo and influencing factors of work family conflict among ICU nurses in tertiary grade A general hospitals

Objective: To understand the current situation of work family conflict among ICU nurses in tertiary grade A general hospitals and analyze its influencing factors. Methods: A cross-sectional survey was conducted among 252 ICU nurses in five tertiary grade A general hospitals in Yantai from February to April 2022 by using the general information questionnaire, work family conflict scale, perceived organizational support scale and psychological detachment scale. Results: The overall score of ICU nurses' work family conflict in tertiary grade A general hospitals was 59.45±15.37, the total score of perceived organizational support was 35.63±12.97, and the total score of psychological detachment was 9.31±4.16. Emotional support, instrumental support, psychological detachment, gender, age, children's situation and working years in ICU accounted for 43.2% of the variation of ICU nurses' work family conflict. Conclusion: The work family conflict of ICU nurses in tertiary grade A general hospitals is at a relatively high level, which is affected by many aspects, among which psychological detachment and perceived organizational support have a greater impact on the conflict. Nursing managers could reduce the level of work family conflict of ICU nurses by improving the psychological detachment and perceived organizational support of ICU nurses.

Objective: To identify the role development process and role responsibilities of clinical nurse specialists in pulmonary hypertension. Methods: Drawing on the participatory, evidence-based, patient-focus process (PEPPA) framework, we used a scoping review methodology to identify the responsibilities of pulmonary hypertension specialist nurses in multidisciplinary collaboration. Results: Totally 16 articles were identified based on the eligibility criteria, including two practice guidelines, one expert consensus, one standard, one data audit, three cross-sectional studies, one qualitative study, and seven reviews. We explained the role development of pulmonary hypertension specialist nurses from five aspects: identifying the target population, identifying the stakeholders, determining the need for a new model of care, identifying priority problems, and defining the nurse practitioner's role. Conclusion: Clinical nurse specialists play an important role in the multidisciplinary management of pulmonary hypertension. The advancement of pulmonary hypertension centers require the development of new nursing roles in a systematic way.

Application and effects of the reproduced surgical safety verification process with ophthalmic characteristics

Objective: The standardization and accuracy of operating drape methods are directly related to the precision of the surgical eye. This study intends to reproduce the key procedure of surgical safety verification to establish and perfect the safety management model of the operation room. Methods: Carry out root cause analysis for 32 non-standard operating drape happened from March 1 to March 31, 2019. According to the characteristics of ophthalmology, the improvement measures were made to reproduce the operation safety verification process. Results: After the improvement measures were performed for six months, the incidence of non-standard operating drape decreased from 3.32% to 0.40% with the difference being statistically significant (P<0.001). Conclusion: The reproduced procedure of surgical safety verification based on the characteristics of ophthalmic surgery could effectively reduce the incidence of non-standard operating drape and further ensure the surgical safety.

Objective: To develop indicators for day-surgery patients' safety management based on the Enterprise Risk Management-Integrated Framework (COSO-ERM framework). Methods: Drawing on the elements of COSO-ERM framework, we used the literature review, survey and Delphi methods to develop nursing safety management indictors for day-surgery patients. The weight of each indicator was determined by analytic hierarchy process. Results: We developed indicators for day-surgery patients' safety management, which included 3 first-level indicators, 12 second-level indicators and 41 third-level indicators. In this study, the return rates of the two rounds of Delphi were both 100%. The authority coefficients were 0.892 and 0.920 respectively, and the Kendall's coordination coefficients were 0.224 and 0.230 respectively (P<0.001). Conclusion: The safety management indicators for day-surgery patients developed in this study provides the standard for safe nursing care of day-surgery patients, and is worthy of clinical application and promotion.

Construction of "Internet +" home care safety management evaluation system based on Wuli-Shili-Renli Approach

Objective: To establish a set of "Internet +" home care safety management evaluation system, in order to provide a practical tool for evaluating the nursing safety management in the practice of "Internet +" home care service in medical institutions. Methods: Based on the results of the literature research and qualitative interviews, as well as the theoretical model of Wuli-Shili-Renli Approach, the "Internet +" home care safety management evaluation system was initially developed. Using two rounds of Delphi expert consultation and hierarchical analysis method, the weight coefficients were given to indicators at all levels. Results: The positive coefficients of the two rounds were 77.78% and 92.86% respectively. The expert authority coefficients were 0.8643 and 0.8577 respectively, and the coefficient of variation of each index ranged from 0 to 0.248. The established index weight consistency ratio was less than 0.1. The index system has been established, including 3 first-level indicators of physical support, affairs planning and human coordination, 14 second-level indicators such as safety rules and regulations, home service team, and 74 third-level indicators. Conclusion: Wuli-Shili-Renli Approach provides a scientific basis for the construction of the index system. Correspondence experts are highly motivated and authoritative, with reliable results and reasonable weight allocation. The constructed "Internet +" home care safety management evaluation index system is scientific.

Evaluation of the psychometric properties of the Chinese version of the Palliative Nursing Care Quality Scale

Objective: To translate the English version of the Palliative Nursing Care Quality Scale (PNCQS) into Chinese, and to preliminarily test its reliability and validity. Methods: According to the Brislin translation model, forward translation, backward translation, and culture adaptation were conducted. In January 2022, the reliability and validity of the Chinese version of PCPS were tested among 326 nurses in three tertiary grade A hospitals in Hubei province. Results: The Chinese version of PNCQS had 20 items. The Cronbach's α coefficient of the total scale was 0.913, and the 3-week test-retest reliability of the total scale was 0.893. The Scale-Content Validity Index was 0.981, and the Item-Content Validity Index ranged from 0.852 to 1.00. This was a single factor scale which explained 76.4% of the total variance, and all fitting coefficients were over 0.9. Conclusion: The Chinese version of the PNCQS has good reliability and validity under Chinese cultural background. It can be used as a reliable assessment tool for the nurses' palliative nursing care quality in China.

Construction of the evaluation system for breastfeeding nursing quality based on the model of three-dimensional quality structure

Objective: To establish the evaluation index system for breastfeeding nursing quality to provide basis for further improvement of pure breastfeeding rate in China. Methods: Based on the three-dimensional quality structure model including structure, process and outcome proposed by Donabedian, a American scholar, the evaluation system for breastfeeding nursing quality was built through literature retrieval, Delphi method and analytic hierarchy process. Results: The recovery rates of the two rounds of questionnaires were 93.94% and 100.00%, respectively, with the expert authority coefficient being 0.964. The Kendall harmony coefficients of the secondary and tertiary indicators of the second round were 0.147 and 0.152, respectively (P<0.01). The final evaluation index system of breastfeeding nursing quality included 3 primary indicators, 11 secondary indicators and 59 tertiary indicators. Conclusion: The evaluation index system of breastfeeding nursing quality constructed in this study has demonstrated high reliability, scientificity, comprehensiveness and practicability, which is conducive to better breastfeeding nursing quality.

Frailty impacts the health outcomes of stroke patients, and timely identification and evaluation of the progress of frailty is of great significance to stroke patients' physical and mental health. Through literature review, this paper summarized the definition and theoretical model relevant to frailty, the evaluation methods of stroke complicated with frailty, the relationship between stroke and frailty, as well as the interventional strategies available for stroke patients with frailty. The result of the review provides a reference for the identification and early intervention tailored for frailty in stroke patients.

Objective: To review literature on preventive interventions for post-ICU cognitive impairment, identify effective prevention and management measures, and summarize cognitive function evaluation indicators and tools. Methods: We conducted a literature search in English and Chinese databases to identify relevant studies from database inception to May 12, 2022. Two reviewers independently screened studies, collated and analyzed study findings. Results: 14 studies were included, of which 12 were randomized controlled trials and 2 were quasi-experimental studies. The preventive interventions included exercise rehabilitation, cognitive training, awakening promotion, virtual reality, psychosocial interventions and integrated nursing care. The outcome measures could be divided in 3 categories: the incidence of post-ICU cognitive impairment, overall cognitive function and cognitive function in specific domains. The evaluation tools included the cognitive function assessment scale and neuropsychological tests. Conclusion: Multi-component integrated nursing interventions are more effective than simple interventions. It is necessary to further explore the optimal intervention contents, timing and frequency, and standardize the evaluation indicators and tools to provide more high-quality evidence for post-ICU cognitive impairment prevention and management.

This paper summarizes the development history of genetic nursing in foreign countries, the definition and role of genetic nurses, as well as the training status of genetic nurses in the United States, Britain, Japan and developed countries, including the qualification requirements of genetic nurses, work roles and core abilities, and summarizes the enlightenment to the development of genetic specialty nursing in China. We hope to integrate the knowledge of genetics and genomics into our nursing work by referring to foreign models of genetic nursing, so as to promote the development of tumor prevention and eugenics in China.
