Current status of nursing quality management and nurses' needs in 22 medical institutions in Dongcheng district of Beijing

Objective: To investigate the current status of nursing quality management and the needs of clinical nurses in 22 medical institutions in Dongcheng district, Beijing. Methods: This cross-sectional study surveyed 22 nursing managers and 3221 clinical nurses from 22 medical institutions in Dongcheng district using a self-designed questionnaire. Results: Half of medical institutions investigated had adopted the 3-level nursing quality management organization system. 90.9% had set up their own nursing quality committees or teams, but only 59.1% had multi-department participation. More than 90% had established evaluation criteria for basic nursing and nursing management quality with only 63.6% having the evaluation criteria referring to specialist nursing quality. 72.7% had developed incentive systems for active reporting of adverse events, yet only 63.6% would analyze early warning cases. 81.8% of the institutions linked the results of supervision to nurses' performance. As to nurses' needs, more training in specific nursing skills (60.3%) and emergency technique (62.1%) were anticipated. 43.0% of the nurses welcomed such training once a quarter, and 89.9% of them prefered online teaching. Conclusion: Nursing managers need to further improve the evaluation criteria of specialist nursing practice quality and adverse event management. In order to promote the continuous improvement of nursing quality, more heed should be paid to the needs of clinical nurses to develop targeted training plans and supervision methods.

Construction and application of "internet+" health education model for lung cancer patients at day surgery ward

Objective: To establish an "internet+" health education management mode based on mobile APP in lung cancer patients at day surgery ward in perioperative period. Methods: A total of 2885 patients with lung cancer who received thoracoscopic radical resection from a tertiary grade A hospital in Hangzhou were enrolled by convenience sampling from February to September 2022. Among them, 1324 patients from February to May 2022 were selected as control group with receiving the health education in routine way, and 1561 patients from June to September 2022 were selected as observation group with receiving the health education provided by APP. Patients' satisfaction, awareness rate of health education, cancellation rate of same-day operation and hospitalization were compared. Results: Cancellation rate of same-day operation of observation group was lower than that of control group (P<0.05). There were no statistically significant difference in patients' satisfaction, awareness rate of health education and hospitalzation cancellation rate between the two groups. Conclusion: "Internet+" health education mode for lung cancer patients at day surgery ward can benefits patients by providing comprehensive accurate health education from pre-admission to post-discharge.

Objective: To develop a nursing-sensitive quality indicators system for perioperative patients with cardiac implantable electronic devices. Methods: Based on the Donabedian's three-dimensional model of "structure-process-result", the relevant indicators, their calculation formulas and data collection methods were preliminarily developed by literature review and group discussion. Then the nursing-sensitive quality indicators system of perioperative patients with cardiac implantable electronic devices was finally established through two rounds of expert consultation and expert interviews. Results: A nursing-sensitive quality indicators system of perioperative patients with cardiac implantable electronic device was established, including 3 first level indicators, 8 second level indicators and 20 third level indicators. The expert authority coefficients of the two rounds of correspondence were 0.916 and 0.921 respectively, and the Kendall harmony coefficients were from 0.190 to 0.223. Conclusion: The nursing-sensitive quality indicators system of perioperative patients with cardiac implantable electronic devices has strong specialty characteristics. Each indicator is scientific and practical, which is conducive to the improvement of perioperative nursing quality of patients with cardiac implantable electronic devices.

Objective: Design a remote management APP for mental health of pregnant women with fetal abnormalities during pregnancy termination, and evaluate its usability and application effect, in order to provide continuous psychological and social support and alleviate their negative emotions. Methods: Based on psychosocial intervention program for women undergoing termination of pregnancy for fetal abnormalities, the psychological, social, and information support in the intervention program was transformed into an APP. Formative and summative usability evaluations were conducted by using focus group discussion and System Usability Scale. Measure the levels of depression and social support of the intervention group and control group. Results: The main parts of the APP include stage assessment, psychological support, peer support, and information support. The summative usability evaluation is good, and the levels of depression and social support in the intervention group after using the APP were significantly improved than the control group (P<0.05). Conclusion: The APP is user centered, and realizes intelligent continuous care, which helps to effectively reduce pregnant women's depression, improve social support level, and promote the mental health management of pregnant women with fetal abnormalities during pregnancy termination.

Objective: To explore the application effect of home frailty management program based on the intelligent follow-up platform in elderly patients after surgery. Methods: The home frailty management program based on the intelligent follow-up platform in elderly patients after surgery was constructed by literature research and Delphi method. A total of 82 postoperative patients admitted to a tertiary grade A hospital in Tianjin from February to April 2022 were included by convenience sampling method, and 41 patients admitted from February 1 to March 15, 2022 were in the control group, 41 patients admitted from March 16 to April 30, 2022 were in the intervention group. Those in the control group adopted the conventional frailty management scheme, and those in the intervention group adopted the home frailty management program based on the intelligent follow-up platform, and the intervention period was 6 months. The differences in health behavior ability, frailty, hand grip strength, gait speed, physical function and quality of life were compared between the two groups. Results: The frailty score of the intervention group was lower than that of the control group (P<0.05). The health behavior ability score, grip strength, gait speed, physical function and quality of life of the intervention group were higher than those of the control group (P<0.05). Conclusion: The home frailty management program based on the intelligent follow-up platform has positive effects on improving health behavior ability, frailty, and quality of life of elderly patients after operation with great clinical application value.

This paper introduces the main content and progress of the domestic and foreign standards concerning Central Sterile Supply Department (CSSD), which mainly focuses on the five aspects of domestic hospital infection control standards, pharmaceutical industry standards and international CSSD standards including: quality management, product instructions, cleaning technology, packaging technology and sterilization technology. It is emphasized that in order to guarantee the safety and quality of the reprocessing of reusable medical devices, CSSD should take the initiative to better ensure their quality control and management according to the both fast updated and highly technical standards.

Objective: To understand the current situation and problems of the centralized management mode in Central Sterile Supply Department (CSSD), so as to provide reference for promoting centralized management. Methods: A questionnaire survey using Wenjuanxing was conducted in 2625 hospitals with CSSD from 28 provinces (including autonomous regions and municipalities) in 2021. Results: Among the 2621 CSSD that completed the effective questionnaire, 2132 (81.3%) had implemented the centralized management. Tertiary hospitals had the highest rate of centralized management (87.4%). Insufficient employees, equipment and area were the top three reasons restricting centralized management. The instrument with the lowest rate of centralized management was flexible endoscopy (27.1%), followed by oral instruments (69.3%). Conclusion: Changing concept, increasing investment of both manpower and equipments in CSSD and improving centralized management of instruments can further improve the implementation of centralized management.

Objective: To construct a training program for managers to cope with nurses' burnout, so as to provide reference for clinical management. Methods: A training program for managers to cope with burnout of nurses was discussed and preliminarily developed based on interview and literature review. A total of 15 experts were consulted by Delphi method, and the final training program was formed. Results: The training program for managers to cope with nurses' burnout included training objectives, training content, training time, training forms and training effect evaluation. The positive coefficients of the experts in the two rounds of consultation were 100%, and the authority coefficients were 0.850 and 0.837. The Kendall harmony coefficients of the items in the second round of consultation were 0.120-0.203, respectively, with statistical significance (P<0.05). Conclusion: The training program for managers to cope with nurses' burnout is scientific and practical, and can provide a scientific reference for the management of nurses in hospitals.

Status quo of out-of-hospital disinfection supply service in medical institutions

Objective: To investigate the use of out-of-hospital disinfection supply services in medical institutions, grasp the key links of disinfection supply management, and provide scientific basis and reference for the implementation of the basic standards and management norms in Chinese Central Sterile Supply Department. Methods: In 2021, a questionnaire survey was conducted among 506 medical institutions in China that use out-of-hospital disinfection supply services by means of target sampling. Results: Totally 49.8%(252) of the medical institutions chose the disinfection supply service provided by nearby hospitals, and 50.2% (254) of the medical institutions chose the disinfection supply service to choose the disinfection supply center. The highest proportion of sterilized instruments sent out of hospital was "conventional surgical instruments", 69.4% (351). The implementation rate of all requirements in the national health industry standard for out-of-hospital disinfection supply services in public medical institutions at all levels was 59.2% to 92.2%, and the difference was statistically significant among different levels of public medical institutions (P<0.05). Conclusion: The overall management level of out-of-hospital disinfection supply service in medical institutions needs to be improved. Quality management and management of full-time disinfection supply personnel should be strengthened, and a temporary storage room for collecting contaminated devices and a room for handing over and distributing sterilized items should be set up reasonably. It is recommended that when revising the hygiene industry standards for disinfection supply centers, medical institutions with a large volume of cleaning and disinfection prioritize establishing self-built disinfection supply centers to ensure the timely and secure reprocessing of medical instruments.

Objective: To understand the experience of hospitalized patients and their families towards patient safety participation, in order to provide references for enabling patient and family participation, promoting hospital patient harmonious and reducing preventable harm. Methods: A describing qualitative approach was used to conduct in-depth interviews with 11 hospitalized patients and 13 family members from four types departments from March to May, 2023. Thematic analysis was used to analyze the data. Results: Three main themes and six sub-themes were identified: high willingness to participate (endogenous motivation for health needs, exogenous motivation for doctor-patient cooperation), inadequate preparation for participation (individual differences in participation ability, deviation in understanding of participation roles), and poor support for participation (limited opportunities for participation, insufficient participation resources). Conclusion: Hospitalized patients and their families still face many difficulties in engaging in patient safety. The healthcare system could empower patients by stimulating endogenous motivation, expanding participation channels, and strengthening institutional support, to promote the development of a cultural atmosphere in which everyone engaging in patient safety. This would effectively promote patients' sense of fulfillment, security, and happiness during the medical treatment process.

Objective: To investigate the status and influencing factors of safety attitude among clinical nurses, so as to provide reference for optimizing patient safety management measures and ensuring patient safety. Methods: A total of 14,302 nursing staff from 40 hospitals across the country from July to August 2021 participated in the survey. The general information, safety attitude, professional value and coping adaptation ability were investigated. Multiple linear regression was used to analyze the influencing factors of safety attitude of nursing staff. Results: The safety attitude score of nursing staff was 122.63±20.27, which was medium level. The score of management perception dimension was the highest, and the score of teamwork was the lowest. The results of multi-factor analysis showed that occupational values, coping adaptation ability, whether to participate in nursing safety training, age, working years, education level, gender, department, position and hospital level were influencing factors of nursing staff safety attitude (P<0.05). Conclusion: The overall level of safety attitude of clinical nurses in China still needs to be improved, especially in the aspect of team cooperation. The safety attitude of nursing staff is affected by occupational values, coping adaptation ability, safety training, education level, position and other factors. Different factors should be paid attention to and targeted intervention measures should be implemented, so as to ensure the quality of nursing and patient safety.

Objective: To explore security management methods in the "Internet +" maternal and infant home care services,and provide reference for security development of "Internet+ nursing service". Methods: We proposed security management strategies from the perspective of establishing of an internet security management group, formulating procedural regulations, conducting personnel training, standardizing service processes, intelligent management of material resources. Results: Over a one-year period from March 2022 to March 2023, our hospital carried out a total of 16 projects of "Internet +" maternal and infant home care, providing a total of 2679 services. No adverse events occurred during the delivery of these services, resulting in a satisfaction rate of 99.25% and a repeat purchase rate of 29.43%. Conclusion: By analyzing security management difficulties of maternal and infant home care services from multiple dimensions and developing corresponding security management strategies, this study will help offer safe and high-quality nursing services to mothers and infants and provide a reference for medical institutions across China in order to conduct and improve the security management of "Internet +" home care services for mothers and infants.

Objective: To investigate the medication safety ability of nurses in secondary and above medical institutions in China, and analyze its influencing factors, so as to provide reference for improving nurses' medication safety ability. Methods: From April to May 2023, convenience sampling was used to conduct a cross-sectional survey of 5193 nurses in secondary and above medical institutions using the general information questionnaire and Medication Safety Competence Scale. Multiple linear regression was used to analyze the influencing factors of nurses' medication safety ability. Results: The total score of medication safety ability of nurses in secondary and above medical institutions was 161.21±20.78, and the item average score was 4.48±0.58. Among the six dimensions, the dimension of influencing factor management had the highest average score, and the dimension of improving medication safety had the lowest score. Multiple linear regression analysis showed that nursing experience, education background, hospital level, professional title, the number of patients cared for per day, and the department were the influencing factors of nurses' medication safety ability (P<0.05). Conclusion: The medication safety ability of nurses in secondary and above medical institutions is good. It is necessary to take appropriate medication safety ability training programs in combination with the personal characteristics of nurses, the level of hospital, the type of department and the medication environment, and pay attention to the improvement of medication safety issues to further improve the medication safety ability of nurses in China.

Objective: To construct and explore the effectiveness of a nurse-led information-based Enhanced Recovery After Surgery (ERAS) clinical pathway in surgical patients. Methods: Based on multidiscipline team collaboration and evidence, a nurse-led information-based ERAS clinical pathway for 22 diseases were constructed and applied to surgical patients from January 2023. Taking liver cancer and lung cancer patients as examples, the compliance rate of ERAS measures, average hospitalization days, and hospitalization costs were compared before and after the application of nurse-led information-based ERAS clinical pathway. Results: The compliance rate of the ERAS measures in liver cancer patients of experimental group was higher than that of the control group, and the average hospitalization days and hospitalization costs were lower than those of the control group, with statistically significant difference respectively (P<0.05). The compliance rate of enhanced rehabilitation measures in lung cancer patients of experimental group was higher than that in the control group, and hospitalization cost was lower, the differences were statistically significant respectively (P<0.05). Conclusion: The nurse-led information-based ERAS clinical pathway can promote the implementation of ERAS measures, promote the rehabilitation of patients, shorten the average hospitalization days, reduce hospitalization costs, and alleviate the current situation of limited medical resources.

Preventive effects of Low Anterior Resection Syndrome in rectal cancer patients based on the theory of Timing It Right

Objective: To explore the effects of prophylactic intervention based on the theory of "Timing It Right" in patients with rectal cancer on reducing the incidence and severity of postoperative Low Anterior Resection Syndrome (LARS). Methods: Totally 101 patients with low rectal cancer who were treated in the gastrointestinal surgery department of a tertiary grade A hospital from February 2020 to July 2021 were selected by convenience sampling method, and they were divided into 50 cases in the control group and 51 cases in the intervention group according to the order of admission time. The control group received routine care, and the intervention group received prophylactic intervention based on the "Timing It Right" on the basis of the intervention in the control group. The incidence, severity of LARS and quality of life were compared between the two groups at 1 month and 3 months after stoma reduction. Results: The LARS score and severity at 3 months after stoma reduction in the intervention group were significantly lower than that in the control group (P<0.05). The incidence (64.70%) was also significantly lower than that in the control group (88.00%) (P<0.05); and the quality of life in the intervention group was significantly higher than that in the control group at 1 month and 3 months after surgery (P<0.05). Conclusion: Prophylactic intervention based on the theory of "Timing It Right" can effectively reduce the incidence and severity of LARS in patients with low rectal cancer at 3 months after surgery, and improve patients' short-term quality of life after surgery.

Objective: To investigate the pain nursing management status in hospitals, so as to provide basis for promoting the further development of pain care management in China. Methods: Totally 145 hospitals were selected using convenience sampling method in June 2023, and a self-designed survey questionnaire was used to collect data. Results: Of the 145 hospitals, 138 (95.17%) hospitals established pain care management system, and 53 (36.55%) hospitals set up pain care outpatient clinic or pain healthcare joint outpatient clinic. There were 92 (63.45%), 92 (63.45%), and 128 (88.28%) hospitals conducting pain assessment for outpatient, emergency, and hospitalized patients, respectively. Thirteen provincial nursing societies where 91(62.76%) hospitals located established pain nursing professional committees, and 10 pain nursing professional committees carried out pain specialist nurse training. Totally 118 (81.38%) hospitals conducted pain care training throughout the hospital. Conclusion: The establishment of a national hospital pain care management system and the implementation of pain care for inpatients are continuously improving. However, there is still room for improvement in exploring a nurse-led outpatient working model, further improving the standardization of pain assessment, implementing the training and usage rules for pain specialist nurses, and strengthening training for nurses.

Objective: To construct the framework of core competence for Advanced Practice Nurse (APN) in China, and to provide a reference for APN training and evaluation. Methods: The element pool of core competence for APN was formed by job analysis and literature review. Delphi method was used for evaluation, screening and integration of the core competence elements. Results: After two rounds of Delphi consultations, the APN core competence framework was formed, including 6 dimensions (care, consultation, coordination, leadership, teaching and research) and 15 secondary indexes. The authority coefficient for both rounds of expert consultation was 0.89. The importance score of the dimensions ranged from 4.58 to 4.95, with a coefficient of variation score of 0.04 to 0.13. The importance score of the secondary indexes ranged from 4.52 to 5.00, with a coefficient of variation score of 0 to 0.23. Conclusion: The constructed framework of core competence for APN is scientific, reliable and practical, and it is expected to provide reference for the related research and practice of APN in China.

Objective: To establish a clinical pain assessment and management system and evaluate its application effect, so as to provide a reference for standardized clinical pain assessment and management. Methods: The pain assessment and management system was established from the aspects of clinical pain assessment methods, management tools, quality evaluation and the closed-loop information management platform of pain score. Nurses' performance in pain assessment, patient pain control effect, and patient satisfaction were compared before and after the application of system. Results: After the application of the system, the completeness and accuracy of pain assessment improved (P<0.05), the proportion of intravenous analgesics decreased (P<0.05), the timely treatment rate for explosive or severe pain increased (P<0.05), and patients' satisfaction with pain control effectiveness increased (P<0.05). Conclusion: This system can objectively reflect the condition and treatment outcome of related painful diseases, laying a solid foundation for improving the overall pain assessment and management level and clinical diagnosis and treatment effect, and is worthy of further promotion.

Objective: To summarize the experience of applying the nurse-led multi-disciplinary pre-hospital management model of patients receiving orthopedic surgeries, in order to improve hospital efficiency and ensure patient safety. Methods: The nurse-led multi-disciplinary management model was constructed and implemented clinically since 2019. Indicators like the case-mix index, the average length of stay before surgery and the average length of hospitalization were compared before and after the application of the model. Results: Ever since the implementation of the model, both the average preoperative hospitalization length and the average hospitalization length had decreased, and the differences were statistically significant (P<0.05). Besides, patients were very satisfied with the treatment safety, efficiency and medical examination. Fifty-two medical staff who had cared those orthopedic patients were 100.0% satisfied with the model. Conclusion: The nurse-led multi-disciplinary pre-hospital management model can ensure safety patients undergoing orthopedic surgery. It not only shortens the hospital stay, ensures patient safety, but also expands the field and value of nursing services.

Evidence summary for drug-related falls prevention and management in older adults

Objective: To retrieve and summarize the evidence on the prevention and management of drug-related falls in older adults and provide evidence-based basis for reducing the incidence of drug-related falls in older adults. Methods: The clinical decision, guidelines, expert consensus, evidence summary and systematic evaluation on the prevention and management of drug-related falls in older adults were systematically searched according to the evidence-based resource "6S" model from top to bottom. The search time limit was from the establishment of the database to December 1, 2023, quality evaluation and data extraction was carried to included literature, and the evidence was extracted and integrated according to the principle of evidence summary. Results: A total of 16 literature were included, including 1 clinical decision making, 3 guidelines, 5 expert consensus, 2 evidence summary and 5 systematic evaluations. Totally 21 pieces of evidence were formed in five dimensions: fall risk assessment, assessment of drug-related falls, preventive strategies, use management and health education of high-risk drugs in older adults. Conclusion: The evidence summary in this study can assist medical staff in developing personalized intervention programs based on the actual situation of patients, so as to reduce the incidence of drug-related falls in older adults.

Investigation of preoperative bowel preparation for urological surgery in 84 hospitals

Objective: To investigate the current status and adverse reactions of preoperative bowel preparation for urological surgery in China, and to provide reference for further optimization of intestinal preparation management in ERAS nursing. Methods: From November 1, 2022 to March 1, 2023, 84 hospitals in China were recruited by convenient sampling method. A self-designed questionnaire was used to investigate the current status of preoperative bowel preparation for urological surgery. Results: The highest frequency of Mechanical Bowel Preparation (MBP) used in urinary system operations were prostate puncture (81.0%), radical resection of bladder tumor (78.6%), radical resection of prostate cancer (70.2%), and the lowest frequency used was ureterolithiasis surgery (35.7%). For most urological surgeries, MBP was more frequently used in tertiary hospitals than in non-tertiary hospitals (P<0.05). The top three adverse reactions during bowel preparation were abdominal distension (31.1%), affected sleep (27.4%) and nausea (14.7%). Conclusion: MBP is still the main method for preoperative bowel preparation in urology surgery. Surgeons in hospitals tend to choose different preoperative bowel preparation methods for urology surgeries. In the future, under the premise of ensuring patient safety, the experience and research results of intestinal preparation before urology surgery should be further popularized to promote the enhanced recovery of patients.

Construction of pain management plan for postoperative critical ill patients during ICU transition

Objective: To construct a transitional pain management plan for postoperative critically ill patients, provide a reference for improving pain management in postoperative critically ill patients during the transition from ICU to specialized ward. Methods: Literature analysis and Delphi method were used to determine the transitional ICU pain management plan for postoperative critically ill patients, and hierarchical analysis was used to calculate the index weights. Results: A total of 18 experts were consulted twice. The coordination coefficient of the two rounds of inquiry was 0.276-0.309 (P<0.05). The constructed transitional pain management plan for postoperative critically ill patients includes 3 primary indicators including structure, process and outcome, 9 secondary and 36 tertiary indicators. Conclusion: The pain management plan for the postoperative ICU of critically ill patients constructed in this study is scientific and operable, and meets the actual needs of critically ill patients after surgical transfer. This protocol helps to improve pain outcomes relative to routine ICU transition procedures in postoperative critically ill patients. This study may provide references and guidelines for ICU and specialty unit medical staff to better develop clinical pain management practices for critical ill patients.

Objective: To translate the Breast Cancer Related Lymphedema Self-care Scale (BCRLSS) into Chinese and evaluate its psychometric characteristics in patients with Breast Cancer Related Lymphedema (BCRL). Methods: The BCRLSS was translated into Chinese using translation guidelines from the Beaton. A forward-backward translation and cultural adaption process between the original language (English) and the target language (Chinese) were performed. Psychometric properties of the final Chinese version of BCRLSS were verified in 244 participants with BCRL. Results: The CR value of most items were higher than 3.00, and the correlation coefficients were higher than 0.30. Validity analysis showed that the average content validity index of the scale was 0.99. The confirmative factor analysis showed that model yielded a reasonable fit:χ2/df=2.895, RMSEA=0.088. Cronbach's alpha coefficient for internal consistency was 0.853. Intraclass correlation coefficient for test-retest reliability of total score was 0.988. Conclusion: The Chinese version of BCRLSS has good validity and reliability, which can be used as an evaluation tool for self-care practice of patients with BCRL in China.

Objective: To translate the Clance Impostor Phenomenon Scale and test its reliability and validity among hospital nursing managers. Methods: The scale was translated, back-translated, and reviewed by the original author. The Chinese version of the instrument was established after cross-cultural adaptation pre-testing. The reliability and validity of the Chinese version scale were tested among 315 hospital nursing managers recruited by convenience sampling from October 2021 to February 2022. Results: The Chinese version of the Clance Impostor Phenomenon Scale had 8 items. The exploratory factor analysis identified 2 factors of fake and self-denial with a cumulative percentage of variance of 54.5%. The confirmatory factor analysis showed that the modified two-factor model fitted well (χ2/df=2.225, P=0.002, GFI=0.952, AGFI=0.909, CFI=0.959, TLI=0.940, RMSEA=0.077). The total Cronbach's α coefficient of the Chinese version scale was 0.837, and Cronbach's α coefficients for the two factors were 0.847 and 0.701, respectively. The test-retest reliability was 0.914. Conclusion: The Chinese version of the Clance Impostor Phenomenon Scale has good reliability and validity among hospital nursing managers and can be used for the assessment of Impostor Phenomenon.

Objective: To understand the cognitive experience of nurses on the influencing factors of High-Alert Medications (HAM) safety practice, and provide a reference for nurses to manage HAM more safely and effectively. Methods: Thirteen clinical nurses and nursing managers from different departments were selected by purposive sampling method. The clinical event analysis model was used as the theoretical framework, and the descriptive qualitative research method was used to conduct semi-structured interviews and key event interviews. Data were analyzed by content analysis method. Results: A descriptive model of the practice of preventing HAM errors, HAM safety: organizational, nursing and cooperation influences, was constructed. Three themes and 8 subthemes were extracted: organizational factors, nurse abilities and responsibilities, communication and cooperation. Conclusion: The medication safety of HAM in nurses is affected by organizational culture, their knowledge and ability, communication and cooperation between doctors, nurses and patients. This study provides a conceptual framework for the practice of safety management of HAM, and a theoretical basis for medical institutions to manage HAM safely and effectively.

Objective: To explore the obstacles to early screening behavior of high-risk population of breast cancer from the perspective of oncology nurses. Methods: Qualitative and descriptive research methods were adopted. A semi-structured interview was conducted on 14 oncology specialist nurses from september 2022 to March 2023. Data were transcribed and analyzed using qualitative content analysis. Results: The obstacles to early screening at high risk population of breast cancer were summarized into 5 themes: lack of self-early detection literacy of breast cancer; perceived lack of support; accessibility constraints for screening; insufficient screening popularity; inadequate support system. Conclusion: The early screening decision of high risk breast cancer population is affected by individual, interpersonal, organizational, community and policy factors. It is necessary to enhance the early detection literacy of breast cancer among high-risk population. At the same time, we should strengthen the support system based on "family strategy", and provide the targeted external support environment, actively implement the relevant policies to promote the popularization of early screening.

Objective: To analyze the implementation effects of evidence-based care bundles in reducing the incidence of feeding intolerance in patients with hemorrhagic stroke, which will provide ideas for the meticulous management of enteral nutrition patients with hemorrhagic stroke. Methods: Four evidence-based care bundle strategies were formed, including establishing a nurse-led multidisciplinary enteral nutrition management team, controlling feeding pathways and speeds based on gastrointestinal dynamics, providing individual enteral nutrition preparation, and using abdominal massage and hot compress to promote gastrointestinal motility. Patients with hemorrhagic stroke admitted to a tertiary hospital from December 2020 to June 2021 were selected as experimental group (n=69) and compared them with patients (n=70) admitted from April to October 2020, to analyze the changes in feeding intolerance and nutritional status. Results: The incidence of feeding intolerance in the intervention group was significantly lower than that in the control group, and the nutritional status were significantly better than that in the control group (all P<0.05). Conclusion: Care bundles should be actively promoted to reduce enteral feed intolerance for patients with hemorrhagic stroke, improving their nutrition status.
