WANG Yue, WU Xiaoying, YUAN Cui, JIN Yanhong, SUN Hong, LI Chunyan, LI Peiyao
Chinese Nursing Management. 2022, 22 (11):  1607-1611.  DOI: 10.3969/j.issn.1672-1756.2022.11.002
Abstract ( 773 )   PDF (2753KB) ( 164 )
Objective: To investigate the clinical practice status of the Clinical Practice Standards for Arterial Blood Gas Analysis published in 2017, and to provide reference for further standardizing clinical practice for arterial blood gas analysis in adults. Methods: From April 12 to May 31, 2022, a self-designed questionnaire was used to investigate the practice status of adult arterial blood gas analysis among medical staff from 134 medical institutions in China by convenience sampling. Results: A total of 500 valid questionnaires were collected, and 28.2% respondents preferred 2% chlorhexidine gluconate ethanol solution as skin disinfectant. In clinical arterial blood gas sampling, 90.4% of the preferred site were radial artery, and the needle entry angles were slightly different. Only 15.0% removed three times the dead space volume of mixture blood; 73.8% correctly mixed the specimens after blood collection; 76.2% could complete the test within 30 minutes. There were 8.6% of the respondents who suffered needle injuries due to arterial blood collection operation. Conclusion: There are differences in the disinfection methods of arterial blood gas analysis, puncture sites for blood collection, and needle entry angles among medical staff. The quality control of arterial blood gas sample collection, transportation, and preservation needs to be optimized. The training and examination related to arterial blood gas analysis are insufficient, and the awareness of occupational protection is weak. The success rate of arterial blood collection can be improved by strengthening the training of adult arterial blood gas analysis operation knowledge and skills, and the quality control of the collection process, to ensure the accuracy of test results.

Chinese Nursing Management. 2022, 22 (11):  1612-1617.  DOI: 10.3969/j.issn.1672-1756.2022.11.003
Abstract ( 749 )   PDF (2932KB) ( 46 )
Objective: To investigate the incidence of complications during peripheral arterial catheterization and analyze the influencing factors of puncture-site bleeding in ICU patients. Methods: Patients admitted to ICU in a tertiary grade A hospital in Beijing from January 2021 to July 2022 were selected as the research objects. A self-designed data collection table was used to record patients' basic information, catheterization, complications and infections. The influencing factors of puncture-site bleeding was analyzed by using Logistic regression. Results: A total of 1197 arterial catheters were observed in 1130 patients, 93.8% were radial artery catheterization, 5.3% were brachial artery catheterization, 82.6% were indwelt in operating room, and 40.4% indwelling time was more than 48 hours, 5.6% were removed unplanned due to complications. The common complications were puncture-site bleeding (15.3%), inaccurate waveform (9.9%), pain complaint (6.5%), catheter disrepair (6.2%), etc. The catheter specimens and skin swabs of 329 patients with indwelling for more than 48 hours were routinely collected for microbial examination. The infection rate of colonization was 2.4%. Multivariate analysis showed that the catheter indwelling time and catheterization site were the influencing factors of puncture-site bleeding. Conclusion: The incidence of common complications in ICU patients during indwelling arterial catheter is still high. It is necessary to strengthen the observation of local complications, reduce the poor function of catheter, pay attention to the patient's colonizing bacteria infection and subjective feelings during indwelling, and take targeted preventive measures early.

YANG Qin, LI Lianye, LI Fengting, CHI Wei
Chinese Nursing Management. 2022, 22 (11):  1757-1760.  DOI: 10.3969/j.issn.1672-1756.2022.11.031
Abstract ( 264 )   PDF (2692KB) ( 47 )
Objective: To review the length of neonatal urethral catheter insertion and provide a reference for clinical practice and related research. Methods: We followed the JBI scoping review methodology for this study. Websites and databases were searched to obtain literature on the length of neonatal urethral catheter insertion. Two researchers then screened the literature and extracted data independently using a standardized data extraction form. We performed descriptive analysis for the extracted data. Results: A total of 10 papers were included. "Urine outflow" was reported as a standard indicator for the length of catheter insertion in most literature; a few hospitals used a fixed length as the standard; some literature suggested that we should insert the catheter a further distance after the urine flows out. Conclusion: At present, there is no consensus on the length of neonatal urethral catheterization in the literature. Multi-center and large-sample studies are needed to establish the specification of neonatal urethral catheter insertion length.
