Application of multidisciplinary team based-home nutrition management in patient with maintenance hemodialysis

Abstract: Objective: To explore the efficacy of multidisciplinary home nutrition management for maintenance hemodialysis patient. Methods: A total of 67 patients who received hemodialysis in the Department of Nephrology from December 2019 to April 2020 in a hospital in Hangzhou were enrolled and randomly divided into intervention group (33 cases) and control group (34 cases). Those in the control group were with regular diet, while those in the experimental group were with home nutrition management based on multidisciplinary cooperation besides regular diet. The nutritional indicators were compared between the two groups before and after intervention. Results: After the intervention, the levels of hemoglobin and albumin in the experimental group were significantly higher than those in the control group (P<0.05), the rate of good nutrition in the experimental group was significantly higher and the rate of severe malnutrition was significantly lower than that in the control group (P<0.05). The levels of serum phosphorus and iPTH, weight change, the changes of gastrointestinal symptoms, dietary changes, muscle consumption score and total score, and the score of malnutrition and inflammation in the experimental group were significantly lower than those in the control group (P<0.05). Conclusion: Home nutrition management based on multidisciplinary cooperation has a significant effect on improving the nutritional status, calcium and phosphorus metabolism and microinflammation status of maintenance hemodialysis patient, and reducing the incidence of malnutrition.
