Objective: To construct an informatics competency measurement system for newly recruited nurses. Methods: Literature review and group discussion were adopted to form the item pool based on the Technology Informatics Guiding Educational Reform (TIGER) initiative. Later the informatics competency measurement system was established after 2 rounds of expert consultation. Results: The new information competency measurement system for newly recruited nurses including 3 first-level indicators, 13 second-level indicators, and 58 third-level indicators was set up after two rounds of expert consultation with the coefficient variation of all indicators ranging from 0 to 0.18 and the coordination coefficient W being 0.23 (χ2 = 441.20, P<0.001). Conclusion: The informatics competency measurement system for newly recruited nurses built in the study proves to be scientific and practical, which can provide references for evaluation and training of informatics competency for new nurses.

Development and usability evaluation of the symptom management module in the support APP for parents of children with leukemia

Objective: To develop the symptom management module in the support APP for parents of children with leukemia, and evaluate its usability. Methods: The parents and medical staff of children with leukemia were interviewed to understand the symptom management items suitable for parents to participate in, patients' need of support when participating in symptom management, and their expectation for functions of the module in the support APP. Later a knowledge base of images and texts pertaining to the symptom management module was constructed to set up the symptom management module in cooperation with software engineers to realize the corresponding functions, after which formative usability evaluation and summative usability evaluation were carried out for the module. Results: The knowledge base relevant to 41 physiological symptoms and 6 psychological symptoms was formed to establish the symptom management module of the support APP for parents of children with leukemia, offering not only active search and browsing of symptom management information, but also symptom management information recommendation based on self-reported symptom assessment results and symptom management information recommendation based on hybrid recommendation. The usability evaluation of the symptom management module demonstrated satisfying results. Conclusion: This study has developed the symptom management module in the support APP for parents of children with leukemia, whose functions meet well the needs of those parents. The module with good usability can provide support for parents to participate in the symptom management of their children.

Establishment and application of an early warning information system for inpatients with Venous Thromboembolism

Objective: To construct an information system for early warning of Venous Thromboembolism (VTE) in hospitalized patients, so as to improve medical staff?'s awareness and ability of prevention and treatment of VTE to offer timely intervention to high-risk patients. Methods: By setting up a multidisciplinary VTE prevention and treatment working group, the information-based VTE early warning system was built to connect medical and nursing electronic record systems, thus ensure automatic risk assessment and early warning, hierarchical prevention and treatment assisted decision-making, VTE quality control and other functions, which altogether guaranteed the steady implementation of clinical VTE prevention and treatment both efficiently and conveniently. Results: Since the application of the VTE early warning information system, VTE risk assessment had been given to all the hospitalized patients with the assessment rate reaching 100.00%. The prevention rates of basic, physical and drug preventive measures in high-risk patients increased significantly (P<0.001). Among the four dimensions of user experience evaluation of the information system, the dimension of early and timely warning scored the highest (9.16±2.79). Conclusion: The establishment and application of the VTE early warning information system for inpatients can booster effective and high-quality prevention and treatment of VTE in hospital to improve patients' outcomes and better safeguard their safety and upgrade healthcare.

Objective: To analyze the feasibility of the key information technique related to the safety of intravenous doses in pediatric patients to enhance safe administration of intravenous doses in children. Methods: The key information technique related to safe administration of intravenous drugs in children was developed. Convenience sampling was then used to include 30 nurses in a tertiary grade A hospital to offer 150 intravenous administration (dose extraction, dose calculation, dose setting) utilizing the new technique method and the traditional method, respectively to compare their accuracy. The usability of the new technique was evaluated by the Usefulness, Satisfaction, and Ease of Use Questionnaire (USE), and the cognitive load of nurses was evaluated by the National Aeronautics and Space Administration Task Load Index (NASA-TLX). Results: The accuracy of dose extraction, dose calculation and dose setting of the traditional method was 94.0%, 88.7%, 96.0%, respectively, while the accuracy of this new technique was 100.0% with the difference being statistically significant (P<0.05). The score of the USE questionnaire was (6.30±0.55) points in new technique method; Compared with the traditional method (45.39±15.42), the new technique method (31.94±7.87) scored lower on the NASA-TLX questionnaire (P<0.05). Conclusion: The application of the key information technique related to safe intravenous doses in pediatric patients can improve nurses’ competence in dealing with dosage calculation. Moreover, the system developed in this study show good usability and cause less cognitive load for nurses, which is conducive to the safety of intravenous administration in children.

Objective: To evaluate the effects of the mobile data application for pressure injuries in clinical nursing management of primary hospitals. Methods: Totally 60 inpatients from a hospital were selected and divided into experimental group and control group, with 30 subjects in each group. Subjects in the experimental group used the pressure injury mobile data application APP to help with the clinical management of the pressure injuries, while the control group carried out only routine pressure injury care. The accuracy of injury image recognition, the time required for evaluation, record description and the response time of consultation were compared. Results: Compared with the control group, the accuracy of injury image recognition were statistically improved (P<0.05), the time required for wound evaluation, record description and the response time of consultation were significantly reduced (P<0.01). Conclusion: The mobile data application based on the electronic medical record platform is an effective means to prevent and dynamically monitor pressure injuries. It can not only promote the management standardization and informatization but also improve the nursing management efficiency.

Investigation on the current management situation of foreign surgical instruments in 1166 public hospitals in China

Objective: To investigate the current management situation of the foreign surgical instruments. Methods: A survey regarding the management of foreign surgical instruments was conducted in 1166 public hospitals from 27 provinces in China. Results: The management model of foreign surgical instruments in 88.8% of the hospitals was four-level including the equipment department, Central Sterile Supply Department, operating room, and the subspecialty of operating room. The turnover rate of technicians in 34.7% of the hospitals was above 10% and 18.9% of the hospitals had no systematic training and assessment. About 54.5% of the hospitals did not know the information regarding the foreign surgical instrument before surgery, 9.3% of the hospitals did not delivery foreign surgical instruments to Central Sterile Supply Department on time, 83.5% of surgical foreign surgical instruments were sterilized urgently. About 21.6% of the hospitals failed to perform four checks on the quantity and completeness of foreign surgical instruments. Conclusion: A management system for foreign surgical instruments should be established and the regulations should be fully implemented; technicians management should be standardized; the principle of checking the items of foreign surgical instruments should be implemented, the time should be strictly controlled when the instruments are delivered to the Central Sterile Supple Department, and the quality of instrument processing before and after surgery should be improved to maintain the safety of surgical patients.

Objective: To explore the effects of a standardized nursing education for family members on on psychological status of patients and family members, and quality of life of patients undergoing robotic pancreaticoduodenectomy. Methods: A total of 60 cases of patients with pancreatic cancer undergoing robotic pancreaticoduodenectomy and their accompanying family members from Faculty of Hepato-Pancreato-Biliary Surgery of a tertiary grade A hospital in Beijing between December 2019 and December 2020 were enrolled in this study. These participants were evenly assigned to the experimental group or the control group according to the odevity of their admission order number. Patients of both groups received routine care. While the family members of patients in the control group and those in the experimental group were given traditional education and standardized education, respectively. The scores of anxiety and depression, quality of life, incidence of nursing adverse events and postoperative rehabilitation indexes of both groups were compared. Results: The anxiety and depression scores of patients and their family members in the study group were decreased significantly compared with those in the control group (P<0.05). The quality of life score of patients increased significantly in the experimental group compared with those in the control group (P<0.05). Conclusion: Anxiety and depression are common in patients undergoing pancreaticoduodenectomy and their family members. Standardized nursing education could effectively reduce the anxiety and depression of patients and their family members, and improve the quality of care, and improve the quality of life of patients. The standardized nursing education for patients' family members is worthy of promotion in clinical settings.

Application of whole-course nursing management mode based on efficiency medical background in patients with hip and knee replacement

Objective: To apply the whole-course nursing management mode to the inpatient management of hip and knee replacement patients, and explore its influence on medical efficiency index and hospitalization satisfaction guided by efficient medical treatment. Methods: The average hospitalization days and total hospitalization expenses of hip and knee arthroplasty patients who were not included in the whole process management mode (control group) from February to August, 2020 and included in the whole process management mode (experimental group) from September 2020 to March, 2021 were collected and statistically analyzed to compare the differences of efficiency indicators and the patient satisfaction of experimental group was investigated. Results: After application of whole-course nursing management mode, the average length of stay of patients with hip arthroplasty decreased from (7.69±2.32) days to (5.24±1.34) days (P<0.001), and the total cost of hospitalization decreased from (52436±6386) yuan to (47435±5383) yuan (P<0.001). The average hospitalization days of patients with knee arthroplasty were shortened from (8.68±2.92) days to (5.47±1.77) days (P<0.001), and the total hospitalization expenses were reduced from (45780±4012) yuan to (39796±4489) yuan (P<0.001). The satisfaction of hospitalized patients reached 100%. Conclusion: The whole process nursing management mode under efficiency medical treatment can significantly shorten the average length of stay, reduce the total cost of hospitalization, improve hospitalization satisfaction and promote medical efficiency.

Construction and evaluation of prediction model for post intensive care syndrome in family members of patients with hemorrhagic stroke

Objective: To analyze the incidence and risk factors of post intensive care syndrome in family members of patients with hemorrhagic stroke and to develop the risk prediction model and evalute its prediction effect. Methods: From August 2020 to August 2021, 230 family members of patients with hemorrhagic stroke from ICU of a tertiary grade A hospital in Shiyan city were selected as the research objects by convenience sampling method. A cross-sectional survey was conducted by using the general information questionnaire, Pittsburgh Sleep Quality Index, Fatigue Assessment Instrument, Hospital Anxiety and Depression Scale, and Impact of Event Scale-Revised. Chi-squared Automatic Interaction Detector (CHAID) decision tree model and Logistic regression model were established respectively, and the differences between the results of the two models were analyzed and compared. Results: Logistic regression analysis showed that gender, age, family income and APACHE II score were independent influencing factors of post intensive care syndrome in family members (P<0.05). CHAID decision tree model analysis showed that APACHE II score was the main factor affecting the occurrence of post intensive care syndrome in the family members, followed by ICU hospitalization time, family income and family member's age. There was no significant difference between Logistic regression model and decision tree model (P>0.05). Conclusion: Logistic regression and decision tree model were combined to screen the risk factors of post intensive care syndrome in family members of patients with hemorrhagic stroke earlier and more comprehensively, and to provide reference for clinical intervention.

Reliability and validity of the Chinese version of Family Inpatient Communication Survey in ICU patients' family members

Objective: To translate the Family Inpatient Communication Survey (FICS) into Chinese version, and to test its reliability and validity. Methods: The Chinese version of FICS was developed through the process of translation and back-translation; we conducted cross-cultural modification through expert consultation and pilot-test; 264 ICU patients' family members were investigated through convenience sampling for testing the reliability and validity of Chinese version of FICS. Results: The Chinese version of FICS contained 2 dimensions with a total of 30 items. The critical value of each item was 4.248-20.322, and the Pearson correlation coefficient between each item score and the total scale score was 0.547-0.798; The I-CVI was 0.857-1.000, and the S-CVI /Ave was 0.952. The Cronbach's alpha coefficient of the scale was 0.926, and the test-retest reliability was 0.941. Conclusion: The Chinese version of FICS has good reliability and validity, and it can be used to evaluate the quality of clinician-family communication for ICU patients' family members.

Influence of Pre-Pregnancy Body Mass Index and Gestational Weight Gain on the establishment of early breastfeeding behavior

Objective: To investigate the influence of Pre-Pregnancy Body Mass Index (PPBMI) and Gestational Weight Gain (GWG) on the establishment of early breastfeeding behavior, and to identify high-risk groups with difficulty in establishing early breastfeeding behavior. Methods: A total of 364 women were selected in a Nanjing maternity and child health care hospital from March 2020 to July 2020 by convenience sampling. The breastfeeding process at the first postpartum few days was observed. The breastfeeding process indicators (including latching and sucking, breastfeeding frequency, breastfeeding preparation time, and single breastfeeding duration) and rate of early breastfeeding were compared. Results: After adjusting confounding factors, compared with women with normal-weight and adequate GWG, maternal underweight increased the risk of latch difficulty (OR=2.025, P=0.044), and low-frequency feeding (OR=2.057, P=0.021). Overweight (OR=0.392, P=0.015) and excessive GWG (OR=0.538, P=0.021) reduced the rate of early exclusive breastfeeding. Conclusion: Women who were underweight or overweight before pregnancy and excessive GWG were at high risk for establishing early breastfeeding behavior. Therefore, it is necessary to comprehensively implement weight management and pregnancy weight gain counseling in women of childbearing age. At the same time, health care professionals should provide targeted breastfeeding support to high-risk groups to improve breastfeeding status.

Junior undergraduate nursing students' professional identity towards the social practice curriculum focused on nursing ideological and political education

Objective: To explore the effects of social practice curriculum focused on nursing ideological and political education in junior undergraduates. Methods: Convenience sampling was used and there were 52 junior undergraduate nursing students joined this curriculum according to their own willingness. The questionnaire of college students' speciality identity was used to compare their professional identity between the curriculum group and non- curriculum group before and after the curriculum. Results: There was no significant difference in professional identity between the curriculum group and non- curriculum group at baseline (P>0.05). The curriculum group showed greater improvement in professional identity than the non- curriculum group after this program, which had statistical significance (P<0.05). Conclusion: This new developed social practice curriculum focused on nursing ideological and political education could bring junior undergraduates to get familiar to the clinical settings and nurses and could improve their professional identity.

Construction of nursing quality evaluation indicator system in perioperative period of small intestine transplantation

Objective: To construct an evaluation indicator system for perioperative nursing quality of small intestine transplantation, and to provide a standard and quantifiable basis for monitoring and evaluation of perioperative nursing quality in small intestine transplantation. Methods: This study was conducted based on the frame work of Donabedian's three-dimensional "structure-process-outcome" quality model, using literature review, Delphi method and analytic hierarchy process to determine the content of the system, and the weight of each indicator. Results: The effective recovery rates of two rounds of expert consultation questionnaires were 100%. The authority coefficients were 0.88. The variation coefficients were 0-0.235 and 0-0.201. The Kendall harmony coefficients were 0.105 and 0.187 (P<0.001). The final evaluation indicator system for perioperative nursing quality in small intestine transplantation included 3 first-level indicators, 11 second-lever indicators and 66 third-level indicators. Conclusion: The evaluation indicator system for perioperative nursing quality in small intestine transplantation is scientific and reliable, which can provide references for the evaluation of perioperative nursing quality in small intestine transplantation.

Construction of a prediction model for postoperative surgical site infection of gastric cancer

Objective: To establish a predictive model based on Perioperative Hypothermia (PH) and clinicopathological features to predict the occurrence of Surgical Site Infection (SSI) in gastric cancer patients. Methods: The clinical data and anesthesia records of patients who underwent radical surgery in a hospital in Xinjiang from January 2020 to April 2021 and were pathologically confirmed as gastric cancer were collected for retrospective analysis. The Logistic regression was used to analyze the independent risk factors affecting the occurrence of SSI, and a nomogram prediction model for the occurrence of SSI was established. Results: A total of 355 patients were enrolled, PH occurred in 99 patients (27.9%) and SSI occurred in 66 patients (18.6%). The Logistic regression analysis results showed that age, diabetes mellitus, BMI, body temperature entering the operating room, heat preservation interruption time, intraoperative PH, operative modalities and operation time were independent influencing factors of SSI in gastric cancer patients (P<0.05), based on which the nomogram prediction model was established with its AUC and Brier score being 0.826 (95%CI: 0.770-0.883) and 1.14, respectively. In addition, the fitting degree of the calibration curve was good, and the decision curve showed that patients could benefit when the threshold was between 0.10 and 0.73. Conclusion: The nomogram model formed by combining intraoperative PH and clinical characteristics can more accurately predict the risk of postoperative SSI in gastric cancer patients.

Investigation and analysis on the status quo of clinical nurse exposure among clinical nurses and post-exposure management

Objective: To investigate the status quo of clinical nurses exposure to respiratory transmitted diseases and bloodborne infections among medical workers in various medical institutions in China and the post-exposure report rate. Methods: Convenience sampling was used to select 19,738 clinical nurses who met the inclusion criteria as the research subjects. A self-made questionnaire comprised of three parts: basic information, basic knowledge of occupational exposure, occupational exposure risk assessment was used for survey. Subjects could fill the questionnaire on wjx.com. Results: A total of 19,738 pieces of questionnaire were collected, all of which were valid. According to the results, 96.5% (19,047 cases) who understood the basics of occupational exposure and knew about standard precautions, and 73.6% (14,527 cases) who thought that standard precautions could be achieved at work. 56.1% (11,067 cases) of the clinical nurse had occupational exposure at work, and most of which occured to their hands and 65.3% (7225 cases) of whom chose to report after occupational exposure. Conclusion: The domestic occupational exposure risk among clinical nurses is high, and the protection against occupational exposure remains to be strengthened.

Objective: The aim of the study was to evaluate the feasibility and potential of the Global Trigger Tool (GTT) for identifying Adverse Nursing Events (ANEs) in hospital. Methods: By retrospective cross-sectional survey method and systematic sampling, a total of 600 adult patient records were selected from a tertiary grade A hospital from June 1, 2020 to May 31, 2021. Totally 14 nursing-related triggers in GTT were used to check the medical records, determine ANEs and conduct a severity level, and evaluate the advantages of GTT to increase ANEs detection rate. Results: Among the 600 medical records, the positive trigger rates of 14 triggers were 100.0%, and the total frequency of positive trigger was 237 times. The positive detection frequency was 41 times, and the Positive Predictive Value of triggers was 17.3%, ANEs incidence was 7.3%, GTT detection rate was 5.0%, the voluntary reporting rate was 1.5%. A total of 2.7% of all identified ANEs resulted in temporary harm (category E), 2.3% in temporary harm, requiring prolonged hospitalization (category F). Conclusion: The application of the GTT can effectively monitor the occurrence of ANEs in adult hospitalized patients, in order to facilitate the statistics and analysis of the occurrence of ANEs in medical institutions, the trigger detection with the GTT is a valuable addition for proactive analyses of high-risk processes. But the GTT needs to be further revised to be applicable to the monitoring of ANEs in various medical institutions.

Objective: To summarize the evidence of non-pharmacological management in patients with the Stroke-related Sleep Disorder, which can provide recommendations for clinical staff. Methods: We searched databases and websites to collect guidelines, clinical decisions, best practice recommendations, evidence summaries, systematic reviews, and expert consensus on non-pharmacological management for Stroke-related Sleep Disorders. Results: Finally, 4 guidelines, 1 clinical decision, 1 best practice recommendation, 2 expert consensus, and 22 systematic reviews were included. A total of 18 pieces of evidence were summarized from 6 aspects: team building, early screening, sleep health education, psychological and behavioral therapy, traditional Chinese medicine and physiotherapy, and Obstructive Sleep Apnea treatment. Conclusion: The evidence of non-pharmacological management for Stroke-related Sleep Disorders is highly targeted. In the process of clinical application, we should follow reasonable evaluation criteria and select evidence for intervention according to the actual situation of patients, which can reduce the adverse impact of Stroke-related Sleep Disorders on patients' life and prognosis.

Objective: To evaluate and integrate the best evidence on the prevention of constipation in cancer patients. Methods: We searched guideline websites, clinical decision-making system websites, evidence-based resources and literature database and professional association websites on constipation prevention in cancer patients. After methodological quality evaluation, the evidence was extracted and summarized based on the themes. Results: A total of 5 articles were included, and 34 items of evidence were finally formed through reading, extraction, and classification. A total of 4 themes finally emerged including assessment of risk factors, general preventive measures, special population preventive measures (patients using opioids), and special population preventive measures (elderly cancer patients). Conclusion: Medical staff should choose the best evidence to prevent constipation according to the individual situation of patients, so as to reduce the incidence of constipation in cancer patients, improve the quality of life of patients, and promote the transformation of evidence-based knowledge into practice.

Objective: To systematically evaluate the effects of card and boardgames on cognitive functions of older adults. Methods: Randomized Controlled Trials on effects of card and boardgames on cognition of older adults were retrieved from the databases from their establishment to 31 December 2021. Review Manager 5.3 software was used for Meta-analysis. Results: A total of 9 Randomized Controlled Trials were included. Compared with older patients in the control group, older adults in the experimental group had better global cognitive functions [MD=0.75, 95%CI (0.35, 1.15), P=0.0002] and digit span backward [MD=1.00, 95%CI (0.52, 1.49), P<0.0001]. Conclusion: Card and board games are effective in improving global cognitive functions and digit span backward in the older adults, but they cannotimprove overall digit span, digit span forward, logical memory and attention.

Objective: To evaluate and integrate evidences on the prevention and nursing of ICU children's Pressure Injury. Methods: We searched clinical decision-making websites, guidelines, professional association websites, evidence-based literature resources, and databases for guidelines, evidence summaries, best practices, systematic reviews and expert consensus on the prevention and nursing of Pressure Injury in critical care children. The JBI evidence appraisal and recommendation system was used to evaluate the quality of studies and the level of evidences. Results: A total of 11 papers were brought into this study, including 1 guideline, 5 evidence summaries, 2 systematic reviews and 3 Randomized Controlled Trials. Finally 33 pieces of best evidences were summarized, covering 5 aspects including risk assessment, medical device-related, position management, therapeutic nursing and surgical prevention. Conclusion: Nursing staff should choose the best evidence to carry out prevention and nursing of ICU children's Pressure Injuries according to the medical environment and the situation of children, in order to improve the standardization and effectiveness of the ICU children's Pressure Injuries management.

Objective: To explore the influence of cultural intelligence on nursing team cooperation, to provide a reference for clinical nursing managers to manage nurse team cooperation. Methods: Based on the Input-Process-Output model, a theoretical model of cultural intelligence-team conflict-nursing teamwork is established. The cultural intelligence level, team conflict, and team cooperation ability of 775 nurses were investigated by convenience sampling in five tertiary grade A hospitals. Results: Cultural intelligence had a direct effect on nursing teamwork, accounting for 52.81%. Cultural intelligence affected nursing teamwork through task conflict, and the mediating effect of task conflict accounted for 38.92%. Cultural intelligence affected nursing teamwork through relationship conflict, and the mediating effect of relationship conflict accounted for 6.33%. Conclusion: Nursing managers need to pay attention to and cultivate the cultural intelligence of nurses, and to improve the performance of nursing team cooperation.
