全病程智能平台在重症急性胰腺炎患者出院后管理中的应用

Construction and application of post-discharge management for Severe Acute Pancreatitis based on a whole-course intelligent platform

摘要: 目的:构建基于全病程智能平台的重症急性胰腺炎患者出院后管理方案,为其出院后管理提供参考。方法:构建基于全病程智能平台的重症急性胰腺炎患者出院后管理方案,组建管理团队并设置多类平台端口,涵盖十大核心功能。采用类实验性研究设计,将2023年1月—9月纳入管理的患者设为干预组,并选取2022年1月—9月出院的符合纳入和排除标准的患者作为对照组。通过平台实施管理,并统计分析方案的运行情况、应用效果及患者满意度。结果:平台累积服务重症急性胰腺炎患者234例,失联率为0;线上咨询实现08:00—22:00全时段响应,非工作时间咨询占比38.9%。与对照组相比,干预组的随访完成率、按时复诊率更高(P<0.05),再入院等待时间更短,并发症发生率更低(P<0.05);患者满意度评分为(9.52±0.59)分。结论:基于全病程智能平台的重症急性胰腺炎出院后管理方案可有效满足患者康复需求,提升管理质量与效率,提高患者满意度,对临床实践具有参考价值。

Abstract: Objective: To develop a post-discharge management model for patients with Severe Acute Pancreatitis (SAP) based on a whole-course intelligent platform and to provide a reference for its management. Methods: A post-discharge management model for SAP patients was constructed based on a whole-course intelligent platform. A management team was established, and multiple platform access points were set up, covering ten core functions. Using a quasi-experimental study design, patients enrolled from January to September 2023 were assigned as the intervention group, while eligible patients discharged from January to September 2022 were selected as the historical control group. Management was implemented through the platform, and the operation status, application effectiveness, and patient satisfaction of the model were statistically analyzed. Results: The platform cumulatively served 234 SAP patients, with a loss-to-follow-up rate of 0. Online consultations provided full-day coverage from 8:00 to 22:00, with 38.9% of consultations occurring during non-working hours. Compared to the non-enrolled group, the enrolled group demonstrated significantly higher rates of follow-up completion and on-time re-examination (P<0.05), a shorter average waiting time for readmission, and a lower incidence of complications during the home care period (P<0.05). The overall patient satisfaction score was 9.52±0.59. Conclusion: The SAP post-discharge management model based on the whole-course intelligent platform effectively meets patients' rehabilitation needs, improves management quality and efficiency, enhances patient satisfaction, and offers valuable insights for clinical practice.
