Experience of inpatients and their families participating in patient safety

Objective: To understand the experience of hospitalized patients and their families towards patient safety participation, in order to provide references for enabling patient and family participation, promoting hospital patient harmonious and reducing preventable harm. Methods: A describing qualitative approach was used to conduct in-depth interviews with 11 hospitalized patients and 13 family members from four types departments from March to May, 2023. Thematic analysis was used to analyze the data. Results: Three main themes and six sub-themes were identified: high willingness to participate (endogenous motivation for health needs, exogenous motivation for doctor-patient cooperation), inadequate preparation for participation (individual differences in participation ability, deviation in understanding of participation roles), and poor support for participation (limited opportunities for participation, insufficient participation resources). Conclusion: Hospitalized patients and their families still face many difficulties in engaging in patient safety. The healthcare system could empower patients by stimulating endogenous motivation, expanding participation channels, and strengthening institutional support, to promote the development of a cultural atmosphere in which everyone engaging in patient safety. This would effectively promote patients' sense of fulfillment, security, and happiness during the medical treatment process.

Current situation and influencing factors of safety attitude among clinical nurses

Objective: To investigate the status and influencing factors of safety attitude among clinical nurses, so as to provide reference for optimizing patient safety management measures and ensuring patient safety. Methods: A total of 14,302 nursing staff from 40 hospitals across the country from July to August 2021 participated in the survey. The general information, safety attitude, professional value and coping adaptation ability were investigated. Multiple linear regression was used to analyze the influencing factors of safety attitude of nursing staff. Results: The safety attitude score of nursing staff was 122.63±20.27, which was medium level. The score of management perception dimension was the highest, and the score of teamwork was the lowest. The results of multi-factor analysis showed that occupational values, coping adaptation ability, whether to participate in nursing safety training, age, working years, education level, gender, department, position and hospital level were influencing factors of nursing staff safety attitude (P<0.05). Conclusion: The overall level of safety attitude of clinical nurses in China still needs to be improved, especially in the aspect of team cooperation. The safety attitude of nursing staff is affected by occupational values, coping adaptation ability, safety training, education level, position and other factors. Different factors should be paid attention to and targeted intervention measures should be implemented, so as to ensure the quality of nursing and patient safety.

Objective: To investigate the medication safety ability of nurses in secondary and above medical institutions in China, and analyze its influencing factors, so as to provide reference for improving nurses' medication safety ability. Methods: From April to May 2023, convenience sampling was used to conduct a cross-sectional survey of 5193 nurses in secondary and above medical institutions using the general information questionnaire and Medication Safety Competence Scale. Multiple linear regression was used to analyze the influencing factors of nurses' medication safety ability. Results: The total score of medication safety ability of nurses in secondary and above medical institutions was 161.21±20.78, and the item average score was 4.48±0.58. Among the six dimensions, the dimension of influencing factor management had the highest average score, and the dimension of improving medication safety had the lowest score. Multiple linear regression analysis showed that nursing experience, education background, hospital level, professional title, the number of patients cared for per day, and the department were the influencing factors of nurses' medication safety ability (P<0.05). Conclusion: The medication safety ability of nurses in secondary and above medical institutions is good. It is necessary to take appropriate medication safety ability training programs in combination with the personal characteristics of nurses, the level of hospital, the type of department and the medication environment, and pay attention to the improvement of medication safety issues to further improve the medication safety ability of nurses in China.

Security management practices in the "Internet +" maternal and infant home care services

Objective: To explore security management methods in the "Internet +" maternal and infant home care services,and provide reference for security development of "Internet+ nursing service". Methods: We proposed security management strategies from the perspective of establishing of an internet security management group, formulating procedural regulations, conducting personnel training, standardizing service processes, intelligent management of material resources. Results: Over a one-year period from March 2022 to March 2023, our hospital carried out a total of 16 projects of "Internet +" maternal and infant home care, providing a total of 2679 services. No adverse events occurred during the delivery of these services, resulting in a satisfaction rate of 99.25% and a repeat purchase rate of 29.43%. Conclusion: By analyzing security management difficulties of maternal and infant home care services from multiple dimensions and developing corresponding security management strategies, this study will help offer safe and high-quality nursing services to mothers and infants and provide a reference for medical institutions across China in order to conduct and improve the security management of "Internet +" home care services for mothers and infants.

Construction of supportive care strategies checklist for cancer patients in the dying period

Objective: To formulate supportive care strategies checklist for cancer patients in the dying period, in order to provide practice guide for clinical nurses to develop end of life care. Methods: Guided by the Supportive Care Framework for cancer care, the supportive care strategies checklist was drafted through the literature review of the care of the dying cancer patients. We consulted 19 experts during May to August 2022 using the Delphi method, and further modified the checklist according to their scores and opinion on the indicators. Results: Two rounds of expert letter consultations were conducted. The final supportive care strategies checklist for cancer patients in the dying period consisted of 5 first-level indicators and 30 second-level indicators. The response rates of 2 rounds were both 100%; the judgment basis coefficient, familiarity coefficient and authority coefficient of expert consultation were 0.94, 0.86, and 0.90 respectively; the variable coefficient for the 2 rounds of consultations were 0-0.206 and 0-0.222, and the Kendall's W coefficients of concordance were 0.116 (P<0.001) and 0.075 (P=0.052). Conclusion: The supportive care strategies checklist for cancer patients in the dying period is reliable and practical, which can provide practice guide for clinical nurses to conduct scientific, standardized and convenient care for the dying cancer patients.

Survey of needs of the whole process management service in day surgery patients based on the Kano model

Objective: To investigate the needs of the whole process management service in day surgery patients. Methods: A questionnaire regarding the needs of the whole process management service was developed based on the theoretical framework and the evidence of previous literature. A questionnaire survey was conducted among 210 day surgery patients in a tertiary grade A hospital in Beijing from April to September 2022. Kano analysis method was used to identify the attributes of the whole process management service needs in day surgery patients. Results: Among the 27 items, 22 (81.5%) of Kano attributes were expected quality, mainly in the dimensions of disease-related information, surgical information, and postoperative continuous care needs; 2 (7.4%) attributes were must-by quality including the preoperative preparation and dietary guidance requirements; 2 (7.4%) attributes were attractive quality including online consultation and adverse drug reaction requirements; and 1(3.7%) attribute was indifferent quality including drug storage requirements. The results of the quadrant map showed that 21 requirements were located in the advantageous area, including 19 expected quality and 2 must-by quality, accounting for 77.8% of the total variance. A total of 6 items were located in the reserved area, accounting for 22.2% of the total variance. The number of the remaining two areas was 0. Conclusion: The whole process management needs in day surgery patients has diversified characteristics. Medical institutions should actively meet patients' expected and must-by quality, and provide and innovate services with attractive quality on this basis to improve patient satisfaction and improve the hospital's competition.

Construction of risk prediction model of Hospital-Acquired Pneumonia in patients with Traumatic Brain Injury based on machine learning algorithm

Objective: To analyze the risk factors of Hospital-Acquired Pneumonia in patients with Traumatic Brain Injury, and to construct a risk prediction model. Methods: The clinical data of 596 patients with Traumatic Brain Injury hospitalized in a tertiary grade A hospital in Guizhou province from January 1st, 2019 to July 31st, 2021 were analyzed retrospectively. Five machine learning algorithms were used to construct risk prediction models, including Logistics Regression, Naive Bayes, Support Vector Machine, Kashin Nearest Neighborg and Multi-layer Representation. The accuracy, recall, F1 value, AUC value were performed to evaluate and compare the model. Results: The incidence of Hospital-Acquired Pneumonia was 34.90%. Among the five models, Multi-layer Representation model had higher accuracy, recall, F1 value and AUC value. Conclusion: Multi-layer Representation model is effective and suitable for the early prediction of Hospital-Acquired Pneumonia in patients with Traumatic Brain Injury. It is expected to provide reference for diagnosis, treatment and prevention strategies for patients with Traumatic Brain Injury.

Application of nursing decision support system in whole process management for Incontinence Associated Dermatitis

Objective: To develop a nursing decision support system and explore its effects in the whole process management of Incontinence Associated Dermatitis (IAD). Methods: Using literature analysis, theoretical analysis, expert group discussion and empirical research, we developed a nursing decision support system for the whole process management of IAD. This decision support system was applied to 2559 nurses in 42 clinical departments from January to December 2021 from assessment, diagnosis, identification to pressure injury, planning, implementation, and evaluation. The IAD knowledge level of nurses, the implementation rate, and the incidence of IAD were compared before and after the use of this decision support system. Results: Compared with the pre-implementation of the system, the incidence of IAD decreased from 0.15% to 0.12% (P<0.05). Nurses' knowledge level of IAD-related knowledge increased from (75.32±0.19) to (87.41±0.10) (P<0.05). The implementation rate of nurses' measures of IAD prevention including etiology management, urine and stool collection, washing frequency, washing solution and use of substances and use of skin moisturizers were improved (P<0.05). Conclusion: The nursing decision support system can assist, supplement, guide, and manage clinical care and effective decision-making, improve the implementation of IAD prevention measures, reduce the incidence of IAD, promote the healing of IAD, and improve nursing quality and nurse-patient satisfaction.

Objective: To develop a questionnaire on the Knowledge, Attitude and Practice (KAP) questionnaire for malignant wound management for oncology nurses and test its reliability and validity. Methods: An initial questionnaire was developed by referring to relevant domestic and foreign literature, expert consultation and pre-investigations, and a formal investigation was then carried out among 687 oncology nurses in China, after which the reliability and validity of the questionnaire were tested in terms of the Cronbach's α coefficient, split-half reliability, test-retest reliability, content validity, exploratory factor analysis and confirmatory factor analysis. Results: The questionnaire covered 3 dimensions and 56 entries totally with the Cronbach's α coefficient being 0.954, the split-half reliability being 0.950, the test-retest reliability being 0.947, the content validity index (S-CVI) being 0.982, and the content validity index of each entry (I-CVI) being 0.875-1.000. Conclusion: This questionnaire can be used as the tool to evaluate the malignant wound management KAP for oncology nurses due to its good reliability and validity.

Objective: To construct and validate the risk prediction model of social maladjustment in preschool children with malignancies. Methods: From July to December 2021, 475 preschool children with malignancies and their primary caregivers were recruited. They were randomly assigned to training set (n=332) and test set (n=143) in a ratio of 7?:?3. The training set was screened for socially maladjusted children by the Social Adjustment Rating Scale, and Logistic regression and XGBoost methods were used to identify predictors and construct the model. The Receiver Operating Characteristic (ROC) curve, accuracy, F1-Score, and Kappa were used to evaluate the consistency and prediction accuracy of the model. Results: The incidence of the risk of social maladjustment in preschool children with malignancies was 44.0%. The XGBoost prediction model outputs the importance of maladjustment risk factors as general family functioning, age, the average length of internet use, region, infant attachment, stage of illness, family coping style, and whether it relapsed. The area under ROC curve of the model was 0.839, the accuracy was 83.9%, the Kappa was 0.826, and the F1-Score was 0.857. Conclusion: The prediction model based on XGBoost has good prediction accuracy and consistency, which can be used to screen individual adaptation maladjustment risk.

Objective: To retrieve, evaluate and integrate relevant evidence on the management of compliance with exercise rehabilitation in stroke patients, so as to provide references for improving exercise rehabilitation compliance of stroke patients. Methods: The relevant literature on the management of exercise rehabilitation compliance of stroke patients in domestic and foreign databases was systematically searched, and the time for the retrieval was from inception of databases until August 2022. The researcher independently evaluated the quality of literature, and extracted and integrated the best evidence. Results: A total of 13 studies were included, including 6 guidelines, 1 expert consensus, 2 practice recommendations, 1 evidence summary, 2 systematic reviews and 1 original research with randomized controlled trial design. A total of 25 pieces from 5 aspects of evidence were summarized, including the general policy, exercise plan, health education, self-management and feedback monitoring. Conclusion: Medical staff should translate the evidence into the adherence management program of stroke patients with exercise rehabilitation in accordance with application practice and patients' willingness, to improve the compliance, completion rate and participation of stroke rehabilitation, and finally improve the clinical symptoms and prognosis of patients, and promote the best rehabilitation.

Narrative nursing training in clinical nurses: a scoping review

Objective: A comprehensive search was conducted on the relevant studies of narrative nursing training in clinical nurses at home and abroad aiming to understand the training status and training effect evaluation. Methods: The Arksey and O'Malley scope review method was used to systematically search CNKI, CBM, WanFang Data, VIP, PubMed, Cochrane Library, Embase. The data related to narrative nursing training were extracted by two researchers independently, and the extracted results were sorted out by descriptive analysis and thematic analysis. Results: A total of 15 studies were included. The results suggested that nurse team was the main target of the training, followed by interdisciplinary team. The duration of the training varied greatly with the majority was from 1 to 6 months. The standards of trainers are not unified and most of them have psychology and education background. Nursing theories and practice were equally important in the training. The effect evaluation method included quantitative evaluation and qualitative description, focusing on the assessment of nurses' narrative ability, humanistic quality, stress level and mastery of specialized knowledge. Conclusion: The models and methods of narrative nursing training are diverse, and there is no unified standard for training duration and trainers. In the future, characteristics of different specialties can be combined and interdisciplinary resources should be made use of to build a narrative nursing team training model.

Scoping review of researches on accurate clinical indicators for nursing diagnoses

Objective: To summarize the content of researches on accurate clinical indicators for nursing diagnoses at home and abroad, to provide reference for nurses to make nursing diagnoses as early and quickly as possible and further prompt the advancement and application of nursing diagnoses with Chinese characteristics. Methods: The Chinese and English databases were systematically searched from the establishment time to January 15, 2022, to extract relevant information and analyze relevant topics and research methods of researches on accurate clinical indicators for nursing diagnoses. Results: Finally 86 articles were included, which were mainly cross-sectional studies adopting content validation and clinical validation as the research methods. Nursing diagnoses related to respiratory diseases were shown to be the hotspot in the study of clinical indicators for nursing diagnoses. Conclusion: Researchers abroad have been quite into the study of accurate clinical indicators for nursing diagnoses involving various fields and categories, which can benefit nursing decision-making to some extent. However, few such researches are seen in China. Thus, it is suggested that more targeted researches on clinical indicators for nursing diagnoses be carried out to develop accurate indicators for nursing diagnoses in accordance with the specific clinical conditions in our country, so as to assist clinical nurses in making diagnoses decisions.

Construction of evaluation index system for midwifery specialist nurses training effect

Objective: To establish an evaluation index system for midwifery specialist nurses training effect, and to provide reference for midwifery specialist nurses training. Methods: Taking the Kirkpatrick's Model as the theoretical framework, the literature review method, Delphi expert consulting method and analytic hierarchy method were used to screen the indicators and determine the weight of the indicators. Results: The effective questionnaire return rate of the two rounds of consultation were 100%. The authority coefficients were 0.845 and 0.859 respectively, and the consulting coordination coefficient Kendall's W values were 0.295 and 0.400 respectively. The training effect evaluation index system included 4 primary indicators, 14 secondary indicators and 72 tertiary indicators. Conclusion: The evaluation index system is reliable and practical, which can provide reference for evaluating the training effect of midwifery specialist nurses.

Construction of a training program for managers to cope with burnout of nurses

Objective: To construct a training program for managers to cope with nurses' burnout, so as to provide reference for clinical management. Methods: A training program for managers to cope with burnout of nurses was discussed and preliminarily developed based on interview and literature review. A total of 15 experts were consulted by Delphi method, and the final training program was formed. Results: The training program for managers to cope with nurses' burnout included training objectives, training content, training time, training forms and training effect evaluation. The positive coefficients of the experts in the two rounds of consultation were 100%, and the authority coefficients were 0.850 and 0.837. The Kendall harmony coefficients of the items in the second round of consultation were 0.120-0.203, respectively, with statistical significance (P<0.05). Conclusion: The training program for managers to cope with nurses' burnout is scientific and practical, and can provide a scientific reference for the management of nurses in hospitals.

Influence of Satir model therapy on stress-coping styles, psychological resilience and self-efficacy of re-employed nurses

Objective: To analyze the influence of Satir model therapy on stress-coping styles, psychological resilience and self-efficacy of re-employed nurses. Methods: The convenience sampling method was used to enroll 72 re-employed nurses in a tertiary grade A hospital as the research objects who were randomly divided into the observation group (n=36) and the control group (n=36). The control group received conventional humanistic care including heart-to-heart talk, group discussion and psychological lectures, while the observation group received Satir model therapy as intervention. The effects were compared between the two groups by means of the Chinese version of the Nurses Stressors Scale (NSS), the Simplified Coping Style Questionnaire (SCSQ), the Connor-Davidson Resilience Scale (CD-RISC) and the General Self-efficacy Scale (GSES) before and after intervention. Results: After intervention, the scores of the observation group were significantly lower than those in the control group in nursing profession and work, management and interpersonal relationship (P<0.05). The scores of positive coping, psychological resilience (both the total score and each entry score) and self-efficacy of the observation group were significantly higher than the control group (P<0.05). Conclusion: Satir model therapy can effectively relieve the work pressure of re-employed nurses and improve their coping styles and psychological resilience to enhance their sense of self-efficacy.

Objective: To investigate the occurrence of symptom clusters in breast cancer patients living at home after chemotherapy and to analyze the influencing factors. Methods: A total of 353 breast cancer patients who discharged after chemotherapy from eight hospitals in Hunan province were investigated. The M.D. Anderson Symptom Inventory was used to investigate the occurrence of symptoms of breast cancer patients. Results: There were two symptom clusters in breast cancer chemotherapy patients living at home, namely perceptual symptom cluster and digestive-sleep symptom cluster. The results of multiple linear regression analysis showed that age, satisfaction of care needs, and discharge with medication were the factors affecting the symptom clusters. Conclusion: Breast cancer patients suffer from a variety of symptoms at home after chemotherapy, and the symptoms affect each other and exist in the form of symptom clusters. In clinical nursing work, the management and intervention of patients' symptom clusters should be strengthened, so as to improve patients' self-management ability and quality of life.

Objective: To construct transitional care scheme for patients with esophageal cancer and analyze its clinical application effect. Methods: The scheme was constructed under the guidance of supportive care theory, by using literature review, group discussion and Delphi method. A total of 60 patients with esophageal cancer who underwent surgical treatment in the Department of Thoracic Surgery of a tertiary grade A hospital from January to December 2019 were selected as the control group, received routine nursing care. From January to December 2020, 60 patients were selected as the intervention group and received transitional care based on WeChat mini program. Two groups of patients were evaluated at four time points: 1 day before discharge, 1 month, 3 months and 6 months after surgery. The Specific Support and Care Needs Scale for Patients with Esophageal Cancer (SSCNS-E), Family Resilience Scale Chinese version (FRAS-C), and Esophageal Cancer Quality of Life Scale (QLQ-OES 18) were used as tools. Results: There were statistically significant differences in the total score of SSCNS-E and FRAS-C between the two groups of esophageal cancer patients in terms of group, time, and interaction effects (P<0.05). There were statistically significant differences in the group and time effects of QLQ-OES 18 scores (P<0.05). Conclusion: The postoperative transitional care scheme for esophageal cancer patients constructed in this study is reasonable and feasible, and conducive to the rehabilitation of patients.

Objective: To develop an information platform for nursing consultation, and explore its application effect. Methods: Based on the establishment of a multi-disciplinary team and the nursing consultation process, the nursing consultation platform was constructed by the Nursing Department and the Information Department together. The platform was later applied to practice in the whole hospital to collect and analyze data including the number of consultations, completion time of each consultation and its clinical outcome as well as patient satisfaction. Results: According to the nursing consultation platform, problems requiring nursing consultation referred to 14 professional categories in total, of which about 90% had been settled. It took 2 minutes only to complete a consultation application. The average time for experts to complete a consultation was 4.5 hours. From 2019 to 2021, the patient satisfaction scores were respectively 98.34%, 99.50% and 99.55%, and the application departments' satisfaction of consultation experts were 95.35%, 96.10% and 96.24%. Conclusion: The development and application of the nursing consultation platform has effectively improved the clinical outcome of patients and the quality of specialized nursing to promote the development of specialist care.

Objective: To develop a smart elderly care management platform based on nursing procedure. Methods: Based on the nursing program, the intelligent decision-making process and the knowledge base of elderly care were formed according to the Nursing Grading and Service Standards of Elderly Care Institutions. The degree of agreement between the system assessment results and the nurse assessment results in terms of the accuracy of nursing grading and the compliance rate of nursing plans was compared. Results: A total of 81 elderly people were evaluated systematically. The compliance rate of the intelligent classification of the smart elderly care management platform was 100.0%, and the compliance rate of the decision-making nursing plan was 98.7%. Conclusion: The smart elderly care management platform based on nursing procedure can meet the nursing grading and formulation of nursing plans for the elderly in nursing homes. It can provide a reliable basis for nursing implementation.

Objective: To explore the effect of the "Internet+" health coaching technology on patients' upper limb function, self-management ability and quality of life after breast cancer surgery. Methods: Random sampling method was adopted to enroll 112 patients undergoing breast cancer surgery from July to December 2021 in a tertiary hospital in Tianjin. The subjects were divided into the trial group and the control group located on different floors through a random digit table. The control group was given routine care, while the trial group received the "Internet+" health coaching technology on the basis of routine care. Patients' upper arm function, self-management and life quality before and after intervention were compared between the two groups employing the Disabilities of the Arm, Shoulder and Hand Questionnaire (DASH), the self-management ability questionnaire and the Functional Assessment of Cancer Therapy-Breast (FACT-B). Results: After the intervention, the DASH score of the trial group was lower than that of the control group, the scores of the self-management ability and FACT-B in the trial group were higher than that of the control group (P<0.05). Conclusion: The "Internet+" health coaching technology can promote the recovery of patients' upper limb function, improve their self-management ability and life quality after breast cancer surgery.

Objective: To explore the application effect of home frailty management program based on the intelligent follow-up platform in elderly patients after surgery. Methods: The home frailty management program based on the intelligent follow-up platform in elderly patients after surgery was constructed by literature research and Delphi method. A total of 82 postoperative patients admitted to a tertiary grade A hospital in Tianjin from February to April 2022 were included by convenience sampling method, and 41 patients admitted from February 1 to March 15, 2022 were in the control group, 41 patients admitted from March 16 to April 30, 2022 were in the intervention group. Those in the control group adopted the conventional frailty management scheme, and those in the intervention group adopted the home frailty management program based on the intelligent follow-up platform, and the intervention period was 6 months. The differences in health behavior ability, frailty, hand grip strength, gait speed, physical function and quality of life were compared between the two groups. Results: The frailty score of the intervention group was lower than that of the control group (P<0.05). The health behavior ability score, grip strength, gait speed, physical function and quality of life of the intervention group were higher than those of the control group (P<0.05). Conclusion: The home frailty management program based on the intelligent follow-up platform has positive effects on improving health behavior ability, frailty, and quality of life of elderly patients after operation with great clinical application value.

Objective: To develop a nursing-sensitive quality indicators system for perioperative patients with cardiac implantable electronic devices. Methods: Based on the Donabedian's three-dimensional model of "structure-process-result", the relevant indicators, their calculation formulas and data collection methods were preliminarily developed by literature review and group discussion. Then the nursing-sensitive quality indicators system of perioperative patients with cardiac implantable electronic devices was finally established through two rounds of expert consultation and expert interviews. Results: A nursing-sensitive quality indicators system of perioperative patients with cardiac implantable electronic device was established, including 3 first level indicators, 8 second level indicators and 20 third level indicators. The expert authority coefficients of the two rounds of correspondence were 0.916 and 0.921 respectively, and the Kendall harmony coefficients were from 0.190 to 0.223. Conclusion: The nursing-sensitive quality indicators system of perioperative patients with cardiac implantable electronic devices has strong specialty characteristics. Each indicator is scientific and practical, which is conducive to the improvement of perioperative nursing quality of patients with cardiac implantable electronic devices.

Laryngeal cancer is one of the common malignant tumors in head and neck. The main treatment method for middle-advanced laryngeal cancer is surgery. Olfactory disorder is a common postoperative complication with high incidence and serious impact on patients' quality of life. This article reviews the causes of olfactory disorder, olfactory assessment and rehabilitation training methods in patients with laryngeal cancer after surgery. The objective of this article is to improve the awareness and attention of physicians to olfactory function, and provide reference for clinical olfactory rehabilitation. It is hoped that the olfactory function and quality of life of laryngeal cancer patients in China will be improved.
