Implementation and effect evaluation of continuing health education for patients with enterostomy after discharge

Objective: To provide follow-up health education to patients with enterostomy after discharge from hospital and to verify its effectiveness. Methods: A total of 215 patients who underwent enterostomy surgery in 17 hospitals in Beijing were selected for the study by convenience sampling, and the health education intervention based on WeChat mini-program was implemented in the 1st, 3rd, 7th, 11th and 23rd weeks after discharge, and the knowledge score of stoma self-care for patients with enterostomy and ostomy adjustment inventory were compared before and after continuing health education. Results: The knowledge score of stoma self-care for patients with enterostomy after the intervention was 17.77±1.81, which was higher than that before the intervention (14.44±7.02), the difference was significant (P<0.001), and the score of ostomy adjustment inventory after the intervention was 45.50±7.97, higher than the pre-intervention score (40.70±7.60), the difference was significant (P<0.001). Conclusion: WeChat mini-program based post-discharge health education for patients with enterostomy can effectively improve patients' self-care knowledge of enterostomy and their psychosocial adaptation. It has great application effect and is worthy of clinical promotion.

Improvement and implementation of an action-research-based care protocol for peristomal irritant dermatitis

Objective: To improve the care protocol for peristomal irritant dermatitis in patients with urostomies, and evaluate its implementation effect. Methods: Taking action research method as the theoretical framework, aiming at the problems in urostomy nursing, and guided by the four steps of planning, action, observation and reflection, the nursing protocol of peristomal irritant dermatitis was formed. The occurrence and wound healing of peristomal irritant dermatitis before and after implementation and the effective replacement of the ostomy bag were compared. Results: After the implementation of the 2 rounds of circulation, the incidence of peristomal irritant dermatitis decreased from 46.67% to 13.51%, the onset time was delayed from 2.64±0.74 days to 4.60±0.55 days, the healing rate of irritant dermatitis was increased from 50.00% to 100.00%, the healing time was shortened from 8.57±0.53 days to 6.60±0.55 days, and the effective replacement rate of ostomy bag was increased from 83.76% to 96.03% (all P<0.05). Conclusion: The implementation of the urostomies care protocol based on action research is scientific, the content is reasonable and the operability is robust, thereby reducing the incidence of peristomal irritant dermatitis and improving the healing rate of dermatitis.

Bacteriostasis and healing effects of Negative Pressure Wound Therapy combined with Topical Oxygen Therapy for chronic wounds

Objective: To analyze the bacteriostasis and healing effects of Negative Pressure Wound Therapy (NPWT) combined with Topical Oxygen Therapy (TOT) for chronic wounds, and the relationship between them. Methods: Totally 96 patients with chronic wounds were randomly divided into the intervention group and the control group, with 48 patients in each one. The intervention group was treated NPWT combined with TOT for 14 days after debridement, while the control group was treated with NPWT for 14 days after debridement, after that followed by standard Moist Wound Therapy 3 times a week, and followed up to healing or 5 months. Efficacy indicators: the change of bacterial positive rate and bacteriostatic rate of the two groups before and 14 days after the intervention, and the wound healing rate and healing time of the two groups during the follow-up period. Results: The bacterial positive rate of the intervention group on the 14th day of intervention was significantly lower than that of the control group (14.58% vs 54.17%, P<0.05), and the bacteriostatic rate of the intervention group on the coexistence of single and two bacteria were significantly higher than that of the control group (73.08% vs 11.11%, 100.00% vs 66.67%, all P<0.05). The wound healing rate in the intervention group was higher than that in the control group (100.00% vs 89.13%, P<0.05), healing time shortened by 20.84 days (P<0.05). Pearson correlation analysis showed that there was a positive correlation between the bacteriostasis rate and wound healing in the intervention group (r=0.480, P<0.05), while there was no correlation between the bacteriostasis rate and wound healing in the control group (r=0.083, P>0.05). Conclusion: NPWT combined with TOT can enhance the bacteriostasis and optimize the healing effects. The bacteriostasis can promote the healing of chronic wounds, and can provide new ideas and methods for the nursing and management of chronic wounds.

This paper reviews the related theories and modes of chronic wounds management, so as to provide a basis for the development of applicable chronic wounds management, help rapid wounds healing, reduce the economic burden of patients' family and medical system, and improve the quality of life of patients with chronic wounds.

Construction of evaluation index system for home care competence of wound ostomy specialist nurses

Objective: To construct the evaluation index system of home care competence of wound ostomy specialist nurses. Methods: Based on the iceberg theory, the first draft of the evaluation index system for home care competence of wound ostomy specialist nurses was developed through literature research and semi-structured interview. Under the guidance of the Delphi method, the research group conducted a letter consultation with 20 experts from 11 provinces in China from December 2021 to January 2022. The analytic hierarchy process was used to calculate the weight of the index system. Results: The effective recoveries of the two rounds of correspondence questionnaires were 100%. The expert authority coefficient was 0.95. After 2 rounds of consultation, the evaluation index system of home care competence of wound ostomy specialist nurses was formed, including 3 first-level indicators (knowledge and skills, comprehensive ability and personal traits), 12 second-level indicators and 47 third-level indicators. In the second round of consultation, The Kendall harmony coefficients of the first-level index, the second-level index and the third-level index were 0.289, 0.357 and 0.239, respectively (P<0.01). Conclusion: The evaluation index system of home care competence of wound ostomy specialist nurses is scientific, reliable and practical for the assessment and evaluation of home care competence of wound ostomy specialist nurses.

Construction of nurse-centered home care plan for breast cancer patients with wound catheter

Objective: To construct a co-management care model from Online to Offline (O2O care model) for breast cancer patients at home receiving wound catheter with nurses as the core, so as to provide a reference for the implementation of comprehensive home care for this population. Methods: Literature analysis and Delphi expert letter consultation were used to construct the O2O care model for breast cancer patients with wound catheter, which was nurse-centered, multidisciplinary collaboration, involving patients, family caregivers, volunteers, and various media. Results: The authority coefficients of experts in the two rounds of consultation reached 0.8 and above. The final care model included 4 first-level items (core links, care services, implementation pathways and role functions), 17 second-level items and 56 third-level items. Conclusion: It is scientific, practical and feasible to construct the O2O care model with nurses as the core of the home care team for postoperative breast cancer patients with wound catheter, which can improve the comprehensive and full-cycle treatment of breast cancer patients.

Construction of early functional exercise program for humerus tumor survivors with Reverse Total Shoulder Arthroplasty

Objective: To develop an early postoperative functional exercise program for humeral tumor survivors with Reverse Total Shoulder Arthroplasty (RTSA), and to provide a reference for early postoperative functional exercise. Methods: Based on the evidence-based practice principles, the research team initially formulated an early functional exercise program for RTSA. Two rounds of Delphi expert consultation were conducted from March 10, 2020 to March 30, 2020, and adjustments were made according to expert suggestions. Results: In the first round, 17 experts were consulted, and 15 valid questionnaires were collected, with an effective recovery rate of 88.2%. The variation coefficient of each item was between 0.061 and 0.286, and the Kendall harmony coefficient was 0.104 (P<0.05). Fifteen experts were consulted in round two with an effective recovery rate of 100.0%. The variation coefficient of each item was between 0 and 0.128, and the Kendall harmony coefficient was 0.112 (P<0.05). The mean value of importance assignment was 4.67 to 5.00. Finally, 3 first-level items, 9 second-level items, and 22 third-level items were formed. Conclusion: The early postoperative functional exercise program developed in this study is scientifically reliable, highly operable, and has clinical application values.

Expectations regarding aging and its influencing factors in elderly patients with Chronic Obstructive Pulmonary Disease

Objective: To investigate the expectations regarding aging and analyse its influencing factors in elderly patients with Chronic Obstructive Pulmonary Disease (COPD). Methods: Totally 335 elderly COPD patients hospitalized in the Department of Respiratory of a tertiary grade A hospital in Urumqi from January to June 2022 were selected by convenient sampling method, and the aging expectation scale, frailty phenotype scale, Morse fall risk assessment scale, etc. were used to collect data. Mutiple Liner regression was used to analyze the influencing factors of expectations on aging. Results: The total score of expectations regarding aging in elderly COPD patients was 35.29±6.28. Age, education level, whether with chronic diseases, pulmonary function grading, CAT score, social support and frailty were the influencing factors of expectations regarding aging, which accounted for 46.6% of the total variation. Conclusion: The expectations regarding aging in elderly patients with COPD need to be improved. An integrated management mode should be adopted to provide personalized health guidance, and help patients improve their ability to cope with the changes in disease, and to increase their expectations of healthy aging.

Development of an early warning model for Postoperative Cognitive Dysfunction in patients with primary Hepatocellular Carcinoma

Objective: To explore the risk factors of Postoperative Cognitive Dysfunction (POCD) in patients with primary Hepatocellular Carcinoma (HCC) and develop an early warning model for such patients. Methods: We selected 211 patients with HCC who underwent radical resection of liver cancer from January 2019 to January 2021 in a tertiary grade A hospital in Haikou city. The ROC curve was used to analyze the single factor to observe the difference of continuous variables and obtain the best cut-off value. Multivariate Logistic regression model was used to figure out the risk factors of POCD in HCC patients. Risk factors were used as predictive indicators for constructing the Nomogram model. Internal data was adopted to verify the performance of the Nomogram model and the decision curve analysis was used to evaluate the clinical net benefit of the Nomogram model. Results: Among 211 patients with HCC, 47 patients were diagnosed with POCD (22.3%). Independent risk factors of POCD in HCC patients included age (>66 years old), American Society of Anesthesiologists (ASA) classification (Class III), anesthesia time (>270 min), NLR (>2.68) and TNF-α (>9.87 nmol/L) (all P<0.05). The C-index predicted by the Nomogram model was 0.735 (95% CI: 0.704, 0.847); when the Nomogram model predicted a risk threshold of POCD in HCC patients higher than 0.16, significant additional clinical net benefits would occur. In addition, the clinical net benefit of the Nomogram model was higher than age, ASA classification, anesthesia time, TNF-α and NLR. Conclusion: The early warning model constructed in this study can help predict the risk of POCD in patients undergoing radical HCC surgery and thus provide a method for medical staff to formulate POCD prediction strategies.

Development and validation of a risk prediction model for Fear of Progression in patients with Chronic Heart Failure

Objective: To investigate the current status and influencing factors of Fear of Progression (FoP) in patients with Chronic Heart Failure (CHF), to construct a risk prediction model and to verify the prediction effect of the model. Methods: A total of 188 patients with CHF who hospitalized in the department of cardiology of 2 tertiary hospitals from November 2021 to March 2022 were selected as the subjects by convenience sampling. The risk factors between dysfunctional FoP group (n=64) and non-dysfunctional FoP group (n=124) were compared. The risk prediction model was constructed with Logistic regression analysis using the self-regulation common sense model as the theoretical framework. Hosmer-Lemeshow test was used to test the goodness of fit and the area under the Receiver Operating Characteristic curve (AUC) was used to evaluate the efficacy of the model. Eighty CHF patients were selected to verify the prediction effect of the model. Results: The incidence of dysfunctional FoP in patients with CHF was 34.04%. Age (OR=0.909), left atrial diameter (OR=1.074), New York Heart Association function classification (OR=9.156), number of non-cardiovascular comorbidities (OR=4.222), Heart Failure Somatic Perception Scale score (OR=1.153), Connor-Davidson Resilience Scale-10 item (OR=0.749), and Self-care of Heart Failure Index (OR=0.926) were independent influencing factors of dysfunctional FoP in patients with CHF (P<0.05). The AUC of the model was 0.926, the sensitivity was 81.5%, the specificity was 89.1%, and the Youden index was 0.706. The prediction accuracy was 88.75%. Conclusion: The risk prediction model for FoP in patients with CHF constructed in this study has good predictive effects, which is suitable for clinical screening. It might provide a scientific reference for nursing staff to identify high-risk populations and to take targeted positive psychological interventions.

Effect of light simulation training on patients' intraoperative coordination during Small Incision Lenticule Extraction eye surgery

Objective: To analyze the effect of preoperative light simulation training on patients' intraoperative coordination during Small Incision Lenticule Extraction (SMILE) eye surgery. Methods: From April 2022 to July 2022, a total of 252 patients with refractive error who underwent SMILE eye surgery in the refractive surgery center of a tertiary eye hospital in Tianjin were selected by convenient sampling method, and were divided into the experimental group (125 cases) and the control group (127 cases) based on the date of surgery. The experimental group received preoperative light simulation training, while the control group was given routine health education. The scores of the preoperative and postoperative anxiety Self-Assessment Scale (SAS), General Self-Efficacy Scale (GSES) and postoperative satisfaction questionnaire, and postoperative visual recovery were compared between the two groups. Results: After intervention, the anxiety score of patients in the experimental group was lower than that in the control group, and the difference was statistically significant (P<0.05). The self-efficacy score of the patients in the experimental group increased and was higher than that in the control group with statistically significant difference (P<0.05). The number of central positioning, abnormal movement of eye position and negative pressure loss of patients in the experimental group were less than those in the control group. Patients in the experimental group were observed to show better outcome in terms of the visual changes than the control group one day, one week and one month after operation, respectively, and their satisfaction was better than that in the control group with the difference being statistically significant (all P<0.05). Conclusion: Preoperative light simulation training can reduce patients' anxiety, improve their self-efficacy to boost their intraoperative cooperation, help patients to obtain the expected vision earlier, and make the operation safer and more efficient. At the same time, such training can also increase patients' postoperative satisfaction, and enhance their compliance and trust in doctors, nurses and hospitals.

Risk factors for neglect in older adults: a Meta-analysis

Objective: To systematically analyze the risk factors for neglect in the elderly. Methods: Databases including PubMed and Web of Science and so forth were searched to retrieve studies on the influencing factors of neglect in older adults from their establishment to May 2022. Meta-analysis was performed by RevMan5.3 software. Results: A total of 22 papers were enrolled, containing 18 cross-sectional studies and 4 cohort studies referring to 47,292 subjects. Meta-analysis showed that advanced age, female, divorce, low income, chronic illness, depression, insufficient social support, declined activities of daily living, cognitive impairment, and poor health status were risk factors for neglect in elderly people. Conclusion: Elderly neglect is influenced by various factors, and thus it is necessary to focus on the high-risk population and take proper measures in time.

Summary of the evidence for the prevention of device related pressure ulcers in patients with prone position ventilation

Objective: To summarize the evidence on the prevention of device related pressure ulcers in critically ill patients with prone position in the domestic and overseas. Methods: Relevant literatures from domestic and foreign guideline websites, professional association and databases were systematically retrieved until Nov 1, 2022. The qualified studies were included and evaluated for quality, and evidence was scientifically extracted, summarized and graded. Results: A total of 24 literatures were included with 6 guidelines, 6 expert consensus, 8 systematic reviews, and 4 evidence summaries. The summary included 26 pieces of evidence in 7 topics, including risk assessment, skin assessment, selection and use of devices, surface management, position changes, moisture management and information provision. Conclusion: Clinical staff should carry out evidence-based prevention of device related pressure ulcers in critically ill patients with prone position based on adequate risk assessment in the early stage, attach importance to the management of patients' pressed skin, take a variety of decompression measures to ensure the prevention strategies are scientific and systematic.

Evidence summary of management of delayed onset of lactation among breastfeeding women

Objective: To retrieve the evidence of management of delayed onset of lactation among breastfeeding women. Methods: We systematically searched databases and websites for evidence on delayed onset of lactation to collect guidelines, best practices, expert consensuses, systematic reviews and randomized controlled trials. The retrieval period was from January 2017 to May 2022. Two researchers with evidence-based nursing background assessed the quality of the literature independently and extracted the evidence. Results: A total of 11 articles were included, including 6 systematic reviews and 5 Randomized Controlled Trials. Twenty pieces of evidence were obtained, including evaluation methods of lactation initiation, influencing factors of lactation initiation, prevention of delayed lactation initiation and intervention. Conclusion: This study summarized 20 pieces of the evidence of management for delayed onset of lactation, which can be applied in clinical practice. Medical staff should formulate management strategies for delayed onset of lactation based on clinical scenarios and influencing factors to reduce the incidence of delayed onset of lactation and to improve breastfeeding rates.

The experience of taking immunosuppressants in renal transplant recipients: a Meta-synthesis

Objective: To systematically review and synthesize the experience of taking immunosuppressants in renal transplant recipients, and to provide evidence-based guidance for interventions to improve the medication taking experience and promote the medication taking behavior. Methods: Relational database were searched from inception to April 2022 for qualitative research studies on the experience of taking immunosuppressants in renal transplant recipients. The Joanna Briggs Institute Critical Appraisal Tool for qualitative research (2016) in Australia was used to evaluate the quality of studies, and the Meta-synthesis method was used to integrate the results. Results: A total of 17 studies were included, with 43 clear research findings grouped into 10 categories by their similarities, and 3 integrated findings were synthesized, including perceived benefit of taking medicine, perceived barriers of taking medicine, and coping strategies. Conclusion: Nurses should pay attention to the difficulties and obstacles faced by renal transplant recipients in taking immunosuppressants, timely evaluate whether the coping strategies are effective, and provide various professional supports for patients to improve their medication experience and promote their medication behavior.

Objective: To investigate the effect of Forced-air Warming and Carbon-fibre Polymer-fabric Resistive Heating on the prevention of Inadvertent Perioperative Hypothermia (IPH). Methods: We searched Chinese and English databases from inception to March 2021 to collect Randomized Controlled Trials on Forced-air Warming versus Carbon-fibre Polymer-fabric Resistive Heating for prevention of IPH in adults. Two reviewers independently screened the literatures, extracted the data, and assessed the risk of bias of included studies. Results: A total of 16 studies were included. Meta-analysis results showed that compared with Carbon-fibre Polymer-fabric Resistive Heating, Forced-air Warming raised core body temperature 90 minutes after induction of anesthesia [SMD=0.42, 95%CI (0.07, 0.77)] and at the end of the procedure [SMD=0.41, 95%CI (0.28, 0.54)]. Conclusion: Current evidence shows that compared with Carbon-fibre Polymer-fabric Resistive Heating, Forced-air Warming can better maintain the core body temperature of perioperative patients. It can provide reference for perioperative temperature management.

Methods: We searched databases such as CNKI, WanFang Data, VIP, SinoMed, PubMed, Web of Science, Cochrane Library and CINAHL. Then we performed Meta-analysis using RevMan 5.4 software. Results: Twenty-six studies were finally included, including 3 527 primiparas. Meta-analysis results show that nursing based on Solution-Focused Brief Therapy can reduce the anxiety and the depression of primiparas; the rate of spontaneous childbirth and the 1 min Apgar score in the solution-focused group was significantly higher than that in the control group, the rate of neonatal asphyxia in the solution-focused group was lower than that in the control group, it is not yet considered that the Solution-Focused Brief Therapy can improve postpartum hemorrhage in primiparas situation. Conclusion: Solution-Focused Brief Therapy can reduce the anxiety and depression of primiparas, and can improve the natural delivery rate, 1 min Apgar score, and neonatal asphyxia rate of primiparas. It has not been found to improve postpartum hemorrhage in primiparas, which needs to be verified by multi-center, large sample and well-designed studies.

From the turnover intention to decision in nurses: a grounded theory study

Objective: To explore and describe the different stages during the process when nurses develop the turnover intention and make their turnover decision, to help nursing managers capture the early signs and predict the turning point before their leaving, so as to take effective countermeasures. Methods: The study used both purposeful sampling and theoretical sampling to select clinical nurses and head nurses who proposed resignation in the past 10 years as the research objects, and collected and analyzed the data using the constructivist grounded theory paradigm established by Charmaz. Results: Nurses with the intention to turnover underwent four stages: willingness incubation period, willingness weighing period, willingness acceleration period, and turnover decision-making period, generating four native elements included blasting fuse, fuel, combustion promoter, and flame retardant. Conclusion: Nursing managers can predict nurses turnover in advance and capture relevant factors according to the stages of nurses turnover experience and the corresponding native elements to discover nurses' turnover thoughts and turnover intentions in time and help nurses re-establish their adherence to the profession to avoid their turnover intention and decision by means of adjusting management strategies in multiple dimensions, tracking the development of turnover trajectory and deeply exploring the reasons.

Objective: To investigate the characteristics of disaster preparedness of the nursing reserve team in Yulin, northen Shaanxi, and analyze its influencing factors in responding to disastrous situations. Methods: From July to November 2021, following convenient sampling, a total of 1440 nurses from 37 hospitals in Yulin were enrolled in the current study and completed the Disaster Preparedness Evaluation Tool. Results: The average score of disaster preparedness among nursing reserve team in Shanbei was 4.42±0.84, and the item which received with the lowest score was "I will be considered as a key leader in my community in disaster situation", while the item which received with the highest score was "I think government support at the national/regional/provincial level is sufficient in disaster relief ". Our findings showed that the main factors that influenced the level of disaster preparedness of nurses were multifactorial, including the title of the nurses, and whether they received training on knowledge of disaster via continuing education, had experience on disaster rescue, received training on disaster skills and had working experience in the fever clinic or COVID-19 isolation ward. Conclusion: The level of disaster preparedness of mumber of nursing reserve team is considered as moderate in the Shanbei region, it is still necessary to offer systematic training on disaster knowledge and skills for nursing reserve team to strengthen their competence in dealing with sudden disasters.

Objective: To explore the practical experience of workplace reciprocity for emergency department nurses, in order to sort out the cooperative culture of their teams, and provide a reference for the development and advancement of reciprocal behavior in the emergency department, as well as the improvement of emergency nursing management. Methods: A semi-structured interview was conducted with 13 emergency department nurses using a qualitative research method, and the data were analyzed and sorted out by content analysis method. Results: Through the collation and analysis of data, a total of four themes were extracted: reciprocity existed widely and conformed to the characteristics of emergency nursing work; the forms and timing of reciprocal behavior varied; interaction between nurses with different working years; emergency care managers encouraged and supported reciprocal behavior. Conclusion: Workplace reciprocity has an important impact on emergency department nurses, and improving workplace reciprocity can create a safer, more convenient, and more efficient work atmosphere. It is suggested to combine the characteristics of emergency nursing work culture and the characteristics of reciprocal behavior of nurses of different seniorities, figure out the existing reciprocal behavior of emergency care, summarize its form and timeliness of development, and encourage reciprocal behavior at the management level to promote emergency nursing teamwork and improve the working environment and atmosphere.

Objective: To understand the situation and feelings of clinical nurses facing time pressure in their work, and to provide reference for coping with time pressure in nursing work and improving nurses' happiness at work. Methods: A descriptive phenomenological research method was adopted. Purposive sampling and maximum difference sampling led to the inclusion of 14 nurses of 3 tertiary grade A hospitals in Suzhou from January to April 2022. Data were collected using semi-structured in-depth interviews and analyzed by the Colaizzi 7-step method. Results: Four themes were extracted through the analysis: time pressure in clinical nurses is ubiquitous, time pressure is negatively related to the quality of nursing and nurses' physical and mental health, time pressure is positively related to creativity and job engagement, poorly individual strategies to cope with time pressure. Conclusion: Working under time pressure leads to various challenges to nurses. Time pressure has a double-edged effect, and nursing managers should establish appropriate time pressure, so as to promote more positive effects.

Objective: To study the effect of kangaroo nursing combined with touching on the pain of preterm infants screened for Retinopathy Of Prematurity (ROP). Methods: Using the convenient sampling method, from January 1, 2019 to January 1, 2021, 70 premature infants who met the ROP screening criteria in the pediatric ward of a tertiary grade A hospital in Nantong city were selected as the research objects, and they were randomly divided into the experimental group (n=33) and the control group (n=37) according to the random number table method. The control group was screened according to the routine procedure, and the experimental group received kangaroo nursing combined with touch during the screening process. Preterm Infant Pain Profile (PIPP), regional cerebral oxygen saturation (rScO2), blood oxygen saturation (SaO2) and heart rate were used to compare the degree of pain in ROP screening between the experimental group and the control group. Results: During ROP screening, the PIPP score of patients in the experimental group was lower than that in the control group (P<0.05). The rScO2 and SaO2 in the experimental group were higher than those in the control group (all P<0.05). Conclusion: Kangaroo nursing combined with touching can effectively reduce the pain of ROP screening in preterm infants.

Objective: To explore the diet therapy care experience of children with Methylmalonic Acidemia (MMA), in order to provide reference and basis for promoting the management of children's diet therapy. Methods: Using the phenomenological research method and the purposeful sampling method, from September to December 2021, 16 caregivers of children with isolated MMA who were treated or revisited in a tertiary grade A hospital in Shandong province were selected for semi-structured interviews. The Nvivo 12 software was used to organize the interview data. Using the thematic analysis method to analyze and refine the themes. Results: A total of 5 themes and 14 sub-themes were extracted through inductive analysis, and the 5 themes were: children's self-restriction, positive coping style, lack of knowledge about diet therapy, difficulty in obtaining medical food, and weak social support system. Conclusion: The caregivers of children with MMA play an important role in the process of die therapy, and a positive coping styles of caregivers can improve compliance, but there are still many obstacles and problems in the treatment process. Medical staff can unite with caregivers to appeal to all sectors of society to increase attention to children with MMA, and improve children's compliance and prognosis.

Objective: To examine the effects of clearance breathing on pulmonary function and treatment compliance in patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease undergoing first non-invasive ventilation. Methods: A total of 99 patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease undergoing the non-invasive ventilation for the first time in the Intensive Care Unit were recruited in a tertiary grade A hospital in Wuxi from June 2020 to December 2021 and they were randomly assigned to the control group (n=49) or the observational group (n=50) by a random number table. Patients in the observational group received the clearance breathing while patients in the control group received routine care. Non-invasive ventilator treatment compliance, blood gas changes at different time periods, pulmonary function, tracheal intubation rate, average length of hospital stay, mMRC and CAT scores were compared between the two groups. Results: The treatment compliance, arterial pH, oxygenation index, and FEV1/FVC ratio of patients in the observational group were significantly higher than those in the control group (P<0.05). The mMRC, CAT score, average length of hospital stay and intubation rate were significantly lower than those in the control group (P<0.05). Conclusion: Clearance breathing for patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease undergoing first non-invasive ventilation can effectively improve the pulmonary function, improve the treatment compliance, and increase the success rate of ventilation. The clearance breathing is worth of promotion in clinical settings.

Objective: To analyse the current situation and the advancement of Comprehensive Geriatric Assessment tools for nursing homes in China and abroad to provide references for the development of Comprehensive Geriatric Assessment tools and implementation of such assessment in the future. Methods: According to the research method of scoping review, related databases were searched. Papers about Comprehensive Geriatric Assessment tools were retrieved from the established time to July 2021. We also searched relevant websites and referred to citation of those included papers. Results: A total of 1619 papers were found, and 24 papers were finally included, 14 Comprehensive Geriatric Assessment tools including interRAI, OARS, CARE and the like were summarized. The assessment focused mainly on the health status of the elderly in order to improve their life quality. Conclusion: There are no available Comprehensive Geriatric Assessment tools for the elderly in nursing homes currently, which calls for such tools to keep pace with the times with dynamism, continuity and integrity.
