In order to give full play to the function of the wound-ostomy specialist nurses, provide the patients with high quality nursing service. Since 2012, based on the concept of advanced nursing practice, our hospital has actively explored ostomy wound professional work mode and operated in the following ways, including authorization of ostomy wound group, establishment of the specialist outpatient clinics, specialist nurses participate in nursing quality control, education training, and intractable cases discussion, as well as to select specialist nurses strictly and cultivate them with a stage way and so on, which allowed the multiple roles of wound-ostomy nurses' function has been given full play, the practice level of hospital advanced nursing practice and the comprehensive ability of specialist nurses have been improved, at the same time to meet the value demand of specialist nurses. It also will provide reference to promote the development of the nursing specialty.

Pilot study on the model and effects of managing complex wounds with interdisciplinary team cooperation

Objective: To explore the model and effects of managing complex wounds with interdisciplinary team cooperation. Methods: According to characteristic of complex wounds, an interdisciplinary team, including senior enterostomal therapist (ET), surgeons and pathological physicians, was built. ET assessed and checked every patient at first, and then analyzed and recognized healthy problems, using “help each other or menu list mode” to contact with related surgeons or physicians to consult and make treatment protocols and wound care plan. Wound care nurses charged for carring out wound care plan, and evaluated wounds healing rate and healing time after interdisciplinary team cooperation. Retrospective analysis about different complex wounds healing rate and healing time was conducted with data before the interdisciplinary team setting up three years ago. Results: Before interdisciplinary team cooperation model, 563 patients with complex wounds were managed ,while with interdisciplinary team cooperation model, 628 complex wounds were managed. Total healing rate was by raised 13.63% (P<0.05) and mean healing time was shortened 25.67 days (P<0.05). Conclusion: Interdisciplinary team cooperation model is beneficial to enhance wound healing rate and decrease healing time. It may be a good method to improve wound care results.

Interpretation of expert standard for nursing care of patient with chronic wound

The article aims to provide wound nurses with reference to regulate clinical wound care practice by interpreting the updated expert standard "Expert standard for nursing care of people with chronic wound". The updated expert standard consists of five levels including assessment, planning, implementation, consulting/training and evaluation. It requires the qualified nurse to conduct an accurate assessment for patients with chronic wounds, formulate a feasible wound care plan and ensure that the plan can be carried out correctly. The participation and relevant knowledge training of patients and their family members are emphasized during the formulation and implementation of the chronic wound care plan. Furthermore, the nurse should evaluate the entire program at a certain time and revise the plan as appropriate. And each hierarchy is managed following three aspects of structure, process and result, so as to ensure the best effect of chronic wound care.

Standardized assessment and management of 56-cases chronic wounds

Nursing practice experiences in 56 cases with chronic wound were concluded using standardized wound assessment. Assessment tools were highlighted including initial assessment tool, evaluation and processing orders, Changhai Wound Measuring Ruler and Changhai Pain Feet. Insistency and integrity of assessment process was guaranteed by qualified nursing staff, timely assessment frequency at least once a week, increasing assessment times when wound changed, implementing first consultation system and strict handover system. Personal qualification ought to be strictly regulated, to ensuring assessment consistency and completeness. The standardized assessment have guiding significance for wound care.The local wound condition, patients’ overall dysfunction and possible reasons for causing disease were identified by evaluation of wound. Suitable wound dressing and targeted wound treatment were selected. Multi-discipline cooperation, specialties in wound assessment deserves to be recommended to have better treatment effect and facilitate chronic wound healing.

Clinical practice of pressure ulcers chain management

Objective: To explore the application mode of pressure injury chain management in clinical practice. Methods: Control the risk of pressure injury on patients from the treatment to the discharge, including the emergency department, the ICU, the operating room, the ward and the continuation of nursing. Results: It is conducive to the scientific management of skin care and pressure injury monitoring in the whole hospital. The incidence of pressure injury in hospitalized patients decreased year by year, which was lower than the average level of the national and Jiangsu province. Conclusion: Through the chain management of pressure injury in clinical use, the organization management chain and link management chain are established, the lateral management and control of the pressure injury care are strengthened, and the continuous chain management structure is formed.

Research hotspots on critical care in China from 2012-2016 based on Co-word analysis

Objective: To identify research hotspots on critical care in China from 2012 to 2016 in order to provide references for further research. Methods: The authors retrieved literature published between 2012 and 2016 from Wanfang Database and CNKI using selected medical subject headings (MeSH) terms. Bicomb and SPSS 23.00 were used to generate high-frequency MeSH terms and identify hotspots by Co-word analysis and cluster analysis. Results: Totally 5 hotspots of domain research were obtained, including studies on care bundles and evidence-based nursing; care of children in NICU; specialized training of ICU nurses; nutrition support; critical care quality improvement. Conclusion: The results of this study reflected hotspots in research on critical care in China, which provide references for further research.

The relationship between practice environment and professional values among nurses with master or doctoral degree in tertiary hospitals

Objective: To investigate the status of professional values and practice environment among nurses with master or doctoral degree, and analyze the impact of perceived practice environment on professional values. Methods: Totally 1223 nurses with master or doctoral degree from 115 tertiary hospitals in 22 provinces were investigated with the Nurses Professional Values Scale -Revised (NPVS-R) and the Practice Environment Scale of the Nursing Work Index (PES-NWI). Results: The mean score of professional values was (4.51±0.51). The factor scores from high to low were caring (4.57±0.52), professionalism (4.50±0.60), justice (4.47±0.65), activism (4.42±0.62) and trust (3.63±0.45). The mean score of practice environment was (3.21±0.49). Hierarchical regression showed that nurses with lower initial education (β=-0.097), higher job satisfaction (β=0.174) and higher monthly income (β=0.063) showed higher professional values (R2=0.063). After controlling for general information, high-quality care services base (β=0.371) and nurse-physician co-operation (β=0.104) in nursing work environment were significant influencing factors of professional values, which explained 16.5% of the total variance. Conclusion: Nurses with master or doctoral degree in tertiary hospitals show positive professional values, which are influenced by initial education, job satisfaction, monthly income and practice environment. Nursing managers should provide proper guidance and management to help them establish correct professional values.

Nurses' behaviors of patients' privacy protection and influencing factors

Objective: To investigate nurses' behavior of patients' privacy protection and its influencing factors. Methods: The convenient sampling method was used, 616 nurses in two tertiary-level hospitals were investigated by the Patient Privacy Scale (PPS). Results: The dimension "favorable environment" got the highest score, while "sexual privacy" got the lowest. The item "when necessary, equipment as folding screen and covering are used to ensure privacy" got the highest score. The item "nobody is allowed to watch the practices unless approved by the patient". The multiple regression analysis showed that age, duty, privacy protect standards in department, and the usage of materials which to protect patients' privacy influenced nurses' behavior of patients' privacy protection (P<0.05). Conclusion: The effect of age and duty should be concerned for nurses' behavior of patients' privacy protection. Standards should be developed to create an ethical environment for patients' privacy protection.

The working status of nurses with master or doctoral degree in tertiary hospitals in China

Objective: To investigate the working status of nurses with master or doctoral degree in tertiary hospitals. Methods: Totally 115 nursing managers and 1 522 nurses with master or doctoral degree were recruited form 115 tertiary hospitals in 22 provinces. They were investigated by using self-designed questionnaires. Results: The proportion of nurses with master or doctoral degree in tertiary hospitals ranged from 0.1% to 8.8% (median was 0.7%). Among them, 28.6% had special nurse qualification certificate; 5.0% took part in special outpatient clinic independently and 46.6% organized rounds or consultation; 36.3% were engaged in management. In the field of teaching, 84.5% participated in clinical teaching and 43.7% in theory teaching, 26.0% held continuing education programs. In aspect of research work, 36.1% participated in research projects as a principal investigator, 77.1% published Chinese papers and 10.7% published papers in SCI journals as a first or corresponding author; 10.4% edited books or guideline as a chief editor and 8.0% were peer reviewers. Manager's satisfaction scores were lower than expectations (P<0.05). Conclusion: The proportion of nurses with master or doctoral degree is relatively low. It is suggested to improve the individualized training mode and objectives of the nurses, further clarify roles to make full use of their work potential and achieve personal value.

Effects of Horticultural Therapy on quality of life and social function in patients with depression

Objective: To explore the effects of horticultural therapy on symptoms, quality of life, and social function in patients with depressive disorder. Methods: Totally 134 patients with depressive disorder were randomly divided into control group (n=67) and therapy group (n=67). The control group was treated with antidepressants combined with conventional care, while the therapy group received the extra horticultural therapy. Hamilton Depression Rating Scale (HAMD), Quality of Life Comprehensive Assessment Questionnaire (GQOLI-74), and Social Function Defect Screening Scale (SDSS) were used to assess the improvement, before and after those treatments. Results: HAMD, GQOL-74 and SDSS score of therapy group after 8 weeks were 6.60±2.42, 13.57±3.63, 7.94±1.32, respectively, which were significantly higher than those of the control group (P<0.05). Conclusions: Horticulture therapy can significantly release the symptoms and improve the quality of life and social function of patients with depression, and it is worthy to be promoted in clinical practice.

The current status and influencing factors of care preparedness of family caregivers among stroke patients

Objective: To investigate the status of care preparedness by family caregivers of stroke patient, to explore factors associated with preparedness, and provide evidence for effective health education for caregivers of stroke patients. Methods: A total of 200 family caregivers of stroke patients were investigated with the Care Preparedness Scale and Warning Symptom Questionnaire. Results: The score of CPS among family caregivers was 13.99±4.95. The multiple linear regression analysis revealed that caregivers' cognitive level of warning symptoms, age, education level, care experience, the number of other caregivers and patient gender were main factors influencing the degree of care preparedness. Conclusion: This study manifests that CPS among family caregivers remains to be improved. As a result, healthcare worker should provide individualized intervention according to certain situation of the caregivers and the patient, so as to improve caregivers' preparedness.

Constructing the frame of nursing role and work content in day-care chemotherapy for cancer

Objective: To construct the frame of nursing role and work content in day-care chemotherapy for cancer. Methods: Through searching literature materials and group discussions, we constructed the primary frame of nursing role and work content. Then 22 experts of relative areas to day-care chemotherapy for cancer were interviewed by mails for three rounds using Delphi method. Results: The returning rates and efficiency of three rounds mailing interview were all 100.0%, respectively. Specialists were quite active while specialists' Cr was 0.88 and coefficient of coordination were 0.119, 0.158 and 0.215 (P<0.05). The frame of nursing roles and work content in day-care chemotherapy for cancer consisting of 6 primary indicators (including clinical practitioner, educator, administrator, consultant, coordinator and researcher), 21 secondary indicators and 62 tertiary indicators was finally determined. Conclusion: The frame of nursing role and work content in day-care chemotherapy for cancer is scientific and reliable, with a relatively high degree of specialists' Cr, authority and concentration of opinions. It could promote the advance and development of management over nurses in day-care chemotherapy for cancer.

The practice and effects of magnetic hospital on nursing management

Objective: To explore the practice and effects of magnetic hospital on nursing management. Methods: The nursing management practice was performed from the four aspects: nursing leadership, structural empowerment, professional independence and research and innovation. Results: The nursing working environment was improved and the organizational structure was optimized. Nurse professional recognition remained at 97% in the past three years. The turnover rate of nurses remained at 1.8%. Conclusion: It is an effective management model to integrate the magnetic hospital culture on nursing management to improve the satisfaction of both patients and nurses.

The mediating effect of psychological capital for psychological distress and perceived organizational support among psychiatric nurses

Objective: To investigate the effect of perceived organizational support and psychological capital on psychological distress among psychiatric nurses. Methods: Eight hundred and twelve psychiatric nurses were recruited from six psychiatric hospitals of Shandong province. They were investigated with the Kessler Psychological Distress Scale, Survey of perceived organizational support, Psychological Capital Questionnaire. Results: About 70.3% of psychiatric nurses had psychological distress. The mean scores of psychological distress, perceived organizational support and psychological capital were 21.29±8.11, 37.17±11.07 and 4.09±0.70 respectively. Psychological distress was negatively related to psychological capital and perceived organizational support (P<0.01). Psychological capital had positive correlation with perceived organizational support (P<0.01). Psychological capital was a mediator (mediating effect was -0.11) of perceived organizational support and psychological distress; and it explained 29.1% of the total effects. Conclusion: Managers of psychiatric department should provide supportive settings and investment in psychological capital to improve psychological distress among psychiatric nurses.

Impact of career planning and management on nurses' career development

Objective: To investigate the impact of career planning and management on nurses' career development in changsha. Methods: From August to October in 2016, nurses from 9 hospitals whose levels were beyond second A in Changsha were surveyed through a questionnaire about their career planning, career management and career development. And results regarding the impact of career planning and management over current career development were analysed. Results: The score of career planning of those nurses was 39.98±8.38, the score of career management was 49.62±8.86 on and the score of career development scale was 42.80±7.12 Pearson correlation analysis showed that there was a positive correlation between all dimensions and the total score of career planning except the two dimensions of relocation and attention (P<0.05). And status of nurses' career development was positively correlated with all dimensions and the total score of nurses' career management (P<0.05). Multivariate linear regression analysis showed that training and establishing career goals and strategies and self-presentation had a significant effect on nurses' career development (P<0.05). Conclusion: Good career planning and management can effectively promote career development of nurses.

Application of film scenes for Case-Based Learning in disaster nursing course

Objective: To investigate the effect of film scenes embedded in a Case-Based Learning (CBL) course on developing nursing knowledge, skills and emotion ability of nursing students. Methods: 123 undergraduate nursing students were assigned into control group (n=60) and experimental group (n=63). Regular PBL was applied to control group, while video clips were produced to embed in PBL as cases in experimental group. At the end of the course, the differences of theoretical knowledge, practical skills and emotional abilities of the two groups were compared. Results: The ability of empathy of nursing students in control group was higher than experimental group on baseline, but there were no significant differences between the two groups in humanistic literacy and nurse-patient communication ability. After the intervention, theoretical knowledge, practical skills, empathy ability, humanistic accomplishment and communication abilities of experimental group were more improved than control group (P<0.05). Conclusion: The case scenarios with instructional video clips using in disaster nursing CBL were designed to improve nursing students’ knowledge, skills and emotional ability.

The application of APP intelligent quality management system in nursing quality evaluation in the operating room

Objective: To analyze the application of APP intelligent system for examination and assessment of nursing care quality in operating room. Methods: The traditional nursing care quality evaluation method was used from January to June 2016, and the new APP intelligent system was implemented from July to December 2016. The nursing quality assessment standards and the methods of on-site evaluation were the same in these two different periods. Results: The assessment frequency, time, records integrity, evaluation timeliness, numbers of potential risks for safety, and the numbers of Level 3 adverse events, were different between these two periods when using two different evaluation methods (P<0.05). Conclusion: APP intelligence system for examination and assessment of nursing care quality created a new pattern in the operating room's quality management; its functions included the intelligence analysis, timely evaluation, integrated recording, timely discovering the potential troubles and forward control. The system can reduce the adverse events effectively.

Medical integration mode in handoffs for patient transitioning from the Intensive Care Unit

Objective: To determine whether medical integration mode improves the quality and safety of handoffs for patient transitioning from the Intensive Care Unit (ICU). Methods: The medical integration mode was applied from July 2015 to December 2016, with a checklist and standardized hand-off procedure. Results: The rate of return to ICU within 48 hours decreased from 1.79% to 0.87% (P<0.05). The incidence of adverse events during transfer decreased from 14.05% to 3.24% (P<0.01). The average time of hand-off was shortened from (18.70±2.62) min to (10.52±2.14) min (P<0.01). The disputes between patients and nurses reduced to none. Conclusion: Medical integration is an accurate, safe, and efficient way to perform handoffs and provide high-quality patient care.

Analysis of transitional care needs in children with congenital anorectal malformation and its influencing factors

Objective: To investigate the transitional care needs in children with congenital anorectal malformation (ARM), and to discuss the relevant influencing factors, so as to provide evidence for the development of transitional care program. Methods: Totally: 195 ARM children were recruited in Children's Hospital of Chongqing Medical University from December 2015 to July 2016 by convenience sampling methods. We investigated and analyzed the transitional care needs using the self-designed questionnaire. Results: Transitional care needs in children with ARM were high; the average score of total needs was (4.07±0.45). The score of each dimension of transitional care needs and the total score had significant differences in different education level of main caregivers, relationship between main caregivers and the child, parents' occupation, disease classification and disease course (P<0.05). Multiple stepwise regression analysis showed that the main caregivers' education level, disease classification, disease course and parents' occupation had influence on the transitional care needs in children with ARM. Conclusion: Main influencing factors of each dimension of the transitional care needs in children with ARM should be analyzed comprehensively. At the same time, interventions should be taken from the aspects of the comprehensive management of intestinal tract, anus function recovery promotion and health management, considering all the influencing factors on the transitional care needs in children with ARM, to establish effective transitional care plan, prevent complications and improve the quality of life of children and family.

The investigation of nursing needs of school-aged children underwent lumbar puncture with Acute Leukemia

Objective: To investigate the nursing needs of school-aged children underwent Lumbar Puncture (LP) with Acute Leukemia (AL) and the main influencing factors. Methods: A total of 216 school-aged children with Acute Leukemia (AL) were enrolled. A structured questionnaire was used to survey. Results: The overall score of nursing needs was (124.37±14.27). The scores of information needs, communication needs, esteem needs, comfort needs, and self-actualization needs were (36.30±8.71), (34.99±4.13), (18.38±1.43), (17.94±1.92), (16.76±2.42), respectively. The result of multiple linear regression showed that nursing needs of school-aged children underwent LP with AL were influenced by the amount of lumbar punctures, gender, age and caregivers. Conclusion: Clinical nursing staff should address the gaps in meeting patients' needs, and eventually improve patients' quality of life.

The care needs assessment of 1 205 elders in Suzhou using Long-Term Care rating criteria

Objective: To formulate the Long-term Care rating criteria in Suzhou through experts consultation, and then investigate the long-term care needs of 1205 elders in Suzhou using the criteria. Methods: Through 7 rounds of group meetings of 14 experts, the Long-term Care rating criteria in Suzhou (Revised) was finalized, which was then used to assess and analyze the long-term care needs of 1 205 elders living in 4 communities and 1 nursing institution by convenient sampling. Results: The care needs levels assessed by the rating criteria were in agreement with the actual situations of the 1 205 elders. Conclusion: The Long-term Care rating criteria is scientific, authoritative and applicable. It provides a scientific basis for the government to provide pension subsidies, allocate human resources rationally in the nursing institutions and implement long-term care insurance system.

This paper reviewed the studies of PICC maintenance service, the establishment and management of PICC maintenance network. It is expected to provide information for the establishment of PICC maintenance network in China.

Objective: To standardize the VTE preventive nursing methods in hospital, and to provide standardized and homogeneous VTE nursing care for the inpatients. Methods: The VTE special nursing group was established in the hospital, and the group members were trained by three steps. We carried out VTE evaluation, prevention and other work on the inpatients, and managed patients with VTE using the nursing information technology system. Results: The level of VTE knowledge and skills of clinical nurses was significantly improved, and the patients received standardized and homogeneous VTE care during hospitalization. Through the application of VTE patients management platform, we followed the dynamic track of patients disease progression. Since the establishment of the group, we have published 7 nursing papers, and won 3 awards. Conclusion: The management model of VTE special nursing group improved the VTE prevention rate of VTE and the health of patients. It also provided a platform for clinical nurses to learn and communicate, standardized the behavior of nurses, and promoted the nursing care in prevention of VTE.

Objective: To investigate the effects of multidisciplinary team-based early rehabilitation model in patients with extensive burns. Methods: Totally 104 cases of patients with extensive burns were enrolled and grouped into control group and intervention group using convenience sampling method. Results: There were significant differences between two groups in hospital stay, medical expenses, the ADL and the quality of life between two groups (P<0.05). Conclusion: Multidisciplinary team-based early rehabilitation model can improve the quality of life and rehabilitation for patients with extensive burns.

A longitudinal study on the supportive care needs and quality of life of patients with breast cancer

Objective: To describe the current situation about supportive care needs and quality of life of breast cancer women at four time points during their treatment; to explore the trends of these changes in order to provide evidence for personalized nursing intervention of patients. Methods: We selected 94 patients who were diagnosed with breast cancer. It was a longitudinal investigation and we used a self-designed questionnaire, Supportive Care Need Survey Short-Form (SCNS-SF34) and Quality of life (FACT-B, V4.0). Results: Data were analyzed by repeated measurement analysis of variance. The scores of physical and daily life, patient care support, and health information and sexual needs showed significant differences (P<0.05). And the scores of the quality of life and each dimension showed significant differences (P<0.05). Conclusion: There were significant differences between quality of life and supportive care needs at different time points. Nurses should do scientific evaluation and provide personalized care to the patients.

Nurses’ cognition of palliative care plays an important role in the nursing practice. This article reviewed the current situation of nurses’ cognition of palliative care and its influence factors and improvement measures. The main influence factors included demographic factors (age, work experience, department, job title, education and religion), nurses lacking of relevant knowledge of palliative care (the weak awareness of pain management and the lack of psychological knowledge), the constraint of traditional idea of death and the lack of social support. The measures included standardizing and popularizing palliative care education, establishing a correct idea of death, and strengthening the support of organizations. The aim of this review is to provide reference for promoting the researches on nurses’ cognition of palliative care in our country.

Relationship between diurnal patterns in Rapid Response Call activation and patient outcome

Background: The Rapid Response Call (RRC) is a system designed to escalate care to a specialised team in response to the detection of patient deterioration. To date, there have been few studies which have explored the relationship between time of day of RRC and patient outcome. Objective: To examine the relationship between the time of RRC activations and patient outcome. Methods: All adult inpatients with a RRC in non-critical care wards of a metropolitan Australian hospital in 2012 were retrospectively reviewed. RRCs occurring between 18:00–07:59 were defined as ‘out of hours’. Results: There were 892 RRC during the study period. RRCs out of hours were associated with a higher rate of ICU admissions immediately after the RRC (19.4% VS 12.3%, P<0.001). Patients experiencing an out-of-hours RRC were more likely to have an in-hospital cardiopulmonary arrest (OR=1.7, P<0.04). In-hospital mortality rate was significantly higher for patients with out-of-hours RRCs (35.5% VS 25.0%, P=0.014). After adjusting for confounders out-of-hours RRC were independently associated with increased need for ICU admissions and in-hospital mortality. Conclusion: The diurnal timing of RRCs appears to have significant implications for patient mortality and morbidity, patient outcomes are worse if RRC occurs out of hours. This finding has implications for staffing and resource allocation.
