风险防控前置的护理质量控制岗位的实践与思考

摘要: 目的:梳理护理质量控制岗位在推进护理风险防控前移中的实践举措,为医疗机构优化护理风险防控提供参考。方法:遵循国家卫生健康委员会医疗质量安全不良事件分级分类管理要求,结合医院管理实际情况,形成院内分级管理标准。将未造成患者伤害但存在潜在风险的事件界定为A类护理不良事件(预前现象),在未增加编制的情况下,由既有的护理质量控制岗位负责此类事件的集中接收、整理和分析,并实行无责上报。结果:2020年—2024年,医院预前现象上报数量分别为2 443、3 705、4 659、4 885、4 436例,呈现先增长后下降趋势。风险归口范围由医嘱类问题扩展至多系统,实现风险信息的统一汇总与反馈;建立了预前现象季度分析与反馈机制,在强化岗位功能的同时,进一步优化了护理人力资源使用方式。结论:通过拓展护理质量控制岗位职责并前移管理重心,将预前现象纳入集中管理,有助于提高风险防控水平、优化岗位功能及人力资源使用方式。

Abstract: Objective: To sort out the practical pathways for advancing proactive nursing risk management through the optimization of nursing quality control positions, thereby providing a reference for medical institutions to improve their nursing risk control systems. Methods: In accordance with the graded classification and management requirements for medical quality and safety adverse events set by the National Health Commission, and combined with the hospital's actual management context, in-hospital graded management standards were formulated. Events that did not result in patient harm but revealed potential risks were defined as category A nursing adverse events (precursor phenomena) . Without adding new staff positions, the centralized reception, collation, and analysis of precursor phenomena were incorporated into the routine responsibilities of the existing nursing quality control positions. A blame-free reporting mechanism was also implemented. Results: From 2020 to 2024, the total number of reported precursor phenomena in the hospital was 2443, 3705, 4659, 4885 and 4436, respectively, showing an initial year-by-year increase followed by a decline. The scope of risk management expanded from medical order-related items to multiple systems, and unified collection and feedback of risk information were achieved. A quarterly analysis and feedback mechanism for precursor phenomena was established, which strengthened the functions of the existing nursing quality control positions and further optimized the allocation of nursing human resources. Conclusion: By expanding the functions of nursing quality control positions and shifting the management focus forward, incorporating precursor phenomena into centralized management can effectively promote nursing risk prevention and control, and optimize post functions and the utilization of human resources.
