中老年心房颤动患者衰弱的潜在类别及影响因素研究

Latent class analysis and the associated factors of frailty in middle-aged and older adults with atrial fibrillation

摘要: 目的:探讨中老年心房颤动患者衰弱的潜在类别及其影响因素,以期为个性化护理提供参考。方法:便利选取2024年2月至2025年2月在南宁市某三级甲等医院住院的431名中老年心房颤动患者为研究对象。采用衰弱量表评估患者衰弱状况,并收集其认知功能、心理状态、营养状况等资料。对患者衰弱进行潜在类别分析,并通过多分类Logistic回归分析其影响因素。结果:中老年心房颤动患者衰弱分为3个类别,分别为“疲劳-功能受限型”(39.4%),“高储备-低风险型”(53.4%),“共病-恶病质型”(7.2%)。多分类Logistic回归分析结果显示,认知功能障碍、高血压、营养不良和抑郁症状、持续性房颤及焦虑症状是衰弱潜在类别的独立危险因素。结论:中老年心房颤动患者衰弱具有明显异质性,医护人员应根据不同类别的特征及影响因素实施个性化的分类干预,以预防和减缓患者的衰弱状态。

Abstract: Objective: To identify distinct frailty classes among middle-aged and older adults with atrial fibrillation and to analyze their associated factors, in order to inform the development of personalized care strategies. Methods: A total of 431 middle-aged and older adults with atrial fibrillation were recruited through convenience sampling in a tertiary grade A hospital in Nanning from February 2024 to February 2025. Frailty was assessed using the Fatigue, Resistance, Ambulation, Illness, Loss of weight scale. Data on cognitive function, psychological status, and nutritional status were collected. Latent class analysis was employed to identify frailty subgroups, and multinomial Logistic regression was used to analyze the associated factors. Results: Latent class analysis identified three distinct frailty classes, "Fatigue-Function-Limited" (39.4%), "High Reserve-Low Risk" (53.4%), and "Comorbid-Cachectic" (7.2%). Multinomial Logistic regression analysis revealed that mild cognitive impairment, hypertension, malnutrition, depressive symptoms, persistent atrial fibrillation, and anxiety symptoms were identified as independent associated factors for membership in the latent frailty classes. Conclusion: Frailty in middle-aged and older adults with atrial fibrillation exhibits significant heterogeneity. Healthcare professionals should formulate targeted and personalized stratified nursing interventions based on the unique characteristics and associated factors of each class to prevent or mitigate the progression of frailty.
