医务人员应用放射性口腔黏膜炎循证指南障碍因素的质性研究

Barriers to implementing evidence-based guidelines for Radiation-Induced Oral Mucositis among healthcare professionals: a qualitative study

摘要: 目的:探究医务人员应用放射性口腔黏膜炎循证指南的障碍因素,为指南的应用和推进提供参考。方法:采用描述性质性研究,基于理论域框架构建访谈提纲,于2025年5月以目的抽样法选择湖南省4家三级甲等医院肿瘤科的14名医务人员进行半结构式访谈,采用框架内容分析法分析访谈资料。结果:从9个理论域(知识、技能、自我效能、动力和目标、记忆力/注意力/决策过程、环境和资源、社会影响、情绪、行为规范)中,提炼出7个障碍因素(专业储备不足与外部证据转化的断层、自我效能低下与动力不足和目标不明确、介入时机滞后与患者参与度低和家庭支持不足、资源缺失与环境限制、自上而下的支持体系和跨领域协作不足、医务人员情感耗竭和不良应用体验、流程和具体措施不具象),19个亚主题。结论:医务人员应用放射性口腔黏膜炎循证指南时面临个人、组织和社会层面的多重障碍。应基于此制定多层次、系统化的干预策略,以有效推进放射性口腔黏膜炎的循证管理。

Abstract: Objective: To explore barriers to implementing evidence-based guidelines for Radiation-Induced Oral Mucositis (RIOM) among healthcare professionals, providing insights for guideline implementation and promotion. Methods: This descriptive qualitative study employed semi-structured interviews guided by the Theoretical Domains Framework. Fourteen healthcare professionals from oncology department across four tertiary grade A hospitals in Hunan province were purposively sampled in May 2025. Framework content analysis was used for data analysis. Results: Seven barrier factors and 19 sub-themes were extracted from 9 theoretical domains (knowledge, skills, beliefs about capabilities, motivation and goals, memory/attention and decision processes, environmental context and resources, social influences, emotion, and behavioural norms). The seven identified barriers included: insufficient professional knowledge reserves and gaps in translating external evidence; low self-efficacy, lack of motivation, and unclear goals; delayed intervention timing, low patient engagement, and insufficient family support; lack of resources and environmental constraints; inadequate top-down support systems and insufficient interdisciplinary collaboration; emotional exhaustion and negative implementation experiences of medical professions; and abstract workflows and lack of concrete measures. Conclusion: Healthcare professionals encounter multifaceted barriers at individual, organizational, and societal levels in implementing evidence-based RIOM guidelines. Multi-level, systematic intervention strategies are needed to advance evidence-based RIOM management.
