Humanistic care with family participation for ICU patients

Objective: To explore the effects of humanistic care with family participation for ICU patients. Methods: A total of 155 ICU patients in a hospital were selected by convenience sampling from June 2019 to June 2020. The humanistic care with family participation was implemented in decision-making, behavioral and emotional supports. Interventions involved participating in music therapy, patient treatment, and visiting through WeChat video. Outcomes included the anxiety and depression leveland satisfaction of patients and their families. Results: The scores of anxiety and depression were 38.1±10.3 and 36.9±8.7, respectively. The satisfaction scores of patients and family members were 146.5±12.7 and 105.1±10.4, respectively after discharge. Conclusion: Humanistic care with family participation can relieve the anxiety and depression of patients, and improve the satisfaction level of patients and their families.

Effects of early progressive resistance exercise program on the clinical outcome of patients after pancreatic surgery and its safety evaluation

Objective: To analyze the effects of early progressive resistance exercise program on the clinical outcome of patients after pancreatic surgery and evaluate the safety of the program. Methods: Totally 131 post-pancreatic patients who were treated in the general surgical ICU from January 2019 to December 2020 were selected as study subjects. Among those, 52 who were enrolled from January 2019 to December 2019 served as the control group, while 79 who were enrolled from January 2020 to December 2020 served as the intervention group. Routine post-operative activity guidance was given to the control group, and the early progressive resistance exercise intervention was given to the intervention group. Differences in the activity ability, recovery time of gastrointestinal function, length of stay at ICU and other indicators between the two groups of patients after intervention, as well as the safety of progressive resistance exercise, were evaluated. Results: Compared to the control group, the activity ability of patients in the intervention group was significantly improved on the third and fifth day (t=0.496, P<0.001; t=1.652, P=0.005, respectively). Exhaust time, the first time getting out of bed, and the length of stay at ICU were significantly shorter in the intervention group (t=10.812, P<0.001; t=11.744, P<0.001; Z=-2.171, P=0.030, respectively); Both groups had no exercise-related adverse events during exercise. Conclusion: The early progressive resistance exercise is safe, which can improve the activity ability of patients after pancreatic surgery, promote the recovery of gastrointestinal function, and shorten the length of stay at ICU.

Application of nursing strategy guided by lung ultrasound in respiratory management of patients with mechanical ventilation

Objective: To explore the application effect of nursing strategy guided by lung ultrasound in respiratory management of patients with mechanical ventilation. Methods: A total of 70 patients with mechanical ventilation in a Critical Care Medicine Department from March 2018 to June 2019 were selected and randomly divided into observation group and control group, with 35 cases in each group. The control group was treated with comprehensive chest physiotherapy, while the observation group was given the nursing strategy of goal-oriented comprehensive chest physiotherapy according to the results of lung ultrasound. Lung ultrasound score, diaphragm function, oxygenation index, vital signs, duration of mechanical ventilation and length of stay in ICU were compared between the two groups. Results: The lung ultrasound scores of the two groups showed a downward trend. Compared with the control group, the lung ultrasound scores in the observation group were significantly lower than those in the control group on the 3rd, 4th and 5th days, and the difference was statistically significant (P<0.05). The diaphragmatic excursion of observation group was higher than that of control group on the 5th day. The diaphragm thickening fraction of observation group was significantly higher than that of control group on the 3rd, 4th and 5th days, and the difference was statistically significant (P<0.05). The oxygenation index of the observation group was significantly higher than that of the control group on the 4th and 5th days, and the difference was statistically significant (P<0.05). The respiratory rate in the observation group was lower than that in the control group on the 3rd, 4th and 5th days, and the Pulse Oxygen Saturation was higher on the 2nd, 3rd, 4th and 5th days, the difference was statistically significant (P<0.05). In terms of duration of mechanical ventilation and length of stay in ICU, the observation group was significantly shorter than the control group, and the difference was statistically significant (P<0.05). There was no statistically difference in heart rate and blood pressure between the two groups. Conclusion: The nursing strategy of comprehensive chest physiotherapy guided by lung ultrasound can improve the lung ventilation, diaphragm function and oxygenation of patients with mechanical ventilation, shorten the duration of mechanical ventilation and the length of stay in ICU.

Effects of the early rehabilitation programme based on 4E model on critically ill patients receiving mechanical ventilation

Objective: To explore the effects of the 4E-based ICU early rehabilitation programme on critically ill patients undergoing mechanical ventilation to provide scientific basis for future clinical practice. Methods: Totally 74 critically ill patients given mechanical ventilation admitted to 2 wards in ICU were recruited by convenience sampling. Patients in Ward 1 was assigned to the experimental group (n=38) and patients in Ward 2 was assigned to the control group (n=36). The control group received routine rehabilitation nursing, while the experimental group received the intervention of early rehabilitation programme based on 4E model. Results: Outcomes including the incidence and duration of ICU delirium, muscle strength scores assessed using Medical Research Council Muscle Strength Grading System, the incidence of Intensive Care Unit Acquired Weakness (ICU-AW), mechanically ventilation duration, duration of ICU stay and the total Length Of Stay (LOS) in hospital of the experimental group were significantly better than those of the control group (P<0.05). No severe early rehabilitation- related adverse events were observed in the experimental group. Conclusion: The early rehabilitation programme based on 4E model is a safe, feasible and effective approach, which can not only improve patients' muscle strength and their activities of daily living, but also reduce the incidence of ICU-AW, decrease the incidence and duration of ICU delirium, and shorten the mechanical ventilation time, ICU stay and the total LOS in hospital.

Objective: To explore the advanced training model for ICU nurses and to evaluate the effects of the training. Methods: The advanced training model was developed based on the Benner's career development theory and Hamric theory of advanced nursing practice. Besides, we also combine the three-track five-ladder nursing career ladder system of our hospital in this training model. A five-level advanced training program was implemented in ICU nurses from 2010 to 2020. Results: After the implementation of the advanced training program, the number and proportion of specialist nurses have steadily increased. Various professional teams have been successfully constructed and developed. The scientific research outputs have been increasing year by year. Conclusion: The five-level advanced training program can promote the improvement and professional development of clinical nursing.

Objective: To investigate the current situation of clinical nurses' practice of arterial blood gas analysis procedure in children. Methods: From July 2 to August 31, 2019, a self-designed questionnaire was conducted to investigate the practice status of arterial blood gas analysis procedure among pediatric nurses in 17 tertiary grade A medical institutions nationwide. Results: A total of 1518 questionnaires were retrieved. Totally 67.7% nurses used the professional arterial blood collection device for blood collection, 74.4% nurses preferred the radial artery as the puncture site, 127 (8.4%) nurses suffered needle stick injuries during arterial blood gas analysis procedures in the past year. Conclusion: The standardization of arterial blood gas analysis procedure are necessary. Clinical nurse managers should strengthen the training of arterial blood gas analysis operation knowledge and skills to improve the success rate of arterial blood sampling and ensure the accuracy of detection results.

Objective: To understand the acceptance of illness in patients with Inflammatory Bowel Disease and analyze the influencing factors. Methods: The convenient sampling method was used to select 178 patients with Inflammatory Bowel Disease who were treated in a tertiary grade A hospital, using the general data questionnaire, acceptance of illness scale, resilience scale for Inflammatory Bowel Disease, the cognitive emotion regulation questionnaire-Chinese version, and perceived stigma scale in Inflammatory Bowel Disease. Results: The average disease acceptance score of patients with Inflammatory Bowel Disease was 23.79±7.64, and 30.90% of patients could not accept their disease. The results of multiple linear regression showed that education level, disease severity, positive cognition, and catastrophizing were significant predictors of illness acceptance, explaining 42.20% of the total variation in acceptance of illness. Conclusion: The illness acceptance of patients with Inflammatory Bowel Disease was at a low-medium level. Medical staff should pay close attention to the assessment of the illness acceptance in this population and take precise interventions based on the patient's education, disease severity, positive cognition, and catastrophizing to improve their illness acceptance.

Research on second victim experience and support, and self-compassion in new nurses after patient safety incidents

Objective: To analyze the correlation between the new nurses' second victim experience and support and self-compassion after patient safety incidents. Methods: Totally 510 new nurses from four tertiary grade A hospitals were included from September 2019 to December 2019, and the Chinese version of Second Victim Experience and Support Tool (CSVEST) and Chinese Self-Compassion Scale-Short Form (SCS-SF) were used to collect data. Results: The score of SVEST was 99.68±16.15, the score of SCS-SF was 24.00±6.71, The effects of organizational support, self-compassion and second victim-related distress on turnover intention were 0.482, -0.337 and 0.182, respectively, and on absenteeism were -0.421, -0.261 and 0.678, respectively. Organizational support was positively correlated with second victim-related distress and negatively correlated with self-compassion. Self-compassion was negatively correlated with second victim-related distress. Conclusion: It's necessary to pay more attention to new nurses as second victims, and establish support programs to recover them from the negative impact of patient safety incidents, and finally to ensure medical safety.

Reliability and validity test of Chinese version of Spirituality Index of Well-Being for community elderly

Objective: To explore the reliability and validity of the Spirituality Index of Well-Being (SIWB) in the elderly community in China, and to provide a reliable measurement tool for the study of spiritual well-being of the elderly in China. Methods: The 557 valid questionnaires were collected from the elderly over 60 years old. The reliability and validity of the scale were tested by Cronbach's α coefficient, exploratory factor analysis and confirmatory factor analysis. Results: The Cronbach's α coefficient of SIWB was 0.925, which had good internal consistency. The Item Content Validity Index of the SIWB was 0.800-1.000, the Scale Content Validity Index/Universal Agreement was 0.833, and the Scale-Content Validity Index/Average was 0.976. The results of exploratory factor analysis showed that the scale was a two factor structure, the factor load of each item was greater than 0.50; the results of confirmatory factor analysis showed that: χ2/df=1.72, GFI=0.946, CFI=0.976, RMSEA=0.021, IFI=0.977, AGFI=0.920, PGFI=0.643, PNFI=0.760, indicating that the fitting degree of each index was good. Conclusion: The SIWB is an effective and reliable tool for measuring spiritual well-being, which provides a feasible and effective tool for evaluating the mental health status of the elderly in China in the future.

Objective: To explore the effects of cardiopulmonary exercise rehabilitation on cardiopulmonary function and quality of life in home-based elderly patients with coronary heart disease accompanied by frailty. Methods: The 98 patients in the community health service center of Hanghai east road in Zhengzhou city were divided into two groups according to their residence, the intervention group and the control group with 49 patients in each group. The control group received cardiopulmonary rehabilitation health education, while the intervention group received cardiopulmonary rehabilitation health education and cardiopulmonary exercise rehabilitation. The changes of cardiopulmonary function, frailty and quality of life in the two groups before and after intervention were compared. Results: After intervention, the heart rate, systolic blood pressure, diastolic blood pressure, and cardiac load of the intervention group were significantly improved compared with the control group, and the differences were statistically significant (P<0.05); The forced vital capacity, forced vital capacity in the first second, and the level of one second rate in the intervention group were significantly improved compared with the control group (P<0.05); The scores of frailty in the intervention group were obviously lower than those in the control group, the differences were statistically significant (P<0.05); The scores of total quality of life in the intervention group were obviously higher than those in the control group, the differences were statistically significant (P<0.05). Conclusion: Cardiopulmonary exercise rehabilitation can improve patients' cardiopulmonary function and improve their quality of life.

The experiences of patient with Surgical Site Infection: a Meta-integration of qualitative studies

Objective: To integrate the experience of patients with Surgical Site Infections (SSIs). Methods: Search PubMed, Embase, CINAHL, Web of science, CNKI, Wanfang, SinoMed, etc. for qualitative study on the experience of patients with SSIs. The search time limit is from the establishment of the database to October 2020. The "Australia JBI evidence-based health care center qualitative study quality assessment (2016)" was as a benchmark, and the results were integrated using the Meta-integration method. Results: A total of 12 articles were included and 4 integrated results were synthesized. Integration result 1: SSI affects the normal life of patients, and negatively affects the patients' physical, psychological and social relationships. Integration result 2: The patients expect to be diagnosed before SSI treatment, the difficulties caused by the treatment, and support for life changes after treatment. Integration result 3: Patients lack SSI-related knowledge and expect to be supported by relevant information. Integration result 4: The patients have a positive attitude towards SSI and participate in the treatment and prevention of SSIs. Conclusion: The SSIs brings great pain to the patients, affects their daily life, and requires support from medical staff. Medical staff should attach great importance to infection and provide targeted support to patients.

The effectiveness of maternal voice on procedural pain reduction in neonates: a Meta-analysis

Objective: To analyze the effectiveness of maternal voice for alleviating procedural pain in neonates. Methods: We searched the database of PubMed, Embase, CINAHL, Web of Science, Cochrane Library, CBM, CNKI and VIP, etc. The randomized controlled trials (RCT) and intervention researches of this topic which were published before May 2020 were included. The critical appraisal tool for RCT and quasi-experimental of Joanna Briggs Institute (JBI) was used to evaluate the methodological quality of literature. Meta-analysis was conducted by RevMan5.3 software. Results: A total of six trials were included. During the painful operation, the neonates in maternal voice group scored lower in neonatal infant pain scale, meanwhile scored higher in comfort scale and showed lower in heart rates (P<0.05). No significant difference was observed between the two groups in blood oxygen saturation. After the painful operation, the intervention group still scored lower in pain scale and slower in heart rates (P<0.05). There were no significant differences between the two groups in the score of comfort scale and oxygen saturation. Conclusion: Maternal voice can reduce the pain scores and maintain heart rates in neonates during and after the painful procedure, and improve the comfort during the painful procedure.

Risk factors of delirium in pediatric ICU: a Meta-analysis

Objective: To systematically analyze the risk factors of delirium observed in pediatric ICU so as to provide clinical evidences and guidance for early prevention of such situations. Methods: Studies regarding risk factors of delirium observed in children during their stay in ICU were retrieved and collected from various databases including PubMed, Web of Science, Cochrane Library, Embase, CBMWEB, CNKI, VIP Database and WanFang Database since the time of their construction till July 10, 2020. Two staff were appointed to do the work independently of literature screening, data extracting and quality evaluation in accordance with the criteria. Then all the data were Meta-analyzed through Stats 14.0. Results: In the end 7 papers were included in the study containing 3398 objects in total with 808 cases in the experiment group and 2590 cases in the control group. And 26 risk factors were extracted at last. Meta-analysis showed that developmental retardation, mechanical ventilation, Benzodiazepines and age were major risk factors causing delirium in children admitted to ICU. Conclusion: Based on the existing evidences, children with developmental retardation, receiving mechanical ventilation and Benzodiazepines are more liable to have delirium in ICU. Since the relationship between occurrence of delirium and age is negative, younger patients tend to have higher risks of delirium. The evidences about risk factors of delirium in pediatric ICU patients at present are far from sufficiency with discrepancies of research samples, assessing tools and frequency of assessment among different studies. Thus more quality studies will be needed for further validation.

Quality management of the nucleic acid detection sample collection via upper respiratory tract based on Donabedian structure-process-result theory

Objective: To analyze the overall quality management on COVID-19 nucleic acid specimen collection based on the Donabedian structure-process-result theory. Methods: To identify associated factors related to the quality control of upper respiratory tract nucleic acid collection, to formulate the implementational strategies on sample collection according to Donabedian structure-process-result theory, and to apply these strategies to 1274 cases of suspected COVID-19 individuals and related groups in our hospital from January 23, 2020 to February 17, 2020. Results: The overall positive rate was 2.59% among all screened individuals, with the false negative rate being 0.24%. The rate of the qualified samples remained at 99.68%. Medical staff reported with 100.00% in mastering the operational skills during sample collection, and the rates of correct sampling in terms of location and method were 99.92% and 99.84%, respectively. The qualified rate on bacteriological detection in air and surfaces was 100.00%, and no infected case was reported among medical staff. Conclusion: The collection and implementation strategy of nucleic acid sampling via upper respiratory tract based on Donabedian structure-process-result theory can improve the qualified rate of collected samples, prevent cross infection, therefore provide references for establishment of a scientific and rigorous nucleic acid detection quality management system.

Professional identity and its relationship with emergency attitudes to public health emergencies among undergraduate nursing interns during the outbreak of COVID-19

Objective: To investigate the professional identity and its relationship with emergency attitudes to public health emergencies among nursing undergraduate interns during the outbreak of COVID-19. Methods: Totally 301 undergraduate nursing interns in a tertiary grade A general hospital in Henan Province were included, and the general information questionnaire, Professional Identity Questionnaire of Nursing Student (PIQNS) and the Emergency Attitude Scale for Public Health Emergencies were used for data collection. Results: The average score of PIQNS was 64.04±12.02, and the average item score of the Emergency Attitude Scale for Public Health Emergencies was 3.95±0.54. There was a significant positive correlation between professional identity and emergency attitudes to public health emergencies (r=0.588, P<0.01). The results of hierarchical regression analysis showed that professional identity independently explained 24.8% of the variation in emergency attitudes to public health emergencies. Conclusion: The nursing undergraduate interns had a high level of professional identity during the outbreak of COVID-19, and the emergency attitude of public health emergencies was at a medium level. Professional identity is an important related factor of emergency attitudes to public health emergencies. It is suggested to set up courses on dealing with public health emergency for undergraduate nursing students, and attach importance to the cultivation of professional identity in teaching, so as to improve their emergency attitudes towards public health emergencies.

Objective: To explore the infection status and analyze the characteristics of COVID-19 among medical staff in Wuhan, so as to provide corresponding targeted intervention. Methods: The questionnaire of infection status for COVID-19 medical staff was developed and the convenience sampling method was adopted to distribute the questionnaire through the "Wenjuanxing" online surveys. Totally 105 medical staffs were investigated, and the questionnaires were screened strictly and data were statistically analyzed. Results: The characteristics of COVID-19 among medical staff are mainly in following aspects (P<0.05): infected medical staff with COVID-19 in Wuhan directly contacted with infectious patients before infection, discomforted before illness but insisted on work, the working scene of possible infection was ward, day shift at the time of infection, performed fever symptom at the time of illness, the type of illness was mild, performed anxiety and tension after infection, thought that the department facilities are not in conformity with requirements of respiratory infectious diseases. There were significant differences between the infection of medical staff and age, type of work, hospital and department at the time of infection (P<0.05). Conclusion: In hospital, we should cut off the route of transmission, take protective and isolation measures, reasonably allocate human resources, restrict the stay and visitation, reduce the risk of infection of medical staff, regularly carry out infection prevention training, evaluate and intervene the mental health and increase the organizational support for all medical staff in hospital, and take targeted prevention and intervention measures according to different infection problems of different medical staff.

Objective: To design a computer virtual case system based on nursing clinical thinking training. Methods: Based on the medical information system in the hospital and computer virtual technology, and combined with the Training Outline For New Nurses (Trial), a computer virtual case system based on clinical thinking training for new recruits was constructed, and the clinical nursing of common diseases and the nursing procedure were included in the system. Results: The system is mainly composed of seven operation modules, including the models of nursing evaluation, nursing diagnosis, nursing plan, nursing implementation, nursing evaluation, patient medical records, patients discharged from hospital, and one assessment and evaluation module. There were 35 cases of common clinical diseases were designed in the system at present. Conclusion: The system is reasonable, since the case design is based on real clinical cases, and the nursing procedure is highlighted during the running of the cases. It's useful for new nurses to train their clinical thinking by this system.

The effects of remote online transitional care on cardiac function and quality of life of patients with heart failure

Objective: To explore the effects of remote online transitional care on cardiac function and quality of life of patients with chronic heart failure. Methods: A total of 121 patients with chronic heart failure in a tertiary hospital admitted from January to August 2019 were recruited. Sixty-two patients admitted from January to April were assigned to the the test group, and 59 patients admitted from May to August were in the control group. Patients in both groups received routine heart failure therapy. Usual care was provided to the patients in the control group while patients in the test group received remote online transitional care. Patients in both groups were followed up for 3 months. Outcomes compared in the two groups included New York Heart Association (NYHA) classification, Six Minute Walk Test (6MWT), B-type Brain Natriuretic Peptide (BNP), echocardiography, Minnesota Living with Heart Failure Questionnaire (MLHFQ), satisfaction and engagement of the transitional care. Results: Three months after discharge, the BNP and MLHFQ score in the test group were lower than those in the control group (P<0.05). 6MWT was longer than that in the control group. The improvement rate of NYHA classification, satisfaction and engagement of the transitional care were higher than those in the control group (P<0.05). There was no significant difference in echocardiography between the two groups (P>0.05). In the test group, the BNP and MLHFQ scores at 3 months after discharge were lower than those at admission (P<0.05), 6MWT was longer than that at admission (P<0.05). The improvement rate of NYHA classification and left ventricular ejection fraction were significantly higher than those at admission (P<0.05). Conclusion: Remote online transitional care can improve the cardiac function and the quality of life of patients with chronic heart failure. Patients have higher satisfaction and participation rate.

Conclusion: The training program is scientific and reliable to provide scientific basis for further researches.

Objective: To investigate the professional training and career development needs of digestive endoscopy nurses, so as to provide reference for clarifying the standardized training program for digestive endoscopy nurses and promoting the sustainable development of the profession. Methods: A self-designed Questionnaire on training and career development needs for digestive endoscopy nurses was distributed to nursing staff in digestive endoscopy in 28 provinces, autonomous regions and municipalities to collecte data. Results: A total of 4509 nurses were recruited. For them, 33.40% regarded that the knowledge and skills acquired in school were unable to meet the current job demands, team assistance ability (76.98%), clinical operational ability (76.56%), and communication ability (74.01%) were very important for digestive endoscopy nurses. Totally 80.53% of the participants regarded the most urgent topic for developing the profession was to establish professional training bases. Conclusion: It's necessary to improve the standardized training system for specialized nurses of digestive endoscopy, and to implement the nationwide qualification certification, so as to meet the training and assessment needs of nursing staff at different levels.

Analysis on effects of multi-point practice by nurses in the blood sampling room of a tertiary hospital in Beijing

Objective: To explore the feasibility of multi-point practice in blood sampling room in tertiary hospitals. Methods: By means of convenient sampling method, the blood collection quality (puncture success rate , blood collection accuracy, etc.),patient waiting time, patient satisfaction, nurse labor cost, multi-point nurse income and satisfaction were compared between multi-point licensed nurses and non-multi-point nurses. Results: The labor cost of nurses in the clinic was reduced by 37.31% and the average waiting time of patients in the high peak period was shortened by 7 minutes after the implementation of nurses multi-point practice. Meanwhile, more nurses used their spare time to realize their own value. Their financial income increased by 22.44% and satisfaction increased by 11.52%. Conclusion: The quality and quantity of blood collection is the same as that of other nurses. The patient satisfaction is improved and the labor cost can be effectively reduced by using homogenized management for cost, and reflects the professional value of nurses. The nurses multi-point practice can be popularized in tertiary hospitals.

Objective: To explore the application of penguin training in the standardized training of new nurses in operating rooms, and evaluate the application effects. Methods: Totally 71 standardized training nurses in the operating room from 2016 to 2019 were divided into two groups, including 33 in the observation group and 38 in the control group. Penguin training was adopted in the observation group, while the control group employed the conventional training mode. The evaluation content included four dimensions: response, learning, behavior and outcome. The assessment included satisfaction survey, theoretical and operational assessment, work experience interview and professional attitude assessment. Results: There were no statistically significant differences in the assessment results of theoretical knowledge and skills of new nurses between the two groups (P>0.05). Nurses in the observation group showed higher acceptance of training, less negative work experience, higher peer satisfaction and occupational spirit scale scores than those in the control group (P<0.05). Conclusion: On the premise of ensuring the theoretical and operational ability of new nurses, penguin training can improve the peer satisfaction and professional spirit of new nurses, reduce negative work experience, and help to form a positive and harmonious learning and working environment among nurses.

Application of lean management principles to optimize patient discharge process in hospital

Objective: To apply the lean management principles to patient discharge process and evaluate its influence on shortening the discharge time and enhancing daily discharge checkout rate between 10:00 and 12:00 in hospital. Methods: We first formulated the lean management team, surveyed the current status of the discharge process in targeted hospital, and identified the weak points during the process by following the Value Stream Mapping method. In addition, we employed the "five-why-strategy" to find out the underlying problems in the discharge process, and collaborated with clinical professionals (doctors, nurses, pharmacists, etc.) to achieve consensus on standardized checkout procedures and to specify appropriate locations for take-home-medications, discharge record and other related materials. Results: The discharge time was shortened from 284 to 106 minutes after the lean management principles was applied into practice, and the checkout rate was increased from 13.3% to 53.6% before 10:00, and from 40% to 89.3% before 12:00 (P<0.05). Conclusion: Applying lean management principles to hospital discharge process can significantly reduce the time for discharge and enhance discharge efficiency.

Effects of emotion management training based on Mindfulness Monitoring and Acceptance Theory in psychological intervention during pregnancy

Objective: To analyze the effectiveness of emotional management training during pregnancy based on Mindfulness Monitoring and Acceptance Theory and test its effects on anxiety, depression and emotional self-efficacy. Methods: Totally 124 pregnant women were induded from May 2018 to February 2019, and were randomly assigned into experimental group or control group, 62 cases in each group. The control group received health education and emotional management courses provided by psychological counselors without mindfulness related education and practice, while the experimental group received health education and online emotional management training focusing on mindfulness attention and acceptance. Before and after the intervention, the score of General Anxiety Scale (GAD-7), Patient Health Questionnaire (PHQ-9), and Regulatory Emotional Self-Efficacy Scale (RESE) were compared between two groups. The feelings, experiences and mastery of mindfulness skills of the experimental group were investigated through interviews. Results: The score of GAD-7 and PHQ-9 significantly decreased after intervention in the experimental group compared with control group (P<0.05), while the RESE scores significantly increased after intervention compared with control group (P<0.05). Participants can basically master mindfulness skills to regulate emotions, but there were still some obstacles during intervention period. Conclusion: Emotional management training based on Mindfulness Monitoring and Acceptance Theory can effectively alleviate anxiety and depression of pregnant women and improve self-efficacy in regulating negative emotions.

Effect of maternal heart sound with voice stimulation on physical development of premature infants in the Neonatal Intensive Care Unit

Objective: To investigate the effects of audio stimulation by maternal heart sound and voice verses music therapy on the overall physical development of premature infants in the Neonatal Intensive Care Unit (NICU). Methods: A total of 90 premature infants who met the inclusion and exclusion criteria were randomly and equally divided into three groups: control group (n=30), music therapy group (n=30) and maternal heart sound and voice stimulation group (n=30). Infants in the control group received routine neonatal care, and those in the music therapy group received music therapy in addition to routine care. Premature infants in the maternal heart sound and voice stimulation group received routine neonatal care, and they were also stimulated by the maternal heart sounds along with mother's voice twice a day for a period of 14 days. At the 7th and 14th day of the intervention, the physical development index, physical growth rate, milk intake and the recovery time to the original birth weight were collected and compared among the three groups. Results: There were no significant differences in terms of infants' body length, head circumference and upper arm circumference among the three groups before and after intervention (P>0.05), and no significant difference was identified in terms of their weight at birth (P>0.05). After 7 and 14 days of intervention, the body weight of premature infants in the group with maternal heart sound and voice stimulation was significantly higher than that in the other two groups (P<0.05), and infants' weight in the music therapy group was significantly higher than that in the control group (P<0.05). The weight growth rate of premature infants in the maternal heart sound and voice stimulation group was also significantly greater than that in the other two groups (P<0.05), with the weight growth rate of infants in the music therapy group being greater than that in the control group (P<0.05). There were no significant differences in terms of the body length, head circumference and upper arm circumference growth rate among premature infants in the three groups (P>0.05). In addition, the average daily milk intake in the first week was no different among premature infants in three groups (P>0.05), however, the daily milk intake during the second week in infants with maternal heart sound and voice stimulation was significantly greater than that of the infants in the music therapy group and control group (P<0.05). Infants in the maternal heart sound and voice stimulation group recovered to birth weight significantly faster than those in the music therapy group and the control group (P<0.05), and with the recovery time in infants in the music therapy group being shorter than that of the control group (P<0.05). Conclusion: Compared with music therapy, maternal heart sound combined with voice stimulation can increase premature infants' milk intake during hospitalization, shorten the time needed to recover to their initial birth weight, and increase the weight growth rate, therefore promote overall physical development of premature infants.

This article analyzes the current status of Cancer-Related Cognitive Impairment of breast cancer patients, summarizes and compares the effects of resistance exercise, aerobic exercise, supplementary exercise therapy, and multimodal exercise on the cognitive function of breast cancer patients and changes in biochemical indicators. It will provide references for patients to conduct a more reasonable exercise intervention.

Objective: To explore the effects of narrative nursing on demoralization syndrome and quality of life in cancer patients. Methods: Ninety patients with cancer treated in our hospital's oncology department from May 2019 to October 2019 were selected as the research participants. The patients were randomly divided into observation group and control group. The control group was treated with routine nursing, and the observation group was treated with narrative nursing based on routine nursing. The intervention effect was evaluated by the Demoralization Scale, EORTC QLQ-C30 and nursing satisfaction questionnaire for inpatients. Results: After 2 months of intervention, the scores of the Demoralization Scale in observation group were lower than those in control group, and the differences were statistically significant (P<0.01), and the quality of life in observation group was better than that in control group (P<0.01). Patient satisfaction in observation group was 95.56%, which was significantly better than 82.22% in control group (P<0.05). Conclusion: Applying narrative nursing to the clinical care of cancer patients can reduce the demoralization syndrome of cancer patients and improve the quality of life and satisfaction of patients.
