The effects of remote online transitional care on cardiac function and quality of life of patients with heart failure

Abstract: Objective: To explore the effects of remote online transitional care on cardiac function and quality of life of patients with chronic heart failure. Methods: A total of 121 patients with chronic heart failure in a tertiary hospital admitted from January to August 2019 were recruited. Sixty-two patients admitted from January to April were assigned to the the test group, and 59 patients admitted from May to August were in the control group. Patients in both groups received routine heart failure therapy. Usual care was provided to the patients in the control group while patients in the test group received remote online transitional care. Patients in both groups were followed up for 3 months. Outcomes compared in the two groups included New York Heart Association (NYHA) classification, Six Minute Walk Test (6MWT), B-type Brain Natriuretic Peptide (BNP), echocardiography, Minnesota Living with Heart Failure Questionnaire (MLHFQ), satisfaction and engagement of the transitional care. Results: Three months after discharge, the BNP and MLHFQ score in the test group were lower than those in the control group (P<0.05). 6MWT was longer than that in the control group. The improvement rate of NYHA classification, satisfaction and engagement of the transitional care were higher than those in the control group (P<0.05). There was no significant difference in echocardiography between the two groups (P>0.05). In the test group, the BNP and MLHFQ scores at 3 months after discharge were lower than those at admission (P<0.05), 6MWT was longer than that at admission (P<0.05). The improvement rate of NYHA classification and left ventricular ejection fraction were significantly higher than those at admission (P<0.05). Conclusion: Remote online transitional care can improve the cardiac function and the quality of life of patients with chronic heart failure. Patients have higher satisfaction and participation rate.
