综合心脏康复方案在射血分数保留型心力衰竭患者中的应用

Effect of comprehensive cardiac rehabilitation program on patients with Heart Failure with preserved Ejection Fraction

摘要: 目的:评价综合心脏康复方案对射血分数保留型心力衰竭患者心肺功能和生活质量的影响,为提高患者康复效果提供借鉴。方法:2023年10月至2024年10月,便利选择吉林省某三级甲等医院心内科收治的142例射血分数保留型心力衰竭患者为研究对象,将患者随机分为对照组74例和干预组68例。对照组接受常规心脏康复计划,干预组接受基于吸气肌训练的分阶段渐进式综合心脏康复方案,均进行12周的干预。比较两组患者干预前后吸气肌强度、心肺功能等变化。结果:两组各有65例患者完成研究。干预后,干预组最大吸气压为(57.11±3.31)cmH2O,吸气峰流速度为(3.87±1.14)L/s,高于对照组的(55.08±2.88)cmH2O和(2.91±1.27)L/s(P<0.05);干预组的峰值摄氧量和最大通气量高于对照组,Borg评分低于对照组,差异均具有统计学意义(P<0.05);干预后两组的6分钟步行距离差异无统计学意义(P>0.05)。结论:基于吸气肌训练的综合心脏康复策略增强了射血分数保留型心力衰竭患者的吸气肌力量及耐力,促进了患者心肺功能的改善。

Abstract: Objective: To investigate the effects of comprehensive cardiac rehabilitation on cardiopulmonary function and quality of life in patients with Heart Failure with preserved Ejection Fraction (HFpEF). Methods: A total of 142 patients with HFpEF admitted to the Department of Cardiology of a tertiary hospital in Jilin province were conveniently selected from October 2023 to October 2024. The participants were randomly divided into a comprehensive rehabilitation group of 68 cases and a control group of 74 cases. Those in the control group received only the conventional cardiac rehabilitation program, while those in the intervention group received a gradual and progressive cardiac rehabilitation program based on inspiratory muscle training, with both groups undergoing a 12-week intervention. Changes in inspiratory muscle strength, cardiopulmonary function were compared between the two groups before and after the intervention. Results: Sixty-five patients in each group completed the study. After the intervention, the maximum inspiratory pressure and peak inspiratory flow of the intervention group were (57.11±3.31) cmH2O and (3.87±1.14) L/s, respectively, which were significantly higher than those of the control group [(55.08±2.88) cmH2O and (2.91±1.27) L/s, P<0.05]. Additionally, the intervention group exhibited higher peak oxygen consumption and maximum ventilatory volume, as well as a lower Borg score, compared with the control group, with all differences being statistically significant (P<0.05). However, there was no statistically significant difference in the 6-minute walking distance between the two groups after the intervention (P>0.05). Conclusion: The integrated cardiac rehabilitation strategy based on inspiratory muscle training has significantly improved inspiratory muscle strength and endurance of patients with HFpEF, promoting the improvement of cardiopulmonary function and quality of life.
