数字健康技术驱动下房颤管理护士角色转型的质性研究

Role transition among nurses in atrial fibrillation management driven by digital health technology: a qualitative study

摘要: 目的:探讨数字健康背景下心房颤动(简称房颤)护理管理中护士的角色转型体验、实践困境与支撑需求,为构建适应数字化发展的护理实践体系提供依据。方法:采用描述性现象学研究方法,基于角色理论框架,于2025年1月—6月,通过目的抽样选取武汉市某三级甲等医院心血管内科15名护士及16名房颤患者进行半结构化访谈,同期开展3个焦点小组(28名参与者)讨论和3个月的参与式观察。采用Colaizzi 7步分析法分析资料。结果:共提炼出房颤管理模式重构(分层监测体系、主动式闭环照护流程、远程康复实现连续性服务延伸)、护士角色内涵重构(数据协调者、远程教练、技术赋能者、创新管理者)、数字健康技术整合与推广的实践障碍(系统互操作性不足、数据过载与职责模糊、数字鸿沟导致服务不均)、角色转型下的系统性支撑缺位(个性化培训体系缺失、多维度考核激励机制欠缺及跨学科协作网络不健全)4个主题及13个亚主题。结论:数字健康技术通过重塑房颤管理模式驱动了护士管理角色的拓展。需要构建支撑体系,通过角色澄清与赋能,化解实践困境。

Abstract: Objective: To explore the role transition experiences, practical challenges, and support needs of nurses in atrial fibrillation management within the context of digital health, in order to provide references for developing a nursing practice system adapted to the digital era. Methods: A descriptive phenomenological study was conducted, guided by the role theory. Between January and June 2025, 15 nurses and 16 patients with atrial fibrillation were purposively sampled from the cardiology department of a tertiary grade A hospital in Wuhan for semi-structured interviews. Additionally, three focus group discussions (with 28 participants) and a three-month period of participatory observation were conducted. Data were analyzed using Colaizzi's seven-step phenomenological analysis. Results: Four integrated themes were identified: model reconstruction of atrial fibrillation (risk-stratified monitoring, proactive closed-loop care pathway, and remote service extension), role reconstruction (data coordinators, remote coaches, technology enablers, and innovation managers), practical barriers to integrating and promoting digital health technology (insufficient system interoperability, data overload coupled with role ambiguity, and the digital divide), support deficits (lack of personalized training, multidimensional performance incentives, and interdisciplinary collaboration networks). Conclusion: Digital health technologies have driven the expansion of nurses' managerial roles by reshaping the model of atrial fibrillation management. Support systems need to be established to resolve practical dilemmas through role clarification and empowerment.
