手术室医护人员骨科俯卧位手术体位管理困境的质性研究

Challenges of prone position management among operating room healthcare professionals in orthopedic surgery: a qualitative study

摘要: 目的:了解手术室医护人员在骨科俯卧位手术体位管理中的困境,为改善骨科手术护理质量提供参考。方法:采用目的抽样法,于2025年3月—5月选择湖北省某三级甲等医院11名手术室骨科专科护士、2名骨科医生、2名麻醉医生为访谈对象,进行半结构访谈。采用主题分析法进行资料分析。结果:共提炼4个主题和6个亚主题,即手术团队专业素养不足(技能与经验欠缺、团队协作沟通不畅)、高风险患者体位管理难度高、设备与技术支持受限(俯卧位用具受限、实时监测技术受限)、规范不够完善与流程执行不到位(操作标准不明确、评估与反馈不到位)。结论:手术室医护人员在骨科俯卧位手术体位管理方面存在多重困境与挑战,与医护人员专业能力、患者个体差异、设备技术条件、流程执行等关键因素有关。

Abstract: Objective: To explore the dilemmas faced by operating room medical staff in the position management of orthopedic surgery in prone position, and to provide references for improving the nursing quality of orthopedic surgery. Methods: Purposive sampling method was used to select 11 orthopaedic specialist nurses, 2 orthopaedic surgeons, and 2 anaesthesiologists in the operating theatre of a tertiary grade A hospital in Hubei province from March to May 2025 for semi-structured interviews. Thematic analysis was used to analyse the data. Results: Four main themes and six subthemes were identified, including insufficient professional competence of the surgical team (lack of skills and experience, poor team communication and coordination), difficulties in positioning high-risk patients, limitations in equipment and technical support (constraints of prone positioning devices, limited real-time monitoring technologies), and inadequate implementation of standards and procedures (unclear operational guidelines, insufficient assessment and feedback). Conclusion: Medical staff in operating rooms face multiple dilemmas and challenges in managing the prone position during orthopedic surgeries, which are related to key factors including the professional incompetence of medical personnel, individual patient variations, equipment and technical conditions, and process execution.
