Factors influencing cognitive training participation in patients with mild cognitive impairment: a qualitative study

摘要: 目的:探究影响轻度认知障碍患者参与认知训练的相关因素,为构建有效的实施策略提供实践依据。方法:采用目的抽样法,选取2024年9月至2025年2月在南京市某三级甲等医院认知中心和认知训练护理门诊就诊以及社区筛查的17例轻度认知障碍患者,进行半结构式访谈,基于多理论模型,运用内容分析法分析访谈资料。结果:共提炼出参与意愿、行为启动、行为维持3个主题和10个亚主题(疾病风险感知、感知训练益处、行为自信、资源的获取与利用障碍、经济成本与价值权衡、积极的情感转变、行为实践中的自我调节、社会环境支持、时间偏好、衰弱与共病的现实限制)。结论:轻度认知障碍患者参与认知训练受到多因素影响,医护人员应根据影响因素优化实施策略,助力认知训练落地。

Abstract: Objective: To explore factors influencing cognitive training participation among patients with mild cognitive impairment, and to provide practical evidence for constructing effective training implementation strategies. Methods: Purposeful sampling was used to select 17 patients with mild cognitive impairment from September 2024 to February 2025. These individuals were recruited from the cognitive center, and cognitive training nursing clinic of a tertiary grade A hospital in Nanjing, as well as through community screening. Semi-structured interviews were conducted, and content analysis was used based on a multi-theory model. Results: Three themes (participation intention, behavioral initiation, and behavioral maintenance) and ten subthemes (perception of disease risk, perceived benefits of training, behavioral self-efficacy, barriers to access and utilization of resources, trade-offs between economic costs and value, positive emotional transformation, self-regulation in behavioral practice, social environmental support, time preference, and practical limitations caused by frailty and comorbidities) were extracted. Conclusion: The participation of patients with mild cognitive impairment in cognitive training is affected by a complex interplay of multiple factors. Medical staff should optimize implementation strategies based on these influencing factors to help transform cognitive training from effective to practical implementation.
