血液净化用中心静脉导管相关血流感染过程质量控制工具包的构建

Development of a process quality control toolkit for preventing central venous Catheter-Related Bloodstream Infections in blood purification

摘要: 目的:构建血液净化用中心静脉导管相关血流感染过程质量控制工具包,为降低感染发生率、提升专科护理质量提供参考。方法:通过文献回顾,构建“预防血液净化用中心静脉导管相关血流感染过程质量控制工具包”初稿。2025年9月、11月,采用德尔菲专家函询法对30名专家开展2轮函询,根据函询结果修订工具包。2026年1月—3月,在广东省44家医院开展临床试用,结合临床反馈进一步完善,形成工具包终稿。结果:2轮函询问卷回收率均为100%,专家权威系数分别为0.927和0.933,肯德尔和谐系数分别为0.274和0.361(均P<0.001),临床试用收集到11条反馈意见,最终形成的的工具包终稿涵盖组织与管理、导管置管操作核心防控原则、导管操作前评估等8个维度,共计55条护理措施。结论:本研究构建的工具包具备良好的科学性及可靠性,内容全面,可为血液净化用中心静脉导管规范化管理提供实用工具。

Abstract: Objective: To develop a process quality control toolkit for the prevention of central venous Catheter-Related Bloodstream Infections in blood purification, in order to provide a reference for reducing infection incidence and improving specialized nursing quality. Methods: The initial draft of the Process Quality Control Toolkit for Preventing Central Venous Catheter-Related Bloodstream Infections in Blood Purification was formulated based on a literature review. Two rounds of Delphi expert consultations were conducted among 30 experts in September and November 2025, respectively. The toolkit was revised according to the results of expert consultations. A pilot clinical implementation was carried out in 44 hospitals in Guangdong province from January to March 2026. The toolkit was further refined with clinical feedback to form the final version. Results: The questionnaire response rate of both rounds of consultation reached 100%. The expert authority coefficients were 0.927 and 0.933, and Kendall's coefficients of concordance were 0.274 and 0.361, respectively (all P<0.001). A total of 11 pieces of feedback were collected during the pilot clinical implementation. The final toolkit covered 8 dimensions, such as organization and management, core prevention and control principles for catheter-related operations, and pre-procedure assessment for catheter-related operations, involving 55 nursing measures in total. Conclusion: The developed toolkit has favorable scientificity and reliability with comprehensive contents, which can serve as a practical tool for standardized management of central venous catheters for blood purification.
