Design and implementation of the "Life Care and Death Education" course for medical students

Objective: To design and conduct a course on "Life Care and Death Education" based on the needs of medical students, and evaluate its effects, with a view to providing a reference for other medical schools in China to carry out life and death education programs. Methods: Taking the "perimortem period" as the time frame, the main line of the course ran through theoretical contents and practical thematic activities. Through the "walking classroom" experiential practical teaching method, the program led the medical students to face the real scenarios of life and death directly, to improve the concepts of caring and to generate caring actions. Results: The course included "life and care" and "death and farewell" two theoretical topics, and "journey of life", "power of care", "self-care", "thoughts on death" and "the day of farewell" five practical topics. The initial practical results of the course were good, and the course was highly positively evaluated by a wide range of stakeholders. Conclusion: The course is reasonably designed, with a clear and explicit implementation plan, which to a certain extent meets the learning needs of medical students for life and death education, promotes the self-knowledge of medical students on life and death issues in the ideology and the self-consciousness of caregiving actions, and has a strong generalizability, and is able to provide suggestions for the development of life and death education courses in various institutions of higher learning.

Evidence summary for shared-care management of adult hospice care

Objective: To extract and summarize the evidence related to hospice shared-care management in adults, and provide an evidence-based support for healthcare professionals in the development of hospice shared-care for end-of-life patients. Methods: We systematically searched Chinese and English databases to collect all evidences about the hospice shared-care management in adults, the search time was limited from the establishment of the databases to November, 2023. Researchers assessed the quality of the literature, reviewed and extracted the evidence, and integrated the evidence after discussion by the evidence-based research group. Results: Totally 14 articles were involved, including 3 clinical decisions, 6 guidelines, 1 expert consensus and 4 quasi-experimental studies. Finally, 25 pieces of evidence have been obtained from 8 aspects, including the team composition, applicable population, service process, service contents, timing of initiation and duration of care, precautions, evaluation of effectiveness, quality control. Conclusion: When applying the evidence, clinical healthcare professionals should take into account the cultural context, make localized adaptations and pay attention to the updating of the evidence, in order to provide patients with standardized and regulated hospice shared-care services.

Hospice care is a medical service full of humanity, and it is an important livelihood cause that benefits the country, the people, the society and the hospital. Nurses should pay attention to medical, policy, cultural, psychological, ethical and legal care exploration in the interdisciplinary professional service of hospice care, and implement the comprehensive practice of "five combinations" that is national conditions, reasonable, legal, compliant and appropriate, which is conducive to providing people-oriented professional and high-quality services, achieving rational compliance, laws to follow and rules to abide by, and promoting the development of hospice care with local cultural characteristics in line with China's national condition and public opinion.

Dilemmas and challenges of signing living will from stakeholders' perspective

Objective: To analyze the dilemmas and challenges faced by various stakeholder groups in the process of signing living wills, and to provide reference for optimizing the signing strategy of living wills. Methods: This study adopted a phenomenological research method to conduct semi-structured interviews with a total of 33 people from Shenzhen city from July to November 2023, including (potential) signers of living wills, family members, healthcare professionals, social workers, and volunteers, and analyzed the data by using the Colaizzi 7-step analysis method. Results: Three themes and six sub-themes were identified: spatial mismatch (outside the hospital, inside the hospital), temporal mismatch (it is too early, it is too late), and subject mismatch (family members "overstepping the bounds", patients "giving way"). Conclusion: The mismatch of the three dimensions of place, time, and subject of decision-making in the practice of signing living wills has led to the cold reception of living wills in the promotion and implementation of living wills. It is suggested that in the future, it is necessary to build a communication platform for all stakeholder groups, strengthen life education, and establish the concept of living wills among the public, so as to promote the consensus related to the signing of living wills.

Objective: To understand the experience of clinical nurses on implementing narrative nursing for terminal patients in combination with Chinese traditional culture, and to provide reference for the clinical promotion of localized narrative nursing. Methods: Using snowball sampling method, 12 clinical nurses with narrative nursing experience in terminal patients were selected for semi-structured interviews, and the transcripts were analyzed by theme analysis method. Results: In the process of implementing localized narrative nursing for terminal patients, the experience of clinical nurses included four themes: clinical revolution, practice course, self-benefit, and dilemma and challenge. Conclusion: The implementation of localized narrative nursing for terminal patients not only alleviates the pain of patients, but also enhances the professional value of clinical nurses. However, narrative nursing practice also faces many challenges, and the support from management and organization level is needed to ensure the sustainable development of localized narrative nursing practice.

Objective: To conduct a scoping review of relevant studies on death education for clinical nurses in China, and to provide a reference basis for the research and practice of death education for clinical nurses in China. Methods: In accordance with the research methodology of the scoping review, relevant Chinese and English databases were systematically searched, and literature screening and extraction were completed by two researchers according to the inclusion and exclusion criteria, and the extraction results were collated using descriptive and thematic analyses. Results: A total of 1678 studies were searched and 19 studies were finally included. The studies were mainly cross-sectional and quasi-experimental, and the main findings included the need for death education, the content of death education, the implementation methods of death education. Conclusion: At present, death education for clinical nurses in China is still in the stage of practical exploration, and in the future, we should learn from foreign research results and experiences, further pay attention to the practicality of the content of death education, enrich the way of carrying out death education, and improve the tools for research on death education, so as to promote the sustainable development of death education for clinical nurses in China.

Supportive care needs of cervical cancer patients undergoing radical surgery based on timing theory: a phenomenological study

Objective: To explore the supportive care needs of patients with cervical cancer at different stages after radical operation, so as to provide basis for developing demand-oriented nursing services. Methods: The phenomenological research approach was used from May to November 2021 to conduct semi-structured interviews with 16 participants. Three semi-structured interviews were conducted at 7 days after operation (adjustment period), 3 months after operation (adaptation period) and 6 months after operation (stable period). Colaizzi 7-step method was used to analysis the data, and the supportive care demand multidimensional model was used for classification. Results: A total of five themes were summarized to describe the need of cervical cancer patients undergoing radical surgery, including the symptoms of discomfort and restoring daily life, negative psychological feelings, multifaceted support and continuation of medical service needs, insufficient information support and sexual needs. Conclusion: The supportive care needs of patients undergoing radical resection of cervical cancer have different characteristics with the course of disease. Medical staff should adopt the concept of dynamic and continuous, scientifically evaluate the needs of patients, and then implement accurate nursing services to improve patients' disease awareness and coping ability.

Quality of life status and influencing factors of adult patients with Pulmonary Hypertension

Objective: To analyze the quality of life status and influencing factors of adult patients with Pulmonary Hypertension (PH), and to provide a reference for the construction of targeted quality of life intervention programs. Methods: A total of 247 PH patients who visited the department of cardiology of a tertiary grade A hospital in Jinan from July 2021 to February 2022 were selected as subjects by convenience sampling method. The General Data Questionnaire, the World Health Organization Quality of Life Questionnaire, the Chinese version of the Simplified Fear Disease Progression Scale, the Perceptive Social Support Scale, and the simplified Chinese version of the Family Resilience Rating Scale were used to survey. Results: The average quality of life score of PH patients was 46.55±10.46 and the scores in four domains were lower than the norm. The results of stratified linear regression showed that the cardiac function, per capita monthly household income, fear of disease progression, perceived social support and family resilience were influencing factors of the quality of life of PH patients (P<0.05). Conclusion: The quality of life of PH patients is seriously impaired. Medical staff should pay more attention to PH patients with poor cardiac function, lower income, and fear of disease progression, help them develop positive and correct disease perception, improve social support and help their families to leverage the inherent advantages and strength, to improve the health level and quality of life of PH patients.

Objective: To analyze the current situation and influencing factors of Peristomal Moisture-Associated Skin Damage (PMASD) around preventive ileostomy, in order to prevent the occurrence and development of PMASD in the early stage, and provide guidance for adopting predictive and targeted nursing measures. Methods: A total of 388 patients who underwent preventive ileostomy in 2 hospitals in Hangzhou from March 2022 to December 2022 were selected by convenience sampling method as research objects. The researchers conducted face-to-face, telephone and cloud platform investigation and follow-up, and the ostomy specialist nurses combined the inquiry results and uploaded photos to diagnose whether PMASD occurred. Finally, univariate analysis and Logistic regression were used to analyze the influencing factors of PMASD around preventive ileostomy. Results: The incidence of PMASD in patients with preventive ileostomy was 36.3%. Logistic regression analysis results showed that age, BMI, surgical wound at the ileostomy, skin folds around the ileostomy, caregiver change, type of stoma bag, and stoma bag replacement period were influencing factors of PMASD around preventive ileostomy. Conclusion: The incidence of preventive ileostomy PMASD is high. Nurses should provide personalized nursing intervention guidance to patients regarding stoma wound and skin folds and stoma products, and actively encourage patients to improve stoma self-care ability and prevent PMASD from occurring.

The training status of new nurses in Intensive Care Units in 30 tertiary hospitals

Objective: To investigate the current status of nurse's induction training in Intensive Care Units (ICUs) conducted in 30 tertiary hospitals. Methods: From July to August 2022, 583 ICU nurses from 57 ICUs belonging to 30 tertiary hospitals in 11 provinces across China were investigated by convenient sampling. A self-designed questionnaire established using the Delphi method was adopted for investigation. Results: Among the 57 ICUs surveyed, all the enrolled ICUs had induction training programmes for new nurses and stipulated that they could work independently after the training period. However, the training forms were diverse and the duration of the training period was not uniform, and only 32 (56.1%) required entry qualifications for new nurses. Totally 91.2% of the head nurses tended to recruit nurses who had received standardized training. The proportion of new nurses in the total number of nurses was positively correlated with the incidence of unplanned extubation (r=0.341, P<0.05). Among the 583 nurses surveyed, 364 (62.4%) believed that 3-6 months of training time was the most appropriate, and 339 (58.1%) nurses were inclined to master general nursing skills before entering ICU. Regression analysis showed that the influencing factors of ICU nurses' adaptation to work stress during induction training were the type of ICU, the form of clinical practice training, and the length of their training. Conclusion: At present, there is a lack of ICU qualification access system and no unified scheme for induction training. The training duration still needs to be further discussed, and programmatic documents are still needed to guide the implementation of induction training for ICU nurses in the future.

Development and validation of health belief questionnaire for phosphorus control behaviors in hemodialysis patients

Objective: To develop a questionnaire on the health belief for blood phosphorus control behaviors in hemodialysis patients and to test its reliability and validity. Methods: According to the Health Belief Model, through literature review and focus interviews with experts, a preliminary item pool was formed. Delphi method was applied to conduct two rounds of expert consultation. The contents of the questionnaire were adjusted according to the opinions of experts. The preliminary questionnaire was used to investigate 308 hemodialysis patients. Item analysis, reliability test, validity test and exploratory factor analysis were carried out. Twenty patients were selected to retest reliability one week later. Results: The response rate of the two rounds' consultation was 100%, Cr was 0.905. The Kendall coordination coefficient were 0.228, 0.352 respectively (P<0.05). The final version of the phosphorus control health belief questionnaire for hemodialysis patients included 26 items, and the exploratory factor analysis produced 3 factors, with a cumulative variance contribution rate of 71.294%. The I-CVI of the questionnaire ranged from 0.8 to 1.0, S-CVI was 0.852. The coefficient of Cronbach's α of the total questionnaire was 0.911, and the retest reliability was 0.866. Conclusion: The questionnaire has good reliability and validity, and is suitable for evaluating health belief of blood phosphorus control behavior in hemodialysis patients.

Objective: To search, evaluate and summarize currently available state-of-the-art studies on the perioperative nutrition management of adult liver transplantation, and to provide evidence-based basis for clinical nutrition strategy development and patients' self-management. Methods: The literature on perioperative nutrition management of adult liver transplantation was systematically searched, evaluated, and extracted from the domestic and foreign databases according to the evaluation criteria. Results: A total of 11 literatures, consisting of 7 guidelines, 3 expert consensus, and 1 systematic review were included. Finally, 20 best evidences and 6 categories were summarized, including nutritional risk screening and assessment, sarcopenia assessment, weight management, preoperative nutrition management, postoperative nutrition management, and follow-up for discharged patients. Conclusion: Our study scientifically and practically summarizes the best evidence of perioperative nutrition management of adult liver transplantation, which provides implications for clinical medical staff and guides targeted interventions for the self-management of patients after liver transplantation.

Effects of exercise on patients with epilepsy: a Meta-analysis

Objective: To conduct a Meta-analysis assessing the intervention effects of exercise participation in patients with epilepsy, aiming to provide evidence-based guidance for clinical practice. Methods: We searched randomized controlled trials on epilepsy patients having exercise from PubMed, Embase, Cochrane Library, Web of Science databases, CNKI, Wanfang database, VIP and CBM from their inception to September 29, 2023. Meta-analysis was performed by RevMan 5.4. Results: A total of 9 studies involving 400 patients were finally included. The results showed that exercise reduced seizure frequency [MD=-2.35, 95%CI(-3.16, -1.54), P<0.001], without an increased occurrence of adverse events [RR=2.54, 95%CI (0.11, 56.25), P=0.56]. Furthermore, it could enhance the daily activity levels of patients [MD=1.39, 95%CI (0.48, 2.29), P=0.003], ameliorate depressive symptoms [SMD=0.90, 95%CI (0.22, 1.58), P=0.01], improve cognitive function [SMD=0.69, 95%CI (0.03, 1.35), P=0.04], and elevate the quality of life [SMD=0.43, 95% CI (0.22, 0.64), P<0.001]. However, there was no statistically significant difference in improving anxiety symptoms [SMD=-1.27, 95%CI (-3.00, 0.46), P=0.15]. Conclusion: The implementation of a scientifically developed exercise in patients with epilepsy was safe and effective in relieving depressive symptoms, improving cognitive function and life quality, but was not that obvious in reducing their anxiety symptoms.

Risk factors for intraoperative pressure injury in patients undergoing posterior spinal surgery: a Meta-analysis

Objective: To systematically evaluate the risk factors for intraoperative pressure injury in patients undergoing posterior spinal surgery, and to provide a theoretical basis for clinical prevention. Methods: Chinese and English databases were searched from the establishment of the databases to December 31, 2022 to collect literature on risk factors of intraoperative pressure injury in patients undergoing posterior spinal surgery. After literature screening, data extraction and quality evaluation, Meta-analysis was performed using RevMan 5.4. Results: A total of 10 articles were included, with 6402 patients. The results showed that the incidence of intraoperative pressure injury in patients undergoing posterior spinal surgery was 13%. BMI≥28 kg/m2 [OR=1.91, 95%CI (1.06-3.41)], diabetes [OR=2.48, 95%CI (1.56-3.95)], duration of anesthesia≥4 h [OR=13.60, 95%CI (6.30-29.34)], operation time≥4 h [OR=2.96, 95%CI (1.97-4.45)] were risk factors for intraoperative pressure injury in patients undergoing posterior spinal surgery. Conclusion: The incidence of intraoperative pressure injury in patients undergoing posterior spinal surgery is high, but the research on its risk factors is still limited. Comprehensive investigations of risk factors with high quality and multiple aspects are needed to provide intervention targets for future preventive measures, correct variable influencing factors, and coordinate doctors, nurses and anesthesiologists, in order to reduce the occurrence of intraoperative pressure injury.

Risk prediction models for Venous Thromboembolism recurrence: a systematic review

Objective: To systematically evaluate Venous Thromboembolism (VTE) recurrence prediction models and provide reference for the development of related prediction models and clinical practice. Methods: We comprehensively searched Chinese and English databases for studies related to VTE recurrence risk prediction models from inception of databases to April 1, 2023. Studies were independently screened, relevant data were extracted by 2 researchers and the quality of the included studies was evaluated using PROBAST. Results: A total of 9 articles were select, including 10 VTE recurrence risk prediction models. Among them, nine are the development of models, one is update of model. Meta-analysis results showed that men [OR=1.88, 95%CI (1.66, 2.13), Z=9.84, P<0.001], elevated D-dimer [OR=2.13, 95%CI (1.51, 3.00), Z=4.30, P<0.001], proximal deep vein embolism [OR=1.64, 95%CI (1.35, 2.00), Z=4.87, P<0.001] and pulmonary embolism [OR=2.69, 95%CI (1.98, 3.65), Z=6.36, P<0.001] were high risk factors for recurrence of VTE. Conclusion: The prediction model for VTE recurrence still needs to be developed, and in the future, a prediction model with low risk of bias and high applicability should be selected or developed according to the actual clinical situation, and its prediction effect should be monitored. Meanwhile, medical staff should pay attention to those patients who are male, have elevated D-dimer, proximal deep vein embolism, and pulmonary embolism, and take reasonable preventive measures for them to prevent recurrence of VTE.

Evidence summary for self-management of adolescents with epilepsy in transition readiness from adolescents to adults

Objective: To summarize the relevant evidence of self-management of adolescents with epilepsy in transition readiness from adolescents to adults. Methods: Clinical practice guidelines, best practice, recommended practice, systematic review, evidence summary, expert consensus, etc, on self-management adolescents with epilepsy in transition readiness were searched. The search period is from database establishment to May 10, 2023. After methodological quality was assessed, evidence was extracted and summarized. Results: Nineteen articles were included, including five guidelines, six systematic reviews, seven expert consensus, and one recommended practice. Finally, a total of 34 pieces of evidence were summarized, including 3 aspects and 9 categories of management principles, environmental support (joint participation, platform construction), behavioral support (transition preparation record, antiepileptic drug compliance skills, lifestyle skills, out-of-hospital AS seizure skills), and cognitive support (patient education, positive psychology, shared decision-making). Conclusion: The evidence summary in this study is comprehensive, scientific, and practical, and provide reference for supporting self-management for adolescents with chronic diseases.

Prevalence and influencing factors for return to work in patients with coronary heart disease: a Meta-analysis

Objective: To explore the prevalence and main influencing factors of return to work (RTW) in patients with coronary heart disease (CHD), so as to provide reference for nursing interventions. Methods: Databases including the English and Chinese databases from inception to February 2023 were retrieved to search for studies on the prevalence and influencing factors of RTW in patients with CHD. Meta-analysis was performed by RevMan 5.3 and StataMP 17.0 software. Results: A total of 10 literatures were enrolled, involving 11394 subjects. Meta-analysis results showed that the prevalence of RTW in coronary heart disease was 58% [95% CI (0.47-0.69) ]. Sex [OR=2.37, 95% CI (1.65-3.40) ], age [OR=1.26, 95% CI (1.07-1.47) ], type of occupation [OR=5.91, 95% CI (3.65-9.57) ], number of diseased vessels ([OR=0.16, 95% CI (0.05-0.46) ], fear of disease progression [OR=0.87, 95% CI (0.82-0.91) ], social support [OR=1.13, 95% CI (1.07-1.18) ] were its influencing factors. Conclusion: The incidence of patients with coronary heart disease returning to work is low and affected by many factors, medical staff should fully pay attention to the above high-risk groups, and give timely intervention and support.

Objective: To construct an acute pain intelligent management platform for inpatients based on Clinical Care Classification (CCC) System and evaluate its application effect, and to provide references for improving the quality of pain management for inpatients. Methods: Based on the knowledge base and coding database of the Chinese version of CCC 2.5, an intelligent acute pain management platform for inpatients was developed, including intelligent recommendation of assessment tools, early warning of high scores, scheduling of nursing tasks, automatic triggering of individualized nursing diagnosis and nursing measures, etc, and was put into trial operation in April 2022. The pain management indicators and satisfaction score were compared before and after the launch of platform. Results: After using the platform, the coverage rate of the first pain assessment, the rate of dynamic pain assessment and the rate of timely pain management for inpatients were all higher than before (P<0.05). Nurses' satisfaction with information system and patients' satisfaction with pain management also were higher than before (P<0.05). Conclusion: The application of acute pain intelligent management platform for inpatients based on CCC System is helpful to improve the pain management effects, patients' satisfaction and nurses' work efficiency, and this platform is with great clinical promotion value.

Objective: To build the automatic management system for outpatient nursing, and evaluate its effect on nursing management, to provide reference for nursing managers to enhance their management efficiency and service quality. Methods: The automated management system was established. It contains four major modules including automated staff scheduling, workload analysis, performance analysis, and salary distribution. The four modules interconnect well with each other and has been applied to practice. Indicators like nurses' overtime cost, the average time spent on performance analysis, salary distribution and staff scheduling, and patients' satisfaction before and after the application of the system have been compared. Results: The overtime hours reduced by 71.60%, average time spent on performance analysis, salary allocation, and scheduling of nurses have significantly decreased after the application with improved satisfaction of both nurses and patients. All differences are statistically significant (P<0.01). Conclusion: The application of the automatic management software system and the formation of a good management orientation can effectively improve human resources and nursing management leading to better satisfaction of patients and nurses to provide reference for nursing managers to enhance their management efficiency and service quality.

Objective: To investigate the status of mindfulness and mental health of front-line nurses during public health emergencies, and to explore the mediating effect of emotional intelligence between mindfulness and mental health of front-line nurses. Methods: Convenience sampling was used to recruit 215 front-line nurses from a tertiary grade A hospital in Hohhot in January 2023. Participants were investigated by the self-designed general data questionnaire, 21-Item Depression Anxiety Stress Scale (DASS-21), Mindfulness Attention Awareness Scale (MAAS) and Wong and Law's Emotional Intelligence Scale (WLEIS). Results: Depression-anxiety-stress score was negatively correlated with mindfulness and emotional intelligence score (P<0.01). The results of Bootstrap test showed that the mediating effect of emotional intelligence score between mindfulness score and depression-anxiety-stress score in front-line nurses was -0.146 (95%CI: from -0.219 to -0.083), accounting for 44.0% of the total effects. Conclusion: Emotional intelligence has a partial mediating effect between mindfulness and mental health of front-line nurses. Hospital managers should pay attention to improving nurses' mental health and strengthening their ability to cope with public health emergencies.

Objective: To investigate the knowledge, attitude and practice in continuous nursing of patients with Diabetic Peripheral Neuropathy (DPN) among nurses in pain department, and analyze the influencing factors, to provid a basis for formulating the patient's continuous nursing plan and improving the quality of continuous nursing. Methods: A total of 561 nurses from the pain department in hospitals at all levels in China were selected by convenient sampling, and a self-made questionnaire was used to collect the data online. Results: The scores of knowledge, attitude and practice in continuous nursing of patients with DPN among nurses in pain department were 42.17±4.17, 34.95±4.12, and 33.68±5.21, respectively. The results of univariate analysis showed that whether nurses received continuous nursing training, whether departments provided continuous nursing norms, the frequency of nurses providing continuous nursing and the frequency of inter-disciplinary communication had a statistically significant impact on knowledge, attitude and practice (P<0.05). Conclusion: Pain department nurses have positive attitudes on DPN continuous nursing, but their knowledge and behaviour are relative poor, needs to be improved. It is recommended to formulate the standards of continuous nursing and strengthen the special training of nurses to improve the quality of continuous nursing.

Objective: To explore the project management mode of multi-center randomized controlled trial of nursing intervention and provide experience for clinical nursing research training and practice. Methods: According to the clinical trial management standard, this study elaborated the project management mode of multi-center nursing intervention randomized controlled trial from the four stages of project evaluation, planning, implementation and evaluation. Patient recruitment rate, retention rate, nurse participation retention rate, and training compliance were used to evaluate the feasibility of the project management mode and the effect of scientific research training in the randomized controlled trial of multi-center nursing intervention. Results: The patient recruitment rate was 73.3%, the retention rate of patients in the intervention group was 96.4%, the retention rate of nurse participation was 89.3%, the training compliance was 78.6%. Conclusion: The project management mode of multi-center randomized controlled trial about the family intervention for breast cancer patients is feasible, which provides an empirical reference for clinical nurses and nursing graduate students to design and implement multi-center randomized controlled trial. Meanwhile, the practice-oriented research training can stimulate nurses' interest in research and improve their research ability.

Objective: To investigate the application status and management problems of Totally Implantable Vennous Access Ports (TIVAP), and propose the intervention strategies, so as to provide reference for scientific and safe management of TIVAP. Methods: A total of 56 hospitals in 14 cities/states in Hunan province were included by convenience sampling method from October to November 2022 to investigate the application and management of TIVAP. Results: Twenty hospitals (35.7%) carried out TIVAP implantation procedure, and 47 hospitals (83.9%) implemented the maintenance of TIVAP. In terms of TIVAP management, 39 hospitals (83.0%) set up intravenous therapy management team, 14 hospitals (29.8%) carried out data monitoring for TIVAP implantation, and the difference between secondary and tertiary hospitals was statistically significant (P<0.05). Forty hospitals (85.1%) provided regular TIVAP training for specialist nurses, and only 17 hospitals (36.2%) regularly conducted quality supervision on the use of TIVAP. Conclusion: The TIVAP management system in hospitals at all levels in Hunan province needs to be improved, including standardize the maintenance process, strengthen the information management and the training of specialist personnel, so as to improve the the maintenance and management quality of TIVAP.

Objective: To construct a comprehensive nursing management program for day surgery in patients undergoing robotic-assisted radical prostatectomy and evaluate its safety and feasibility. Methods: According to the inclusion and exclusion criteria, we recruited patients admitted from January to December, 2021, as the control group, and patients admitted from January to December, 2022, as the experimental group, respectively. The experimental group adopted the comprehensive rehabilitation nursing scheme, while the control group received routine nursing care. Indicators including the proportion of patients discharged within 48 hours, the time needed to have the first ambulation, the incidence of orthostatic intolerance and surgical complications, unplanned readmission within 30d after discharge and incidence of moderate to severe urinary incontinence 1 month after surgery, were compared and analyzed. Results: A total of 126 patients were included in the experimental (n=62) and the control group (n=64), respectively. The time needed to have the first ambulation after surgery in the experimental group was significantly shorter than that in the control group (P<0.05). The incidence of patients' discharge within 48h and the occurrence of postoperative orthostatic intolerance were smaller in the experimental group when compared to the control group (P<0.05). Also, the incidence of moderate and severe urinary incontinence and the unplanned readmission rate within 30d of discharge were significantly lower in the experimental group (P<0.05). The difference between the two groups in terms of surgical complications was not statistical significant (P>0.05). Conclusion: The comprehensive nursing management program for day surgery in patients undergoing robotic-assisted radical prostatectomy can ensure the success of day surgery for robotic-assisted radical prostatectomy and promote patient recovery after surgery. Also, by applying the mode, the incidence of severe urinary incontinence in the first month after surgery is decreased and the patients' life quality is enhanced. The approach facilitates the high-quality management of day surgery for robotic-assisted radical prostatectomy.

Objective: To evaluate the effects of implementing bundled nursing under the concept of accelerated rehabilitation surgery on elderly patients after pancreaticoduodenal surgery, in order to promote early postoperative recovery. Methods: Elderly patients who underwent pancreaticoduodenectomy at a tertiary grade A hospital in Beijing from August 2019 to March 2023 were selected and divided into the control group and the observation group which received bundled care measures based on the concept of Enhanced Recovery After Surgery. The two groups of patients were matched using a 1:1 propensity score ratio according to their baseline data, and the postoperative recovery of the two groups of patients was compared. Results: Better postoperative recovery of gastrointestinal function and shorter hospital stay were observed in the observation group than those in the control group. The incidences of postoperative gastric paralysis and lower limb venous thrombosis were lower than those in the control group (P<0.05). Conclusion: Bundled care based on the concept of Enhanced Recovery After Surgery can promote the recovery of gastrointestinal function in elderly patients after pancreaticoduodenal surgery, reduce the incidence of postoperative gastric paralysis and lower limb venous thrombosis, and shorten their hospital stay after surgery.

Construction of eating behavior management program for patients with anorexia nervosa

Objective: To construct the eating behavior management program for patients with anorexia nervosa. Methods: Semi-structured interview, literature review and Delphi method were used to develop the eating behavior management program for patients with anorexia nervosa. Results: A total of 15 experts were interviewed by correspondence for two rounds. The effective recovery rate of the questionnaire was 100.0%. The authority coeffificients of the experts in the two rounds were 0.910 and 0.930, respectively. The coordination coeffificients of the experts in the two rounds were 0.295 and 0.332, respectively (P<0.001). The final version of eating behavior management program for patients with anorexia nervosa consisted of 6 first-level items, 16 second-level items and 32 third-level items. Conclusion: This eating behavior management program for patients with anorexia nervosa is scientific and reliable, and can provide a reference basis for clinical practice.

Decision conflict of delivery modes among pregnant women after cesarean section: a qualitative study

Objective: To analyze the decision conflict as to delivery modes of pregnant women after cesarean section, and provide basis and assistance for future decision-making. Methods: The descriptive qualitative research design was used to conduct in-depth semi-structured interviews with 13 pregnant women who had received cesarean section by purposive sampling. Content analysis was adopted to analyze the obtained data. Results: Conflicts in deciding delivery modes of pregnant women after cesarean section included limited information for decision-making as the antecedent of their behavioral conflict (complex process of decision-making, information restrictions), inadequate informed decision-making as the cause of behavioral process conflict (conflict of values, conflict among multiple decision-making subjects), and experience of decision-making conflict (decision-making behavior performance, emotional experience of decision-making). Conclusion: Pregnant women after cesarean section are affected by multiple factors when determining the modes of delivery. Nurses should take appropriate measures to assist patients to make proper decisions, so as to avoid conflict in decision-making, improve the quality of patients' decision, and reduce the rate of repeated cesarean section.

Mediating effect of family hardiness on psychological resilience and benefit finding among mothers of preschool children with autism spectrum disorder

Objective: To analyze the mediating effect of family hardiness on psychological resilience and benefit finding among mothers of preschool children with autism spectrum disorder, and to provide reference for further family interventions. Methods: A total of 205 mothers whose children with autism spectrum disorder received rehabilitation treatment in 4 institutions in Putian city from February 2022 to February 2023 were included by convenience sampling method. The Benefit Finding Scale, Connor-Davidson Resilience Scale and Family Hardiness Index were used to collect data. A structural equation model with AMOS 23.0 software was used to analyze the mediating effect. Results: The total scores of benefit finding, psychological resilience and family hardiness in the participants were 65.27±15.41, 54.89±17.04 and 34.98±6.89, respectively, and they were all positively correlated (P<0.001). Family hardiness played a mediating role between psychological resilience and benefit finding, and the mediating effect value was 0.230, accounting for 35.44% of the total effect. Conclusion: Family hardiness partially mediates the impact of psychological resilience on benefit finding in mothers of preschool children with autism spectrum disorder. Positive measures are expected to improve mother's psychological resilience and family hardiness, so as to improve their level of benefit finding.
