Construction and practice of a homogeneous management strategies for Continuous Renal Replacement Therapy

Abstract: Objective: To construct specialist nurse-led homogeneous management strategies for Continuous Renal Replacement Therapy (CRRT), evaluate the application effect, to improve patient treatment outcomes. Methods: A specialized nursing team was established and standardized training was implemented to enhance competencies. Standardized clinical procedures and quality control measures were applied, alongside structured nursing documentation and information-based support systems. In addition, a multidisciplinary bedside rounding mechanism involving physicians, nurses, and technicians was established. The strategy was implemented in a tertiary grade A hospital in Beijing, with patients treated from January to December 2023 were selected by convenience sampling as the control group, and those treated from January to December 2024 as the intervention group. Outcomes including delivered dose achievement rate, incidence of unplanned interruptions, and incidence of central venous catheter dysfunction were compared between the two groups. Results: A total of 788 CRRT sessions were performed, including 356 in the control group and 432 in the observation group. Compared with the control group, the observation group showed an improvement in delivered dose achievement rate from 89.33% to 95.14% (P=0.002), a reduction in the incidence of unplanned treatment interruptions from 16.57% to 9.49% (P=0.003), and a decrease in the incidence of central venous catheter dysfunction from 3.51% to 2.03% (P=0.001). Conclusion: The implementation of these strategies was associated with improved treatment outcomes and promoted the standardization and professionalization of nursing practice, providing a scalable practical pathway for homogeneous management in specialized nursing.
