Development and application of a comprehensive intervention program in Traditional Chinese Medicine nursing clinic for patients with mild to moderate Non-Alcoholic Fatty Liver Disease

Abstract: Objective: To develop a comprehensive intervention program in the TCM nursing clinic for patients with mild-to-moderate Non-Alcoholic Fatty Liver Disease (NAFLD) and to evaluate its clinical efficacy, with the aim of providing a standardized practice pathway for the TCM outpatient management of chronic diseases. Methods: A comprehensive intervention program in the TCM nursing clinic for patients with mild to moderate NAFLD was developed through evidence synthesis and Delphi expert consultation. Patients with mild to moderate NAFLD who visited the TCM nursing clinic in a tertiary grade A hospital in Shanghai between June 2025 and January 2026 were recruited as study participants by using convenient sampling method. The intervention period lasted 8 weeks. The control group (n=42) received routine nursing care, while the intervention group (n=48) received comprehensive TCM nursing program in addition to routine care. Liver fat accumulation, body composition, blood pressure, liver enzymes, blood lipids, nursing compliance, and satisfaction were compared between the two groups before and after the intervention. Results: The overall effective rate for improvement in liver fat in the intervention group was 47.92%, significantly higher than the 14.29% in the control group (P<0.01). The intervention group showed significant improvements in body mass index, waist-to-hip ratio, blood pressure, and liver enzyme levels (ALT and AST) compared to the control group (P<0.01). The overall nursing compliance rate and satisfaction rate in the intervention group were higher than those of the control group (P<0.05). Conclusion: The comprehensive TCM nursing intervention model is safe, feasible, and highly practical. It can effectively improve hepatic steatosis, body composition, and hepatocyte damage in patients with mild to moderate NAFLD, while enhancing nursing compliance and service satisfaction.
