Evidence summary for Transanal Irrigation management in patients with spinal cord injury and Neurogenic Bowel Dysfunction

摘要: 目的:检索、评价并总结脊髓损伤伴神经源性肠道功能障碍患者经肛门灌洗管理的证据,为临床实践提供循证依据。方法:基于PIPOST模式确定循证问题,按照“5S”模型对国内外相关学会、指南、数据库网站进行文献检索,检索时限为建库至2025年11月30日。研究者对文献进行质量评价和证据提取,并根据证据汇总原则和澳大利亚JBI证据分级系统总结相关证据。结果:共纳入17篇文献,其中临床决策2篇、指南7篇、证据总结1篇、专家共识2篇、系统评价2篇、随机对照试验1篇及类实验研究2篇,并从护理规范、团队培训、干预前评估、干预要点、并发症管理、干预后评估及随访、健康教育7个维度总结了32条证据。结论:该研究总结的证据具有一定的科学性和实用性,可为医护人员开展脊髓损伤伴神经源性肠功能障碍患者管理循证实践提供依据。

Abstract: Objective: To retrieve, evaluate, and summarize evidence regarding the management of Transanal Irrigation in patients with spinal cord injury and Neurogenic Bowel Dysfunction, providing evidence-based support for clinical practice. Methods: Evidence-based questions were identified using the PIPOST model. A literature search was conducted using the "5S" model, covering relevant domestic and international academic societies, guidelines, and databases from database inception to November 30, 2025. The researchers conducted quality assessment and evidence extraction of the literatures, and summarized the relevant evidence in accordance with the principles of evidence synthesis and the Australian JBI Evidence Hierarchy System. Results: A total of 17 articles were included, comprising 2 clinical decision-making articles, 7 guidelines, 1 evidence summary, 2 expert consensus articles, 2 systematic reviews, 1 randomized controlled trial, and 2 quasi-experimental studies. Thirty-two pieces of evidence were summarized across seven dimensions: nursing standards, team training, pre-intervention assessment, key points of the intervention, complication management, post-intervention assessment and follow-up, and health education. Conclusion: The evidence summarized in this study possesses scientific validity and practical value, providing an evidence-based basis for healthcare professionals to manage patients with spinal cord injury and Neurogenic Bowel Dysfunction.
