急诊抢救室护士呼吸机相关性肺炎预防认知与实践现状

Current status of knowledge and practice regarding Ventilator-Associated Pneumonia prevention among emergency nurses

摘要: 目的:调查全国范围内不同级别医院急诊抢救室护士呼吸机相关性肺炎(Ventilator-Associated Pneumonia,VAP)预防认知及实践现状,为制定急诊早期VAP防控的精准化护理干预策略提供参考。方法:采用方便抽样法,于2024年12月至2025年2月,使用自行设计的问卷对二、三级医院的急诊抢救室护士进行调查。比较不同级别医院护士在VAP核心防控知识与实践行为方面的差异。结果:共调查全国27个省、自治区、直辖市的83家医院,其中二级医院27家,三级医院56家,回收有效问卷980份。在认知层面,护士对预防VAP护理核心知识的总知晓率为56.46%;二、三级医院护士在体位管理、气囊监测、吸痰护理、镇静评估知识方面存在差异(均P<0.05)。在实践层面,二级与三级医院护士在声门下吸引(方式与频率)、气囊压力监测频率、气道内常规吸引方式、口腔护理溶液选择、是否行口腔冲洗、是否气管插管后1 h内胃肠减压及吸痰顺序方面,均存在差异(P<0.05)。结论:急诊抢救室护士对VAP预防认知与实践水平均有待提高,建议针对不同级别医院实施分级干预策略,以系统化提升急诊护士的VAP防控能力。

Abstract: Objective: To investigate the differences in knowledge and practice regarding Ventilator-Associated Pneumonia (VAP) prevention among emergency nurses at different hospital levels across the country, providing a reference for developing precise nursing intervention strategies for the early prevention and control of VAP in emergency settings. Methods: We employed a convenience sampling method to survey emergency nurses from secondary and tertiary hospitals from December 2024 to February 2025, using a self-designed questionnaire. Then statistical analysis of the data was conducted to compare the status of core VAP prevention knowledge and practice among nurses at different hospital levels. Results: A total of 980 valid questionnaires were collected from 83 hospitals across 27 provinces (autonomous regions, municipalities), including 27 secondary and 56 tertiary hospitals. The overall correct response rate for core VAP prevention knowledge was 56.46%. Significant differences were observed between nurses from secondary and tertiary hospitals in their knowledge of position management, cuff pressure monitoring, endotracheal suctioning care, and sedation assessment (all P<0.05). Regarding practice, statistically significant differences were identified between nurses in secondary and tertiary hospitals as to the method and frequency of subglottic secretion drainage, cuff pressure monitoring frequency, endotracheal suctioning method, selection of oral care solution, oral irrigation, gastric decompression within 1 hour after intubation, and suctioning sequence (all P<0.05). Conclusion: Both the knowledge and practice regarding VAP prevention among emergency nurses in China require improvement. Therefore, it is recommended that hospitals implement tiered intervention strategies tailored to themselves to systematically enhance the VAP prevention and control capabilities of emergency nurses.
