聊天机器人生育指导在育龄期乳腺癌化疗患者中的应用效果

Application effect of Chatbot fertility guidance in breast cancer patients of childbearing age undergoing chemotherapy

摘要: 目的:评价聊天机器人生育指导在育龄期乳腺癌化疗患者中的应用效果,旨在满足乳腺癌患者的生育信息需求。方法:依托信息技术,建立育龄期乳腺癌患者全诊疗周期的生育指导平台。便利选取2023年12月至2024年5月湖南省某三级甲等医院乳腺内科就诊的46例育龄期乳腺癌化疗患者为对照组,2024年6月至11月的46例患者为干预组。对照组接受常规健康教育与生育指导,干预组依据课题组前期构建的方案,在首次化疗前、出院前及居家期分阶段借助聊天机器人主动发起对话、推送生育相关知识、自动回复咨询问题等方式开展集中生育指导干预。评价两组患者的生育忧虑、生育意愿、生育信息支持水平和生活质量。结果:干预组患者在出院前的生育忧虑得分45.00(42.50,49.50)分低于对照组的62.50(54.50,67.00)分,居家期干预组得分35.00(34.00,38.00)分仍低于对照组的58.00(51.00,70.25)分,差异具有统计学意义(均P<0.05)。干预组在出院前和居家期的生育意愿、生育信息支持水平和生活质量高于对照组,差异具有统计学意义(均P<0.05)。结论:实施聊天机器人生育指导可以降低育龄期乳腺癌化疗患者的生育忧虑水平,提高其生育意愿及生育信息支持水平,对育龄期乳腺癌化疗患者的生活质量具有积极影响。

Abstract: Objective: To evaluate the effects of Chatbot fertility guidance in breast cancer patients of childbearing age undergoing chemotherapy, in order to meet the fertility information needs of breast cancer patients. Methods: A fertility guidance platform was developed using internet technology to support patients throughout their treatment. Breast cancer patients of childbearing age undergoing chemotherapy from a tertiary grade A hospital in Hunan province from December 2023 to May 2024 were selected by convenience sampling method assigned to the control group (n=46), while patients selected from June to November 2024 were assigned to the intervention group (n=46). The control group received conventional health education and fertility guidance. In the intervention group, according to the plan constructed by the research group in the early stage, before the first chemotherapy, before discharge and at home, the intervention group carried out centralized fertility guidance intervention by means of Chatbot, such as initiating dialogue, pushing fertility-related knowledge and answering consultation questions automatically. Patients' fertility concerns, fertility willingness, fertility information support level and quality of life were evaluated. Results: The score of fertility concerns in the intervention group 45.00(42.50, 49.50) before discharge was lower than that in the control group 62.50(54.50, 67.00), and the score in the intervention group 35.00(34.00, 38.00) at home was still lower than that in the control group 58.00(51.00, 70.25), and the differences were statistically significant (P<0.05). Fertility willingness, fertility information support level and quality of life in the intervention group before discharge and at home were higher than those in the control group, and the differences were statistically significant (P<0.05). Conclusion: Implementing Chatbot fertility guidance can reduce fertility concerns, enhance reproductive intentions and fertility information support, and positively impact the quality of life in breast cancer patients of childbearing age undergoing chemotherapy.
