摘要: 目的:开发辅助设备帮助低风险足月初产妇准确识别临产开始时间,以选择适宜入院待产时机,避免过早入院的不良影响。方法:以“双钻”设计模型为方法学指导,跨学科合作进行可穿戴智能宫缩监测系统的设计、开发,包括传感设备的选择和设计、运用人工智能算法构建宫缩模型,以及部署算法和显示入院建议的用户终端。采用量性和质性相结合的方法对初步开发的系统进行可用性检验。结果:可穿戴智能宫缩监测系统包括硬件设备部分及手机应用程序部分。系统提供的入院建议和医生基于金标准监测数据给出的入院建议的一致性检验显示Kappa系数为0.75,距离临床可用的精准度还有差距。对使用后的产妇进行的访谈结果显示出较好的感知有用性,但在感知易用性方面还有提升空间。结论:初步开发的可穿戴智能宫缩监测系统虽然受到目标客户的欢迎,具有一定准确性,但鉴于准确性尚未达到临床可接受水平,需要进一步收集数据精进算法模型,同时需要进行技术优化,改善产品的易用性。

Abstract: Objective: To develop an auxiliary device to help low-risk full-term primiparas accurately identify the onset of labor and determine the optimal time for hospital admission, in order to prevent the adverse effects of early hospital admission. Methods: Using the Double Diamond design process model, interdisciplinary cooperation was employed to design and develop a wearable intelligent uterine contraction monitoring system. This included the selection and design of sensor devices, the construction of a uterine contraction model using artificial intelligence algorithms, and the deployment of algorithms and user interfaces displaying recommendations. Quantitative and qualitative methods were employed to evaluate the usability of the preliminary system after delivery. Results: The wearable intelligent uterine contraction monitoring system included hardware and mobile App components. A consistency test between the system recommendations and the gold standard showed that a Kappa value was 0.75, indicating a gap between the current accuracy and the clinical acceptability. Interviews with postpartum women who used the system indicated high perceived usefulness but suggested room for improvement in perceived ease of use. Conclusion: The preliminary wearable intelligent uterine contraction monitoring system is well-received by the target users and demonstrates a certain level of accuracy, requires further data collection to refine the algorithm model, as its accuracy not yet reached clinically acceptable levels. Additionally, technical optimizations are needed to enhance the product's ease of use.
