临床护理操作姿势负荷评估与护士肌肉骨骼损伤风险分析

Clinical nursing operation posture load assessment and nurse musculoskeletal injury risk analysis

摘要: 目的:基于工效学视角,评估临床常见护理操作中的姿势负荷,分析护士肌肉骨骼损伤风险,为制定针对性的护士职业健康防护措施提供依据。方法:便利抽取2025年6月—7月长沙市某三级甲等医院53名临床护士,采用快速上肢评估(Rapid Upper Limb Assessment,RULA)方法,对其执行32项常见护理操作的过程进行视频采集与姿势分析。由3名经过培训的评估者依据RULA准则独立评分,计算其均值获得最终RULA分值,并划定损伤风险等级。结果:32项操作中,12项(37.50%)为极高风险,9项(28.13%)为高风险,11项(34.38%)为中风险;不同风险等级操作间的颈部、躯干和腿部RULA分值差异具有统计学意义(P<0.05)。结论:临床护理操作普遍存在中高等级的肌肉骨骼损伤风险,建议从优化操作环境、规范物品摆放、推广辅助工具、优化团队协作流程、加强系统化培训等方面进行综合干预,以降低护士肌肉骨骼损伤风险。

Abstract: Objective: Based on the perspective of ergonomics to evaluate the posture load in common clinical nursing operations, and to analyze nurses' musculoskeletal injury risk, so as to provide a basis for the development of targeted occupational health protection measures for nurses. Methods: Using the Rapid Upper Limb Assessment (RULA) method, video capture and posture analysis were performed on the process of 53 clinical nurses selected by convenience sampling method in a tertiary grade A hospital in Changsha performing 32 common nursing operations from June to July 2025. Three trained evaluators independently scored according to the RULA criteria, calculated the mean value to obtain the final RULA score, and delineated the risk level. Results: Among the 32 operations, 12 (37.50%) were extremely high risk, 9 (28.13%) were high risk, and 11 (34.38%) were medium risk. The difference of neck/trunk/leg RULA score between different risk levels was statistically significant (P<0.05). Conclusion: There is a common risk of middle and high level musculoskeletal injury in clinical nursing operations. It is suggested to carry out comprehensive intervention from the aspects of optimizing the operation environment, standardizing the placement of items, promoting the auxiliary tools, improving teamwork process, and strengthening systematic training, so as to reduce the risk of musculoskeletal injury of nurses.
