轻度认知障碍伴失眠老年人睡眠限制方案的构建及应用

摘要: 目的:构建针对轻度认知障碍伴失眠老年人的睡眠限制方案,并验证其效果,为改善该人群睡眠状况提供借鉴。方法:基于健康行动过程取向理论,通过文献回顾构建了睡眠限制方案初稿,结合德尔菲专家函询结果,确定轻度认知障碍伴失眠老年人睡眠限制方案。采用便利抽样法,于2024年5月—7月在福州市某社区卫生服务中心选取8名轻度认知障碍伴失眠老年人为研究对象,进行方案初步应用。结果:共纳入15名专家进行了2轮德尔菲专家函询,2轮问卷回收率分别为83.3%、100.0%,专家权威系数为0.890,肯德尔和谐系数分别为0.224和0.385(均P<0.05)。最终方案包括4个一级条目及16个二级条目。初步应用结果显示,干预后8名老年人失眠严重程度指数得分从(14.00±4.72)分下降至(10.75±3.41)分,差异具有统计学意义(P<0.05)。结论:本研究构建的方案具有科学性和实用性,在改善轻度认知障碍伴失眠老年人睡眠状况方面的效果得到了初步验证。

Abstract: Objective: To develop a sleep restriction program for older adults with Mild Cognitive Impairment (MCI) and insomnia, and validate its effectiveness, thereby providing implications for sleep improvement among this population. Methods: Based on Health Action Process Approach, a preliminary sleep restriction program was developed through literature review and refined via the Delphi expert consultation method. Using convenience sampling, eight eligible participants were recruited from a community health center in Fuzhou between May and July 2024 to preliminarily validate the program. Results: The Delphi consultation process involved 15 experts across two rounds, achieving response rates of 83.3% and 100.0%, respectively. The expert authority coefficient reached 0.890, with Kendall's W coefficients of 0.224 and 0.385 for successive rounds (all P<0.05). The finalized protocol comprised 4 first-level domains and 16 second-level components. Preliminary application results showed that the Insomnia Severity Index scores of 8 elderly individuals decreased from 14.00±4.72 to 10.75±3.41 after the intervention, with a statistically significant difference (P<0.05). Conclusion: The program shows scientific rigor and clinical applicability, with preliminary efficacy in improving sleep conditions.
