波特奇早期教育在孤独症高危儿家庭康复护理中的应用

Application of Portage early education method in family-based rehabilitation nursing for infants at high risk of Autism Spectrum Disorder

摘要: 目的:评价目标-活动-丰富运动(Goals-Activity-Motor Enrich-ment,GAME)模式下波特奇早期教育方法在孤独症高危儿家庭康复护理中的应用效果,为孤独症高危儿家庭干预提供依据。方法:于2024年3月—10月便利选取在安徽省某三级甲等医院就诊的孤独症高危儿90例为研究对象,采用随机数字表法分为两组,对照组(n=45)接受常规家庭康复护理,干预组(n=45)接受GAME模式的波特奇早期教育方法。比较两组患儿发育水平、父母育儿压力及育儿胜任感、孤独症量表筛查阳性率。结果:干预后3个月及6个月,干预组患儿Gesell发育量表得分、父母育儿胜任感得分高于对照组(P<0.05),干预组父母育儿压力得分低于对照组(P<0.05)。患儿18月龄时,干预组的改良婴幼儿孤独症量表筛查阳性率(22.2%)低于对照组(64.4%),差异有统计学意义(P<0.001)。结论:GAME模式下波特奇早期教育方法可提高孤独症高危儿生长发育水平,降低父母育儿压力,值得临床推广。

Abstract: Objective: To evaluate the effectiveness of the Goals-Activity-Motor Enrichment (GAME)-based Portage early education method in family-based rehabilitation care for high-risk infants with Autism Spectrum Disorder (ASD), and to provide evidence for family intervention in this population. Methods: Totally 90 high-risk infants with ASD who were treated in a tertiary grade A hospital in Anhui province were recruited from March to October 2024. They were randomly assigned to either the control group or the intervention group using a random number table. The control group (n=45) received conventional family care and rehabilitation, while the intervention group (n=45) received the GAME-based Portage early education method. Gesell Developmental Quotient scores, parenting stress, parenting competence, and the positive screening rate for ASD were compared between the two groups after the intervention. Results: At 3 and 6 months after intervention, the intervention group showed significantly higher Gesell scores and parenting competence scores than the control group (P<0.05), and significnatly lower parenting stress (P<0.05). At 18 months of age, 10 children (22.2%) in the intervention group and 29 children (64.4%) in the control group screened positive on the Modified Checklist for Autism in Toddlers (M-CHAT). A comparison between the two groups showed a statistically significant difference (P<0.001). Conclusion: The GAME-based Portage early education method can improve developmental outcomes in high-risk infants with ASD, and reduce parenting stress, showing promise for clinical application.
