医疗失效模式与效应分析在手术急消器械流程管理中的应用

Application of Healthcare Failure Mode and Effect Analysis in the process management of surgical emergency instruments

摘要: 目的:分析医疗失效模式与效应分析(HFMEA)在手术急消器械流程管理中的应用效果,以进一步提升手术室医疗器械管理质量。方法:通过绘制手术急消器械管理流程,找出失效模式,分析潜在原因,制定行动方案并组织实施,最后进行效果评价。结果:应用HFMEA进行手术急消器械管理流程改进后,手术急消器械通知准确率由91.44%提升至99.04%、面对面交接完成率由91.58%提升至99.18%、准时送达率由90.82%提升至99.49%,差异均具有统计学意义(P<0.001),连台手术翻台时间由(20.02±2.06)min缩短至(12.68±2.02)min,差异具有统计学意义(P<0.001)。结论:HFMEA应用于手术急消器械流程管理有利于规范手术室与消毒供应中心急消器械的交接与管理,可有效提高手术急消器械周转速度,加快手术翻台时间,提升手术室管理质量。

Abstract: Objective: To analyze the application of Healthcare Failure Mode and Effect Analysis (HFMEA) in the process management of surgical emergency instruments, in order to further improve the management quality of medical devices in the operating room. Methods: A workflow chart for surgical emergency instruments was created, potential failure modes in the management process were identified, root causes were analyzed, action plans were developed and implemented, and outcomes were evaluated. Results: After the implementation of HFMEA, the accuracy rate of notifications for surgical emergency instruments improved from 91.44% to 99.04%; the completion rate of face-to-face handovers increased from 91.58% to 99.18%; and the on-time delivery rate rose from 90.82% to 99.49%. All these differences were statistically significant (P<0.001). Additionally, the turnover time between consecutive surgeries was reduced from (20.02 ± 2.06) min to (12.68 ± 2.02) min, a change that was also statistically significant (P<0.001). Conclusion: The application of HFMEA in the process management of surgical emergency instruments helps to standardize the handover and management between the operating room and central sterile service department. It effectively improves the turnover efficiency of surgical emergency instruments, shortens surgical table turnover time, and enhances the overall quality of operating room management.
