血液透析患者饮食管理旅程地图的研究

Dietary management journey map of hemodialysis patients

摘要: 目的:绘制血液透析患者饮食管理旅程地图,动态识别其多维需求与体验,为患者全程饮食照护提供参考。方法:采用描述性质性研究,利用目的抽样法,于2024年3月—5月选取乐山市某三级甲等医院血液净化中心的14名血液透析患者进行半结构化访谈,并运用传统内容分析法整合资料,制作形成患者旅程地图。结果:按照透析准备期、过渡期、维持期3个阶段,血液透析患者饮食管理从任务、情绪、痛点和需求4个维度共析出症状管理、信息障碍、情绪交织、共同决策、社会适应、持续照护等28个主题。结论:血液透析患者饮食管理旅程复杂多变,建议以患者饮食偏好为基点,联合多方利益相关者,研发透析饮食信息化辅助工具,满足不同透析时段饮食管理需求变化,促进患者社会回归。

Abstract: Objective: To map the dietary management journey of hemodialysis patients, and to dynamically identify their multidimensional needs and experiences, in order to provide a reference for improvement of the whole process of dietary management. Methods: A descriptive study was used and 14 hemodialysis patients from the Blood Purification Center of a tertiary grade A hospital in Leshan were interviewed by the semi-structured questions from March to May 2024. The traditional content analysis method was used to integrate the data. A patient journey map was developed. Results: Across the three phases of hemodialysis- preparation, transition, and maintenance, the dietary management journey of patients was reflected through four dimensions (tasks, emotions, challenges, and needs) revealing 28 themes including symptom management, information barriers, emotional conflicts, shared decision-making, social adaptation, and continuous care, etc. Conclusion: The dietary management journey of hemodialysis patients is complex and dynamic, and it is recommended to develop dialysis dietary support tools based on patients' dietary preferences, cooperating with multi-stakeholders. It is expected to meet the changes in dietary management needs in different dialysis periods, and promote their social reintegration.
