中青年2型糖尿病患者改变生活方式动机与障碍的质性研究

Motivations and barriers to lifestyle changes in young and middle-aged patients with type 2 diabetes mellitus: a qualitative study

摘要: 目的:探究中青年2型糖尿病患者改变生活方式的动机和障碍,为开展干预性研究提供依据。方法:采用目的抽样法,于2024年9月—10月,对在天津市某三级甲等医院内分泌科住院的13例中青年2型糖尿病患者进行半结构式访谈,采用现象学研究方法和内容分析法对访谈资料进行分析、提炼主题。结果:中青年2型糖尿病患者改变生活方式的动机共归纳为3个主题:感知糖尿病危险性、感知并发症易感性、感知健康生活方式的益处或成效;改变生活方式的障碍共归纳为4个主题:饮食习惯难改变、疾病无感知症状、工作及就餐条件限制、社交聚会导致控制松懈。结论:中青年2型糖尿病患者在改变生活方式方面存在一定的阻碍和促进因素,临床医护人员需要重视患者改变生活方式的阻碍因素,进行针对性宣教,促进患者坚持健康的生活方式。

Abstract: Objective: To explore the motivations and barriers to lifestyle changes in young and middle-aged patients with Type 2 Diabetes Mellitus (T2DM) and inform intervention studies. Methods: In-depth semi-structured interview was conducted with 13 young and middle-aged T2DM inpatients in a tertiary grade A hospital in Tianjin from September to October 2024, purposive sampling method was used to select patients. The phenomenological method and content analysis were applied to analyze the interview data. Results: Three main themes emerged regarding motivations: perceived risk of diabetes, susceptibility to complications, and benefits of a healthy lifestyle. Barriers to lifestyle change were categorized into four themes: difficulties in changing dietary habits, lack of disease symptoms, work-related and dining constraints, and social gatherings leading to relaxed control. Conclusion: Young and middle-aged T2DM patients face specific barriers and facilitators to lifestyle changes. Clinical staff should address these barriers through targeted education to enhance healthy lifestyle adherence.
