Analysis on the status of nurses' basic psychological needs during career growth process in Sichuan province

Objective: To investigate the basic psychological needs among nurses during their career growth process and to further analyze its influencing factors. Methods: Multistage stratified cluster sampling was performed to recruit 858 participants from Sichuan province. The Basic Needs Satisfaction in General Scale, the Perceived Organizational Support Questionnaire, and a researcher-developed demographic questionnaire were used to collect data. Results: The mean total score on basic psychological needs satisfaction was 5.13±0.90, and the mean score on needs of autonomy, competence, and relatedness were 4.79±0.99, 5.08±1.03, and 5.47±0.92, respectively. The multiple linear regression analysis result showed that regions and nurses' perceived organizational support were influencing factors of their basic psychological needs satisfaction, which could explain 49.4% of the total variation. Conclusion: Nursing managers should pay more attention to nurses' basic psychological needs satisfaction, especially the needs of autonomy. It is also necessary for managers to provide a supportive work environment to stimulate nurses' work motivation and promote their career growth.

Based on the data of Chinese Licensure Examination for Nurse from 2011 to 2021, this paper analyzed the development and issues in the number of examinees, educational-level composition, quality and distribution of the initial nursing education in China. It is suggested that the number of nursing graduates could meet the demand for qualified nurses if simply the number was considered. The educational-level composition of initial nursing education needs further improvement by increasing the recruitment of graduates from general high schools into baccalaureate and advanced diploma nursing educational programs, while gradually decreasing the recruitment into secondary diploma programs and optimizing the educational models for students upgrading from secondary diploma to advanced diploma programs. There is still more room for the quality of nursing education to be better. An unbalanced distribution of the number and educational-level composition in nursing education among regions was observed, indicating that strategies should be strengthened to facilitate a balanced development of higher nursing education in all regions.

Objective: To explore the correlation between the Chinese Licensure Examination for Nurse scores and the professional ability of nurses in their first working year. Methods: A total of 267 newly recruited nurses from 2017 to 2019 were recruited by cluster sampling. The pre-job training and first-year rotation training for new nurses were carried out according to the training outline, and the professional ability were assessed after training. The correlation between the Chinese Licensure Examination for Nurse scores and the professional ability of nurses in the first year was analyzed. Results: The total score of the National Nursing Licensure Examination, the pre-job training examination and the normalization examination was (174.85±5.98) points, (173.60±11.94) points and (171.70±9.85) points, respectively. The total score of the Chinese Licensure Examination for Nurse was positively correlated with the total score of the normalization examination (r=0.415, P<0.01). The professional practice score of the Chinese Licensure Examination for Nurse was positively correlated with the normalization theory examination (r=0.049, P<0.01). There is a positive correlation between the practice ability score of the Chinese Licensure Examination for Nurse and the pre-job training operation test score (r=0.121, P<0.05), as well as the operation test score in normalization examination (r=0.140, P<0.01). Conclusion: A positive correlation is detected between the scores of the National Nursing Licensure Examination and nurses' first-year professional ability. It is thus recommended that managers pay more attention to the training of nurses' self-directed learning ability and motor transfer ability, and promote the rapid improvement of practice ability of newly recruited nurses with different backgrounds.

Application of integrated fall prevention strategy in discharged elderly

Objective: To construct the integrated STEADI fall prevention strategies based on the Transtheoretical Model and STEADI toolkit and validate its effects on the elderly. Methods: The integrated STEADI fall prevention strategies were constructed according to literature review results and expert discussion. From October to November 2019, the elderly admitted to the Gerontology Department in a tertiary hospital with fall risk screened by the Morse Fall Scale (Chinese version) were divided into two groups using the random sampling method. The experimental group (n=93) were given the integrated STEADI fall prevention strategies, and the control group (n=95) conventional fall prevention interventions. Then the implementation rate of fall prevention, the incidence of falls and fall injuries between the two groups within 24 weeks after their discharge were compared. Results: The implementation rate of fall prevention measures for the elderly in the experimental group was higher than that of the control group 4 weeks (except for vision and foot management), 12 weeks (except for foot management) and 24 weeks (except for foot management) after discharge (P<0.05). Both the incidence of falls (9.7%) and fall injuries (22.2%) in the experimental group were lower than those of the control group (20.0%, 68.4%) (P<0.05). Conclusion: The integrated STEADI fall prevention strategies are conducive to improve the execution rate of fall prevention and reduce the incidence of falls and fall injury in the elderly after their discharge.

Qualitative study on role expectation of visiting nurses in Critical Care Outreach Services from multiple perspectives

Objective: To explore the role expectation of visiting nurses in Critical Care Outreach Services from multiple perspectives. Methods: The phenomenological research method in qualitative research was adopted. The purpose sampling and snowball sampling were combined to conduct in-depth interviews with all stakeholders involved in Critical Care Outreach Services in a tertiary hospital in Qingdao, including visiting nurses, ward nurses, ICU doctors, ward doctors, managers, patients and caregivers. The interview data were analyzed by Colaizzi phenomenological data analysis method and Nvivo 12.0 coding software. Results: The role expectation of the nurses in Critical Care Outreach Services mainly focused on six roles: nursing practitioners, team liaison, case managers, training providers, health educators and service providers. Conclusion: The role of Critical Care Outreach Services visiting nurses is centered on the role of nursing practitioners, and other roles are focused and adjusted according to the actual service content of patients. Visiting nurses should be proactive in enhancing their professionalism and improving their professional competence to promote continuous improvement in the quality of nursing services for patients transferred out of the ICU.

Evaluation of comprehensive pain management in children aged 3-7 with initial PICC catheterization

Objective: To evaluate the effects of comprehensive pain management in children with hematological malignancies aged 3-7 who received PICC catheters for the first time. Methods: Children with hematology oncology aged 3-7 who were hospitalized in the Department of Hematology and Oncology of our hospital from June 2019 to January 2021 and received PICC catheterization for the first time were divided into control group (n=52) and intervention group (n=52) by using convenient sampling method. The children in the control group received routine intubation care, and the intervention group received comprehensive pain management scheme. The pain score and pain degree assessed by FLACC scale were compared between the two groups. Results: The pain score and pain degree in the intervention group were significantly lower than those in the control group during first-time PICC catheterization (P<0.01). Conclusion: Implementation of comprehensive pain management scheme in children with hematological tumors aged 3-7 who received PICC catheterization for the first time can effectively reduce the pain.

Objective: To investigate the associated factors of Kinetophobia in patients with Angina Pectoris and Coronary Heart Disease, therefore to provide references for developing tailored intervention strategies for targeted patients. Methods: Following convenient sampling, between September 2020 and April 2021, eligible patients with Coronary Heart Disease and Angina Pectoris from the North China University of science and technology affiliated hospital were recruited for the study. The Tampa Scale for Kinesiophobia Heart was used to screen patients with Kinetophobia. A total of 299 patients were enrolled in the study, among whom 141 patients were identified with Kinesiophobia, and 158 patients were with simple Coronary Heart Disease angina pectoris without Kinetophobia. We used chi-square and binary Logist regression analysis to identify the associated factors for Kinetophobia in this sample. Results: Logistic regression analysis showed that fatigue, self-perceived burden, female gender, and frequency of angina pectoris were risk factors of Kinetophobia, while the recognition of exercise benefits was demonstrated as a protective factor for Kinetophobia (P<0.05). Conclusion: Patients with Coronary Heart Disease and Angina Pectoris have a high incidence of Kinetophobia, healthcare providers should pay more attention to this group and take actions to intervene and promote effective rehabilitation.

Effects of the Shared Decision Making intervention on outcomes of patients with diabetes: a Meta-analysis

Objective: To evaluate the effects of the Shared Decision Making intervention on outcomes of patients with diabetes. Methods: Databases including PubMed, Embase, the Cochrane Library, Web of Science, CNKI, WanFang Data, VIP Data and CBM were comprehensively searched for randomized controlled trials on the effects of the Shared Decision Making intervention on outcomes of patients with diabetes from July 2, 2011 to July 2, 2021. There were 2 researchers independently screened the articles, evaluated the quality and extracted data of the included studies. The Meta-analysis was performed with RevMan 5. 3 software. Results: A total of 14 articles and 2325 diabetes patients were included. The Shared Decision Making intervention could alleviate the decisional conflict of patients with diabetes (P=0.0007), improve their decision self-efficacy (P=0.0002), improve their medication compliance (P=0.03) and reduce their BMI (P=0.01). The intervention could reduce HbA1c level in 3 months (P=0.001), however, it did not change the HbA1c level of patients in 6 months (P=0.23) or 24 months (P=0.27). Conclusion: The Shared Decision Making intervention can improve decision-making quality in patients with diabetes, improve their medication compliance, and reduce their HbA1c values in 3 months. It is suggested that more studies can be conducted to further investigate the effects of the Shared Decision Making intervention on outcomes of patients with diabetes in the future.

Objective: To develop the medication safety quality control indicators for operating room, to provide standards and evaluation of medication safety quality. Methods: Based on the evidence of medication safety of the operating room, the Donabedian structural-process-results model was adopted as the theoretical framework to preliminarily formulate the quality control indicators of medication safety in operating room. The Delphi method was used to conduct two rounds of consultation among 24 experts to screen, verify and correct the indicators. Results: The response rates of the two-round consultation were 100.0%. The expert authority coefficients were 0.890 and 0.906. The Kendal coefficients were 0.246-0.329. The final indicators of medication safety quality control for operating room included 3 first-level indicators, 9 second-level indicators and 19 third-level indicators. Conclusion: The development of medication safety quality control indicators for operating room can supervise throughout the medication, which is feasible, reliable, and practical, and can provide reference for medication safety and quality evaluation and monitoring in operating room.

Objective: To construct an intervention program based on the concept of Child Life for the symptom group of children with Acute Leukemia. Methods: Under the guidance of the concept of Child Life, through literature review, a preliminary intervention plan for the symptom group of children with Acute Leukemia was formulated, the specific content, mode and time of the intervention were determined through Delphi expert consultation. Results: The effective recovery rates of the two rounds of expert consultation questionnaires were 79.0% and 100.0% respectively, the expert authority coefficient was 0.823, and the expert coordination coefficient of the two rounds was 0.413 and 0.135, respectively. Finally, a symptom group intervention program with an intervention period of 2 months was formed, including 3 first-level items, 8 second-level items and 18 third-level items in the developmental game stage, medical simulation game stage and supportive game stage. Conclusion: The intervention program based on the concept of Child Life for the symptom group of children with Acute Leukemia has reliable results and has certain feasibility.

Objective: To analyze the application of Healthcare Failure Mode and Effect Analysis (HFMEA) in the risk management of Intraoperation-Acquired Pressure Injury (IAPI). Methods: Established a HFMEA work team of IAPI project, made work flow chart, found out the failure mode in the risk management of IAPI, analyzed the potential root reasons, developed action plan and implementation, and evaluated the effect. Results: After applying HFMEA, the accuracy of IAPI risk assessment, the standard rate of using pressure-lowering pads and the standard rate of body position placement were significantly improved (P<0.05), and the incidence of IAPI was significantly decreased (P<0.05). Conclusion: The application of HFMEA in the risk management of IAPI can effectively help standardize the management process of IAPI prevention and decrease the incidence of IAPI, ensure patient safety.

Current situation and influencing factors of health beliefs of young nurses

Objective: To investigate the current situation of young nurses' health beliefs and analyze the influencing factors to provide basis for intervention. Methods: Using the convenient sampling method, 557 nurses from a tertiary grade A hospital in Beijing were selected from October to November 2020. The general information questionnaire, Multi-dimensional Health Belief Scale, Wallston Health Value Scale and Perceived Organizational Support Scale were used for cross-sectional survey, and the factors affecting the health belief of young nurses were determined by multiple linear regression. Results: The young nurses' health belief score was 117.87±17.02. Willing to be a health model, organization support and title were important factors affecting the health belief of young nurses, accounting for 26.7% of the total variation of health belief. Conclusion: The health belief level of young nurses is above the middle level. Nursing managers should actively mobilize the internal driving force of nurses, enhance their willingness to become health models, and give sufficient organizational support to improve their health belief level, so as to give better play to the role of health promotion.

Objective: To understand the current status of the perceived professional value among medical staff who worked at the Disinfection Supply Centers, therefore provide references for administrative management. Methods: Following snowball sampling, a total of 651 medical staff at the disinfection supply centers from 20 hospitals in 14 cities across the province was included in the current study. The Nurse Professional Value Scale was used to collect information related to participants perceived professional value. Results: The mean overall score of the perceived professional value among participants was 117.10 (SD=27.23). Following analysis, age, hospital classification, professional title, type of employment and job category were the main relevant factors of professional values among participants in this study (P<0.05). Conclusions: The professional value among medical staff from Gansu Disinfection Supply Center is at a medium level, the administrative personnel from the center should take actions and further enhance their professional values by strengthening the basic construction of the setting, improving benefits and coverage, paying attention to key groups, and implementing humanized management.

Objective: To systematically review the needs assessment tools for family caregivers of patients with cancer. Methods: We searched 9 databases. Original studies of the needs assessment tools for family caregivers of patients with cancer were screened. Included tools were appraised using the quality criteria for judging the psychometric properties. And needs themes were extracted based on the original tools' dimension contents. All of the needs themes were clustered based on Maslow's hierarchy of needs theory. Results: We collected 14 needs assessment tools, extracted 27 needs themes and clustered in 6 needs levels. Among them, the most widely involved needs were self-actualization needs, love and belonging needs and cognitive needs. Esteem needs and safty needs were less tested, while aesthetic needs were omitted. The content validity, internal consistency and construct validity of the assessment tools were appraised as "good" accounted for 43%, 21% and 14% respectively. Conclusion: The content of the needs assessment tool is relatively rich, but it should be further expanded, and the overall level of its psychometric properties needs to be improved further.

Effects of Virtual Reality on maternal delivery: a Meta-analysis

Objective: To evaluate the effects of Virtual Reality (VR) on puerpera. Methods: A systematic search of Sinomed, VIP, CNKI, WanFang Data, EMBASE, PubMed, Web of Science, The Cochrane Library database, China Clinical Trials Registry and ClinicalTrials.gov was conducted to collect randomized controlled trials of VR applied to puerpera. The retrieved literatures were then screened and assessed by two researchers to extract relevant data for further Meta-analysis through software RevMan 5.3. Results: The study eventually enrolled 9 studies with a total of 853 subjects. Meta-analysis results showed that compared with traditional care interventions for childbirth, the rate of maternal cesarean section, anxiety score, the pain of cervix 9-10 cm dilated, and the pain during childbirth were significantly lower than those in the control group, but there was no statistically significant difference in the pain of cervix 4-6 cm dilated and the time spent in the first, second and third stage of labour. Conclusion: VR technology plays a positive role in parturient women during delivery. The promotion and popularization of VR technology can benefit patients.

Objective: To describe the current status, research methods and contents on the establishment of a prediction model by Chinese nursing scholars. Methods: Chinese databases including SinoMed, CNKI, Wanfang Data, VIP were searched. English databases including Embase, PubMed, CINAHL, and Web of Science were searched. The JBI scoping review methods were used to analyze the included articles. Results: A total of 141 articles (29 theses and 112 journal articles) were included and mainly published in the core journals after 2019. Patients were selected as research subjects and whose disease complications and health outcomes were the main focus. The research settings were in single centers (82.3%), and prospective studies accounted for 19.9%. All studies constructed prediction models, but the completion of model evaluation and verification is poor. Conclusion: The studies on the establishment of prediction models by Chinese nursing scholars are in the initial stage. The establishment of prediction model has called high attention from researchers. The number of prediction model has increased dramatically. However, the research methods and model quality are urgently needed to be reinforced. A more scientific, accurate and practical predictive model should be established to help improve quality of clinical nursing.

Objective: To search, evaluate and summarize the best evidence for monitoring Intra-Abdominal Pressure in critically ill patients. Methods: The relevant evidence of Intra-Abdominal Pressure monitoring in critically ill patients was searched. The quality of the included articles was evaluated and those that met the quality standards was included and summarized. Results: A total of 10 articles were selected, including 3 guidelines, 1 expert consensus, 5 cohort studies, and 1 recommended practice. Totally 30 pieces of evidence on 6 aspects including policy management, method selection, body position and reference zero point, volume and temperature of perfusion fluid, eliminating interference factors, and education and training were finally summarized. Conclusion: The summarized evidence for can be used in clinical practice to promote standardized management of Intra-Abdominal Pressure monitoring in critically ill patients.

Effects of "Internet+" three-level linkage on self-management ability of diabetic patients and family members

Objective: To explore the intervention effect of "Internet+" three-level linkage management mode on self-management ability and blood glucose index of diabetic patients and family members. Methods: From December 2017 to December 2018, 43 patients and 43 first-degree relatives were conveniently selected from a diabetes diagnosis and treatment center of a tertiary grade A hospital in Gansu province. The intervention time was 18 months with "Internet+" three-level linkage management mode. Before inter vention and at 0, 3, 6, 9, 12 and 18 months after the intervention, the self-management ability scores, blood glucose-related indicators and family members' self-efficacy scores were measured. Results: There were statistically significant differences in the 5 dimensions and total scores of blood glucose monitoring management, diet management, exercise management, handling high/low blood sugar, and foot care in diabetic patients at 6 time points (P<0.05). There were statistically significant differences in the self-efficacy scores of diabetic family members measured at 6 time points (P<0.05). Conclusion: The "Internet +" three-level linkage management mode can improve the self-management ability of diabetic patients and family members, and also provides a certain reference value for the internet management of other chronic diseases.

Objective: To design and develop the chronic wound grade management information platform based on the "Internet+" concept and explore its application effects. Methods: The information platform was established. Then chose one Grade A hospital and five hospitals within its medical alliance network for trial operation which was led by wound specialist nurses implementing. The number of referral and remote consultation of chronic wound patients as well as the wound cure rate and patient satisfaction were recorded and analyzed through the platform. Results: A total of 216 long-distance consultations were carried out by specialist nurses with the cure rate of chronic wound reaching 90.90% (60/66) and patients' satisfaction of the platform 96.96% (64/66). Conclusion: Relying on the resources of grade A hospitals, the chronic wound classification management information platform was constructed, which not only improved the cure rate of chronic wounds but also patient satisfaction to procure better chronic wound grade management.

Objective: To analyze the clinical application effect of HIS intelligent information management of narcotic drugs and psychotropic drugs in the department of anesthesiology. Methods: Since November 2019, HIS intelligent information management of narcotic drugs and psychotropic drugs has been implemented in the department of anesthesiology, comprehensive rectification on the access and prescription management of narcotic drugs and psychotropic drugs has been carried out. The time required for taking and returning drugs, the frequency of taking drugs, prescriptions error rate, drugs abandonment rate, the incidence of adverse events related to narcotic drugs and psychotropic drugs and the satisfaction of medical staff with workload of narcotic drugs and psychotropic drugs were counted 12 months before and after using HIS intelligent information management. Results: Compared with before, the time required for taking and returning drugs, the frequency of taking drugs, prescriptions error rate, drugs abandonment rate, the incidence of adverse events related to narcotic drugs and psychotropic drugs decreased significantly after the application of HIS intelligent information management for narcotic drugs and psychotropic drugs (P<0.05); the satisfaction of medical staff with workload of narcotic drugs and psychotropic drugs was significantly improved (P<0.05). Conclusion: The intelligent information management of narcotic drugs and psychotropic drugs can significantly improve the management quality, reduce the incidence of adverse events related to narcotic drugs and psychotropic drugs, promote the use of narcotic drugs and psychotropic drugs more safe, efficient and standardized.

Objective: To construct an integrated respiratory therapy and rehabilitation program, and evaluate its curative effect in ICU patients with mechanical ventilation. Methods: Patients who were admitted to a tertiary grade A hospital in Anhui province for mechanical ventilation from January 2019 to December 2020 selected as research objects. The patients who implemented the individualized respiratory treatment and rehabilitation integrated program formulated by the team composed of doctors, respiratory therapists, rehabilitation therapists and nurses in 2020 were taken as the observation group, and the patients who received routine care of mechanical ventilation in 2019 were taken as the control group. Mechanical ventilation time, tracheal intubation time, 48h reintubation rate, Ventilator-Associated Pneumonia (VAP) incidence, delirium incidence, ICU acquired weakness incidence, early functional exercise rate, muscle strength after intervention and out of bed rate for the two groups were compared. Results: Totally 3124 mechanically ventilated patients were included, 1673 cases in the observation group and 1451 cases in the control group. Mechanical ventilation time, tracheal intubation time, 48h reintubation rate, VAP incidence, delirium incidence, and ICU acquired weakness incidence of the observation group were significantly lower than those of the control group, the differences were statistically significant (P<0.05). In the early functional exercise rate, muscle strength after intervention and the out of bed rate, the observation group was significantly higher than the control group, and the differences were statistically significant (P<0.05). Conclusion: This study shows that the integrated program of respiratory therapy and rehabilitation is scientific and feasible, the application of an integrated respiratory therapy rehabilitation program can shorten mechanical ventilation time and tracheal intubation time for mechanically ventilated ICU patients, reduce the 48h reintubation rate, the incidence of VAP and delirium.

Objective: To explore the pregnancy experience of women who undergoing fetoscopic laser photocoagulation due to the Twin-to-Twin Transfusion Syndrome (TTTS), and to provide references for developing a psychological support system of women with complicated pregnancy. Methods: The semi-structured interview was conducted with 15 women who underwent fetoscopic laser photocoagulation due to TTTS, and the data was analyzed by using the Colaizzi method. Results: Three themes of pregnancy experience were extracted: emotional reaction during pregnancy, influencing factors of emotional reaction and adjustment methods of negative emotions. Conclusion: TTTS could cause women significant mood swings during pregnancy, and could cause long-term adverse effects on women's mental health. Medical staff should pay more attention to their mental health and establish psychological support and information support systems based on their emotional response characteristics to reduce their adverse emotion during pregnancy and improve their pregnancy experience.
