The reform and development of nursing education under the background of nursing education coordination

Under the background of healthy China strategy, breakthrough progress has been made in the construction of nursing talent team in China, with the continuous improvement of quantity and education level, and the support of three-level nursing education system and education policy and conditions becoming more and more perfect, which provides a good opportunity for the progress of disciplines and the coordinated development of nursing education, so as to meet the higher requirements of the public and the industry for the overall quality and education level of nursing team. However, there are still some problems in the development of education, such as the deviation of talent supply structure and health demand, the lack of professional operability standard, the lack of cooperative education mechanism of nursing and education, and the weak level of discipline development. From the aspects of basic strategy, overall goal, basic task and development orientation, based on policies, industries and colleges and universities, this paper analyzes the causes of a series of problems and the advantages of developing nursing education coordination in improving supply structure, education quality and discipline development, and then proposes to build a four-level nursing education coordination management system and post training in nursing industry in an all-round way training standards to improve the education management system, reform the education teaching and support system, promote the reform of talent education mode, and ultimately cultivate outstanding nursing talents.

Ideological and political education in nursing courses refers to the integration of ideological and political education into all aspects of professional nursing courses teaching, and the cultivation of nursing professionals with noble medical ethics and to serve the people both morally and ingeniously. Advanced nursing practice is the core course for Master of Nursing Specialist, which is both humanistic and professional. This course can lay a solid foundation for students to adapt to and be competent for complex clinical, teaching and research work in the future. In order to establish a more unobstructed access to ideological and political education, internalize the ideological and political level of students, based on the characteristics of the course and students, this paper analyzes the necessity and obstacles of ideological and political education, and expounds the practical path of the ideological and political education in advanced nursing practice course.

In response to the Party Central Committee's decisions and arrangements to win the battle of COVID-19 epidemic prevention and control firmly, the Ministry of Education put forward the requirement of "stop class, but no stop teaching nor stop learning", and universities have opened online teaching. School of Nursing in Nanjing University of Chinese Medicine gave full play to its professional characteristics, and excavated the excellent nursing deeds of frontline in fighting against the epidemic. The college carried out the online ideological and political theory education reform based on online platforms, explored the realization path and guarantee mechanism of online teaching of "San Quan Education System" from three aspects of all staff, whole course and all-round education. Besides, we also formed a curriculum ideological and political team, innovated teaching strategies, told the story of fighting the epidemic, and cultivated "mental vaccine". The college ran the course for ideological and political education through the whole process of online teaching to cultivate professional, caring and responsible outstanding nursing talents. Finally, the teachers and students had high satisfaction and good teaching effects. The results indicate that the online teaching reform of "San Quan Education System" has a significant effect. The innovation of ideological and political education of the whole course combined with online and offline teaching mode is an important research direction in the future.

Objective: To evaluate the application effects of the integration platform construction based on the simulation medical theory and practice system in the integrated healthcare training mode. Methods: Based on the basic principles of system integration of Healthcare Simulation, the integrated healthcare service of cerebrovascular disease was selected as the target scene. Healthcare Simulation integration platform covering the whole process training and operation evaluation was constructed. Guided by the Salas principle and the core competence of TeamSTEPPS, the content and evaluation were designed and constructed scientifically. Mini-CEX, DOPS,SP and participants' 360° evaluation tool were used to evaluate the training effect. Miller validity framework was used to explore the operation effect of the platform. Results: The Healthcare Simulation integration platform had built a training, evaluation and actual operation effect evaluation system which fit in with practical concerns that was showing good consistency with the actual clinical practice. After the training, the participants' abilities in process management, risk early warning, professional skills, communication, team cooperation were improved (P<0.05). Conclusion: The construction of the integration platform based on the theory and practice system of Healthcare Simulation provides good training programs, research tools and evaluation systems, and it is of great value and significance to promote the management mode in the overall patient-centered diagnosis and treatment.

Objective: To introduce the Medication Adherence Self-efficacy Scale-Revision (MASES-R) to China, and to test its reliability and validity. Methods: Authorization was obtained from the original authors of MASES-R. Cross-cultural revision of the MASES-R was conducted according to the guidelines recommended by the American Academy of Orthopaedic Surgeons Evidence-based Medicine Committee. January to March 2019, Totally 445 patients with hypertension were recruited in Changsha through convenience sampling from a tertiary hospital and investigated by using the Chinese version of MASES-R(MASES-R-C).Reliability and validity of the scale were assessed. Results: In MASES-R-C, the correlation coefficient between each item and the total score ranged from 0.660 to 0.919, and the correlation coefficient between each item ranged from 0.514 to 0.872. The I-CVI of each item ranged from 0.83 to 1.00, and the S-CVI was 0.961. KMO value of exploratory factor analysis was 0.920, Bartlett's spherical test chi-square value was 6405.74 (P<0.001), values of factor loading ranged from 0.640 to 0.916, and the total variance interpretation rate was 68.72%. In confirmatory factor analysis, average variance extracted (AVE) value was 0.647, composite reliability (CR) value was 0.947, and the standardized loading coefficient for each item ranged from 0.597 to 0.932. The single-dimensional model fit well. The Cronbach's α coefficient of the scale was 0.960, Spearman-Brown split-half reliability was 0.927, and the test-retest reliability was 0.868. Conclusion: The psychometric properties of Chinese version of MASES-R proved to be good. MASES-R-C can be used as a tool to evaluate the medication adherence self-efficacy of patients with hypertension in China.

The readiness for Advance Care Planning and its influencing factors among patients with Chronic Heart Failure

Objective: To investigate the level of readiness and its influencing factors for Advance Care Planning (ACP) in patients with Chronic Heart Failure. Methods: A convenient sample of 190 patients with Chronic Heart Failure were enrolled from two tertiary hospitals in Ji'nan, between June and October in 2019. Patients completed the demographic and clinical characteristics questionnaire, the Advance Care Planning Readiness Scale and the Death Attitude Profile-Revised. Results: The mean score of ACP readiness among patients with Chronic Heart Failure was 76.10±8.86, which was above the medium level. Multiple regression analysis found that education level, religion, perceived quality of life, and attitudes toward death were the main factors influencing the level of ACP readiness among patients (P<0.05). Conclusion: The level of ACP readiness in patients with Chronic Heart Failure is relatively high, with a considerably negative attitude towards death. Health care providers should strengthen educational components toward death and help patients establish a correct perception for death, therefore to further promote the acceptance and development of Advance Care Planning.

Objective: To investigate the status quo and influencing factors of knowledge in family caregivers of pediatric patients on long-term maintained peritoneal dialysis. Methods: A convenience sample of 121 family caregivers from four tertiary pediatric hospitals in eastern, middle, and southwest of China was selected in the study. The demographic information of caregivers and children, as well as the knowledge about management of peritoneal dialysis was collected using a self-developed questionnaire. Results: The total score for the knowledge on management of peritoneal dialysis was 59.26±5.24. Caregivers' gender, educational background, types of training projects and whether receiving retraining had impacts on knowledge about management of peritoneal dialysis. Conclusion: Family caregivers have basic understanding of peritoneal dialysis. However, understanding of complications and nutrition management in peritoneal dialysis is still inadequate. It is suggested that healthcare providers design appropriate training and retraining programs using various health education strategies based on gender and educational background of caregivers to improve their knowledge on peritoneal dialysis.

Development and psychometric testing of the Self-Rated Scale for Knee Joint Unpleasant Symptoms in middle-aged and elderly people

Objective: To develop a self-rated scale for knee joint unpleasant symptoms and test its reliability and validity among middle-aged and elderly people. Methods: According to the result of literature review, semi-structured interviews, experts consultations and group meetings, a item pool was established, and the first draft of the scale was developed after two rounds of experts consultations. The scale was administered among 528 middle-aged and elderly people aged ≥45 in Changsha. The reliability and validity of the scale was analyzed by using SPSS 20.0 . Results: The self-ratedscale of knee unpleasant in middle-aged and elderly people included 21 items. The results of theexploratory factor analysis indicated two factors, named self-conscious symptoms and self-examination performance, explaining 64.983% of the cumulative variance. The result of the confirmatory factor analysis showed that the fit index χ2, df, χ2/df, RMSEA, CFI, GFI, AGFI, NFI, IFI, TLI were 729.238, 188, 3.879, 0.098, 0.888, 0.781, 0.730, 0.855, 0.888,0.875, respectively. The I-CVI of the scale was 0.777-1.000, S-CVI/Ave was 0.891.the correlation coefficient between the score of each dimension score and the total scale was 0.707-0.980. The Cronbach's α coefficient of the total scale was 0.949, split-half reliability was 0.888 and the test-retest reliability was 0.914. Conclusion: The self-rated Scale for Knee Joint Unpleasant Symptoms has good reliability and validity in middle-aged and elderly people, which can be applied to investigate the unpleasant symptoms of knee joint in middle-aged and elderly people in China.

Cognitive frailty in elderly patients with hemodialysis and its relationship with fear of falling

Objective: To investigate the status of cognitive frailty and its relationship with fear of falling in elderly hemodialysis patients. Methods: A total of 255 elderly patients with hemodialysis in two hospitals in Zhengzhou were investigated with general data questionnaire, Tilburg Frailty Indicator, Modified Fall Efficacy Scale, Montreal Cognitive Assessment, and Clinical Dementia Rating. Results: The incidence of cognitive frailty in elderly hemodialysis patients was 25.9%, and the total score of falling fear was 66.09±19.15. Age, gender, history of falls, whether exercise, serum albumin and hemoglobin were the influencing factors of cognitive frailty (P<0.05). There were significant differences between the groups with and without cognitive frailty in fear of falling (P<0.05). Conclusion: The incidence of cognitive frailty and the level of fear of falling in elderly hemodialysis patients are high, and cognitive frailty may affect the level of fear of falling, suggesting that medical staff should strengthen their attention and conduct early identification and intervention.

The effect of short-term training on COVID-19 related knowledge

Objective: To evaluate the effect of short-term training on the knowledge of COVID-19. Methods: Based on real-time updated knowledge of COVID-19, a six-stage roller-type training was conducted for 2099 nurses in a tertiary grade A general hospital in Beijing. After the training, the online-based coverage assessment and offline-assisted interpenetrating training quality evaluation was implemented, and a self-designed questionnaire was used to complete the evaluation of the training methods by all nurses. Results: There were no significant differences in the assessment results of the stages (P=0.482). In the training quality evaluation, the score of offline examination was 90.35±12.01, and the score of quality inspection was 90.47±12.84. In the evaluation of nurses training system, 98.9% of nurses liked this training method, and 99.3% of nurses thought the training content was updated in a timely manner and set reasonably, which was conducive to mastering the training content (99.4%), improving self-protection during the epidemic (99.5%), improving self-learning ability (99.4%) and enhancing the importance of COVID-19 (99.5%). Conclusion: During the outbreak of COVID-19, short-term training was a new method for online staff training for the nature of infectious diseases, and has achieved good results in practice. It can be non-aggregated and updated quickly during the outbreak, and effectively improve nurses' knowledge of COVID-19.

Effect analysis of online wound clinic during the prevention and control of the COVID-19

Objective: To analyze the feasibility and effect of online wound clinics during the COVID-19 pandemic, and provide a basis for future Internet + wound care services. Methods: In response to the requirements of epidemic prevention and control, an emergency team was set up to conduct online wound care clinics by WeChat, Good doctor website, and wound course WeChat public platform. A three-level quality management mode (charge nurse, mediate and senior wound care specialty nurse) was adopted to ensure the quality of professional nursing. Special personnel collected data and answered patients' questions in a timely manner. After 4 weeks, a random anonymous online assessment of satisfaction was conducted according to 10% of the patients. Results: A total of 200 people and 560 person-times were served online for 29 days. The effective rate of wound healing was 79.50%, chronic wound maintenance rate was 10.00%, and shedding rate was 10.50%. Compared with the offline clinic in February 2019, there was no significant difference (P>0.05). The sources of patients in online clinic were wider (P<0.001). The satisfaction of online clinics was 100%. Conclusion: The online outpatient clinic was feasible and effective. It has made beneficial attempts for the development of Internet+ wound care service in the future.

In order to give full play to the professional advantages of medical colleges and universities, actively explore new models of community prevention and control, contain the spread and extending of epidemic situation, do a good job in community epidemic prevention. Through the establishment of community protection teams to fight against new coronavirus pneumonia, the establishment of an online long-distance health consultation platform, assistance to community health service centers in house-to-house investigation, family-based popular science education, Chinese Medicine epidemic prevention guidance and other initiatives, a "school-community-family" triple epidemic prevention and control program was adopted for about 200,000 residents of four communities in Chengdu Longdu and Park Road. The results showed that there was no case of infection in the four communities, which achieved good results and ensured the health and life safety of families. In the face of public health emergencies, multiple agency including medical schools, community health service centers and families participated, which helped community residents master convenient and effective epidemic prevention measures, made positive contributions to the community's anti-epidemic efforts, and provided reference for the community to deal with public health emergencies.

Impact of the Internet+three-level linkage on the dietary structure of diabetes family

Objective: To evaluate the effect of the Internet+ three-level linkage management model on the dietary structure and patient's blood glucose in diabetic family. Methods: From April to July 2016, 43 families of type 2 diabetes (including 43 patients with type 2 diabetes and 43 family numbers) diagnosed in the Endocrinology Department of a tertiary grade A hospital and with poor blood glucose control were recruited. The International+ three-level linkage management model was applied to the subjects for 18 months. Data on dietary structure and changes in blood glucose in diabetic patients were collected at preliminary screeing, 3, 6, 9, 12, and 18 months respectively, and compare with the recommended amount. A single set of time series was used for ANOVA of repeated measuring. The trend of family lifestyle sedatives and of blood glucose changes were observed. Results: Through the 18-month intervention, six repeated ANOVA results of diabetes patient's animal products, aquatic products and edible oil were statistically significant (P<0.05). There were significant statistical differences in family member's staple food, vegetables, fruits, aquatic products, eggs, soy products, and edible oil (P0.05). The fasting blood glucose of the patients decreased from 8.23±1.95 mmol/L to 7.91±2.02 mmol/L at 18 months of intervention. Conclusion: The Internet + three-level linkage management model can urge diabetic families to improve their current lifestyles, helping diabetic patients control blood sugar better.

Objective: To evaluate health-promoting lifestyles status and their influencing factors among the medical nursing assistants. Methods: A cross-sectional descriptive design and convenience sampling method study was conducted. Totally 634 nursing assistants form east, central and west of China were investigated using a general demographic questionnaire, the Health-promoting Lifestyles Profile-II, Health Conception Scale and Minnesota Satisfaction Questionnaire to measure their health-promoting lifestyles status from March 2019 to October 2019. Results: The average score of the Health-Promoting Lifestyles total score was 124.79±35.47. For the six dimensions, the item average scores of health responsibility, physical activity, nutrition, spiritual growth, interpersonal relations, and stress management were 2.30±0.75, 2.13±0.83, 2.50±0.71, 2.53±0.67, 2.65±0.75, and 2.23±0.76, respectively. Multiple regression analysis revealed that the predictor variables, such as working years, health conception, workplace, education, job satisfaction and number of care recipients, explained 85.3% of the variance in health-promoting lifestyles. Conclusion: The level of health-promoting lifestyles is average in medical nursing assistants, with the highest score in interpersonal relations and the lowest score in physical activity. The variables of working years, health conception, workplace, education, job satisfaction and number of care recipients can serve as effective predictors of health-promoting lifestyles.

Objective: To evaluate the effect of fourteen oral hygiene methods on preventing Ventilator-Associated Pneumonia (VAP) systematically. Methods: The electronic portion of the search covered seven computer databases (PubMed, Cochrane library, Web of Science, EMbase, CBM, CNKI, Wanfang Data). Articles and reference lists were searched for Randomized Control Trials (RCTs) of oral hygiene methods for preventing VAP. The literature search covered articles that had been published from inception to June 2019. Results: A total of 26 RCTs involving 3 548 patients and 14 oral hygiene methods were included. The network Meta-analysis showed that, the incidence of VAP is higher in traditional swab scrubbing group than brushing the teeth+0.12% chlorhexidine [OR=3.00, 95% CI=(1.40, 7.30)], rinsing [OR=3.80, 95% CI=(1.10, 14.00)], brushing the teeth [OR=5.10, 95% CI=(1.80, 15.00)], brushing the teeth+0.12% chlorhexidine digluconate [OR=0.44, 95% CI=(0.23, 0.80)], brushing the teeth+rinsing [OR=0.22, 95% CI=(0.13, 0.37)], brushing the teeth+rinsing+0.12% chlorhexidine [OR=0.26, 95% CI=(0.09, 0.70)], brushing the teeth+toothpaste [OR=0.40, 95% CI=(0.21, 0.75)], Rinsing with nasopharyngeal cavity [OR=0.26, 95% CI=(0.10, 0.66)], brushing the teeth+toothpaste+toothpaste [OR=0.25, 95% CI=(0.08, 0.63)], brushing the teeth+toothpaste+deep pharyngeal suction [OR=0.21, 95% CI=(0.05, 0.81)], and all the differences had statistically significant (P<0.05). The result of rank probability showed that brushing the teeth+toothpaste+deep pharyngeal suction had the highest probability to be the optimum method. Conclusion: This study suggested that brushing the teeth + toothpaste + deep pharyngeal suction was the best method to prevent VAP. In future, researchers should pay attention to the economic costs, safety outcomes, nursing work difficulty and nurse workload of oral hygiene.

Objective: To summarize the evidence to prevent Emergence Agitation in children undergoing general anesthesia, and provide reference for clinical practice. Methods: The Chinese Guideline Network, National Guideline Clearinghouse (NGC), Scottish Intercollegiate Guidelines Network (SIGN), Up To Date, Joanna Briggs Institute (JBI), Embase, BMJ Best Practice, Guideline International Network (GIN), National Institute for Health and Care Excellence (NICE), Registered Nurses Association of Ontario (RNAO), CINAHL, Cochrane Library, PubMed, CNKI, VIP, CBM, WAN FANG and anesthesia professional association websites were systematically searched. The evidence on the prevention of Emergence Agitation in children undergoing general anesthesia including guidelines, expert consensus, evidence summary, best practice information sheet, systematic review and original study was collected. Two authors independently assessed the quality of the studies that met the inclusion criteria and extracted data. Results: A total of 21 articles were selected, including 3 guidelines, 3 expert consensus, 6 systematic reviews and 9 randomized controlled trials. Totally 14 best evidences were summarized, including high risk factors assessment, preoperative cognitive behavior intervention, perioperative mood regulation, adverse stimulation reduction, and drug prevention. Conclusion: The healthcare workers should receive regular training on agitation after the general anesthesia, combine the best evidence with medical condition and patients' intention, to develop individualized intervention strategies in order to reduce the incidence of Emergence Agitation in children undergoing general anesthesia.

Evidence summary of early enteral nutrition in patients with acute pancreatitis

Objective: To evaluate and integrate evidence of nutritional support pathways and timing of enteral nutrition treatment, route of enteral feeding and nutrition supplements in patients with acute pancreatitis. Methods: Cochrane Library, PubMed, Embase, CINAHL, medlive, CNKI, WanFang Data, GIN, NICE, SIGN, JBI Library, RNAO were searched. The Evidence and Grades of Recommendation system of the Australian JBI Evidence-Based Health Care Center and Appraisal Guidelines for Research and Evaluation (AGREE II) were used to evaluate the quality of included studies. Results: A total of 538 studies were retrieved. After removing the duplicates and checking the inclusion and exclusion criteria, 11 guidelines were included. This study summarized 16 pieces of evidence of nutritional support in patients with acute pancreatitis, including nutritional support pathways, timing of enteral nutrition treatment, route of enteral feeding, and nutrition supplements. For patients with mild acute pancreatitis, it is recommended to take oral nutrition as early as possible. If oral nutrition is not tolerated, it is recommended to provide enteral nutrition first. Only when enteral nutrition fails or is contraindicated, total parenteral nutrition is considered to be given. In terms of dietary choices, patients with mild acute pancreatitis are not limited to clear liquid diets, and can be given low-fat or normal-fat soft or solid diets according to their preferences. In patients with moderate to severe acute pancreatitis, it is recommended to start enteral nutrition with standard polymeric formula after adequate fluid resuscitation. While evidence on semi-elemental, elemental formulations, immune-enhancing preparations, probiotics, fiber formulations, and ω-3 fatty acids is limited, and should be reconsidered carefully based on patients’ clinical condition. Conclusion: Through multidisciplinary teamwork, providing nutritional support at the right time and in an appropriate manner can reduce the intensity and duration of abdominal pain in patients with acute pancreatitis, and shorten hospital length of staying. It can also reduce infection complications and mortality, improve the patient's satisfaction and the clinical nursing quality.

Objective: To investigate the clinical nurses' attitudes and willingness on part-time position for new nurses, and to provide references for the formulation of policies with part-time nursing positions. Method: A questionnaire survey was conducted in 163 new nurses and 145 other part-time nurses in a hospital in Beijing. Results: Totally 59 (36.2%) and 78 (47.9%) new nurses, 62 (42.8%) and 61 (42.1%) other part-time nurses expressed strong supports and supports for the establishment of part-time position for new nurses. The awareness of benefits with part-time nursing positions for new nurses included improving nursing skills, increasing income, and promoting the effective use of human resources, and the awareness of disbenefits included affecting the quality of nursing care and increasing workload. Totally 119(73.0%) new nurses expressed willingness to participate in part-time work. The score of the current situation and prospect of part-time nursing position for new nurses were over 8. Conclusion: Clinical nurses generally hold a positive attitude towards part-time nursing positions. In order to promote the healthy development of part-time nursing position for new nurses, it's necessary to clarify the duties, admission qualification, evaluation standards and salary system for part-time nurses, and to establish an independent assessment mechanism.

Objective: To investigate the characteristics of pressure injury related to medical devices, and to explore the relevant factors. Methods: A retrospective investigation was conducted in the incidence of medical device related pressure injury from 2014 to 2018, and collected the data about the characteristics of patients, the types of medical device, and the distribution, location and stage of pressure injury, and the above data was compared with the collected data from other pressure injuries. Results: Totally 134 cases of pressure injury and 39 cases of device related pressure injury were collected. The devices included oxygen inhalation device, blood pressure cuff, antithrombotic elastic socks, and so on. The sites of injury included bony protrusion and some non-bony protrusion, such as upper arm, thigh, finger. There were significant differences in the age, distribution of wards and score of Braden between the group with medical devices and other groups. The incidence of medical device-related pressure injury was 4.561 times higher in the emergency department than general ward. Conclusion: The incidence of medical device-related pressure injury increased year by year, and there were many related risk factors with it. Effective countermeasures should be formulated to prevent its occurrence.

Objective: To analysis the track and rules of improved nursing quality by conducting periodic measurement and continuous improvement of nursing quality of the core work of nurses, including drug administration, blood transfusion, drug allergy experiment, specimen collection and submission, and monitoring. Methods: From 2015 to 2018, 201254 pieces of corresponding data were extracted from the clinical information system during the implementation of medical orders by nurses. Nursing quality was classified and measured by indicators including accuracy, timeliness and effectiveness. Three months was referred as a cycle of quality and the data of each quality indicator at different periods were compared. Results: Compared with the baseline quality, statistically significant difference (P<0.05) appeared in the second quality improvement cycle, almost all results of the quality cycle were superior to the baseline data. Compared with the quality of the previous cycles, statistical difference (P<0.05) appeared in the quality improvement cycle and maintenance time. Conclusion: The off-site quality measurement of big data achieves objective, fair and real-time quality measurement, forms a specialized, scientific and accurate quality control process with specificity, collection and replication, and reveals the trajectory and rules of quality improvement.

Objective: To explore the effect of digital prevention and control system based on full risk management in the care of Venous Thrombosis (VTE). Methods: A digital prevention and control system based on full risk management of venous thrombosis was established relying on the nursing information platform by our hospital and was applied to clinical practice. Data randomly collected by the nursing management system from October 2017 to August 2018 as the baseline data before the application of the digital system was compared with the data acquired after the application from January to November of 2019. Differences in the timeliness of VTE risk assessment before and after applying the digital system, the accuracy of VTE assessment, the execution rate of VTE preventive measures, and the awareness rate of VTE prevention were compared. At the same time, the nursing sensitive indicators of VTE incidence 11 months before and after the application of the digital system were monitored and compared. Results: After the employment of the digital prevention and control system based on full risk management in the care of VTE, the timely rate, accuracy of VTE risk assessment, the execution rate of VTE preventive measures and the awareness rate of VTE prevention were significantly improved with statistically significant differences (P<0.05). The nursing sensitive indicators of VTE incidence after application was 0.05% lower than the baseline value of 0.25%, and the difference was statistically significant (P<0.05). Conclusion: The digital prevention and control system based on full risk management has been applied to the care of VTE, which can optimize the management of VTE prevention and control to obviously improve the quality of VTE nursing, reduce the incidence of VTE, and thus deserve further clinical promotion.

Objective: To investigate the quality of discharge teaching service, readiness for hospital discharge, and post-discharge health outcomes of cataract patients in the day surgery ward, and to explore the relationships among these variables. Methods: This cross-sectional study design employed a convenience sample of 194 cataract patients from the ophthalmic day surgery ward in a tertiary level general hospital in Sichuan province from October 2017 to January 2018. The general information questionnaire, Quality of Discharge Teaching Scale (QDTS), Readiness for Hospital Discharge Scale (RHDS) and Post-discharge Health Outcome Questionnaire were used for the survey. Results: A total of 194 patients completed the study, and 185 (95.40%) patients answered that they had been ready to leave the hospital on the day of discharge. The median score of quality of discharge teaching service, readiness for hospital discharge, and post-discharge health outcomes were 196.00, 179.50 and 78.00, respectively. Two weeks after discharge, the median visual acuity of the patients was 0.80, and no complications occurred. However, 36 (18.56%) patients had eye discomfort. It was found that the score of quality of discharge teaching service was positively correlated with the score of readiness for hospital discharge (r=0.59, P<0.001).The score of readiness for hospital discharge had a positive correlation with the score of post-discharge health outcomes (r=0.27, P<0.001), and the score of quality of discharge teaching service was also correlated with the score of post-discharge health outcomes (r=0.25, P=0.001). Conclusion: Cataract patients in the day surgery ward receive good quality of discharge teaching service. Their readiness for discharge and post-discharge health outcomes are good, too. However, the discharge teaching content provided to patients couldn't fully meet their needs. Therefore, we as medical workers should pay more attention to the patients' individualized needs of discharge teaching service, and evaluate their understanding of the discharge teaching content. Meanwhile, we should emphasize the importance of compliance behaviors to patients in order to better prevent complications and promote postoperative rehabilitation.

Objective: To investigate the effect of family integrated-care in reducing precrural pain due to heel stick among premature infants in the Neonatal Intensive Care Unit (NICU). Methods: Following convenient sampling, between June, 2018 and February, 2019, a total of 72 premature infants were included in the study and randomized into control group (n=35) and family integrated-care group (n=37) which included daily care, sensory intervention and kangaroo care with cooperation between parents and nurses. The Preterm Infant Pain Profile-Revised was used to assess the procedural pain after family integrated-care was implemented at three time points: after the third, fifth and seventh intervention. Results: Among infants in the family integrated-care group at each of the three time points, the scores of PIPP-R were 8.14±1.03, 6.65±0.86, 6.51±1.37, respectively, while the control group reported were 11.14±0.88, 11.00±084, 10.43±0.70, respectively. The result showed that, compared to the control group, the physical status and sensitivity to pain among infants in the family integrated-care group were significantly better after the fifth and seventh intervention (P<0.05). Conclusion: The family integrated-care could relieve premature infant procedural pain in NICU, promote effective communication among neonatal families, which is proved as an effective, safe and economical approach to neonatal pain relief among premature infants and is worthy of being introduced to NICU.

Objective: To explore the status of lower extremity Deep Vein Thrombosis (DVT) and its related factors in neurocritical patients. Methods: The related clinical data of 223 neurocritical patients were collected, and Logistic regression was used to analyze the risk factors of DVT. Results: Univariate analysis showed that age over 60 years, stay in bed for a long time, higher D-dimer, left leg muscle strength level with 3 or less, lower GCS score, central venous catheter placement were related factors (P<0.05) with DVT in lower extremity in neurocritical patients. The Logistic regression analysis showed that age over 60 years, stay in bed for a long time and higher D-dimer were independent risk factors with DVT. Conclusion: Those with the age over 60 years, in bed for a long time and with higher D-dimer were the focus of prevention and control of DVT in neurocritical patients, and the related nursing strategies are needed in the future.

Objective: To analyze the feasibility and safety of the improved ECG dedicated conversion clip in PICC tip positioning. Methods: Prospective control research was conducted. In the test group (n=551), the non-sterile ECG dedicated conversion clip was wrapped in a sterile sheath to guide the intracaval ECG. In the control group (n=551), the sterile ECG dedicated conversion clip was directly used to guide the intracaval ECG. The CRBSI, stability of the ECG complexes and accuracy of catheter tip position were compared. Results: CRBSI did not occur in either group. The stability of the electrocardiogram complexes (P=0.376) and accuracy of catheter tip position (P=0.367) were similar between the two groups. Conclusion: The using of improved ECG dedicated conversion clip to guide the intracaval ECG is a safe, aseptic and convenient way.
