The intelligent care team was built up based on the hospital high quality resources and the opportunity of medical partnership. We established the hospital-community-family intelligent care by the aid of phone application and the wound/stoma care clinic. Through the model, we provided convenient and high quality services to the patients. Besides, a continuous care network was also set up. All the efforts we made were aimed to increase the number of visiting community health centers, decrease the average cost of dressing change, and improve the sense of gain for patients. Thus, we can improve the specialized nursing care level in the community and promote the policy implementation of hierarchical medical system.

Objective: To explore the effects of the nursing homogeneity platform developed in pediatric integrated health care organization in Huaihai economic zone. Methods: In July 2016, the nursing homogeneity platform was built up in pediatric integrated health care organization based on the information technology. We compared differences regarding the average length of stay in hospital, cost, satisfaction level of children's parents, and care quality before and after the platform in the pediatric department of a primary hospital. Results: After the platform was constructed, the average hospitalization days and hospitalization expenses of children with medical diseases were reduced; the satisfaction of parents of discharged children was improved; the quality of graded care, basic nursing quality, nursing safety quality score and nursing training of grassroots hospitals were improved (P<0.05). Conclusion: Nursing homogeneity platform developed in pediatric integrated health care organization can help integrating the resources of the primary hospital, reduce patient's average length of stay in hospital and cost, promote the level of caring of children, and increase the service ability of pediatric nursing in the primary hospital. All these can promote the development of hierarchical medical organization.

Objective: To establish a chronic wound care model of hospital community linkage and to explore its effects on health professionals' examination performance and patient's satisfaction level. Methods: Firstly, we established a team of chronic wound care of hospital community linkage. Team regulations and personal responsibilities were developed. The wound care skills and patient management ability of community health professionals were trained through chronic wound care management platform in respective communities. The chronic wound care model was accordingly built up through the measures we took. The self-control study design was used. We compared the number of patients admitted to the community stations, patients' satisfaction with the care, and community nurses' knowledge and skill levels before and after the model used. Results: After the implementation of the model in 2015, a total of 54 community stations joined in the chronic wound care platform. A total of 53 patients with chronic wounds were treated by this model and 51 patients of them were totally cured with a cure rate of 96.23%. During the model was implemented, we totally conducted 88 on-site consultations and 452 remote directions. Before and after the model implementation in 2015 and 2018, the performance regarding the examinations passing rates community health professionals had been improved (P<0.001). Patients with chronic wound had increased satisfaction level (P<0.001). Conclusion: The chronic wound care model of hospital community linkage can improve the wound management abilities of health professionals in the community. It can standardize the chronic wound management strategies as well. It is helpful to ensure the treatment continuity of patients and improve their senses of gain in medical treatment. On the other hand, it is also beneficial to classified wound management and improve the nursing care system of chronic wound.

Study on community hypertension management model linked with senior nurses

Objective: To explore the effects of community hypertension management model with senior nurses as a link. Methods: Senior nurses in cardiovascular department of a tertiary general hospital were arranged into community health service center, and formed characteristic management team in the medical union. One hundred hypertensive patients were managed with senior nurses as a link. The differences of basic-level hypertensive management ability, blood pressure compliance rate and self-management ability of community staff before and after intervention were compared. Results: The community hypertension management model with senior nurses as a link can improve the basic level hypertension management knowledge of community staff, the blood pressure compliance rate and self-management ability of patients apparently. Conclusion: The community hypertension management model with senior nurses as a link has good effects and can be extended to more patients with hypertension.

Based on the internet and information communication technology, we discussed an "Internet+" hospital-community-family nursing service model which is operated in the form of "applying online and serving offline", and in collaboration with necessary medical and health services, naming as integrated health care organization. In this study, we introduced the organizational framework, service provider, target population, main services, adverse event processing, responsibilities, risk prevention and control, bill payment as well as the platform construction, and explained the operational and collaborative mechanism of this service model. After in-depth analysis of the development needs and suggestions, with adequate preparation and practice, this collaborative nursing service model has been well established and implemented. This "Internet+" hospital-community-family collaborative nursing service model opens up the channel for combining nursing and internet, improves the satisfaction from both patients and nursing staff, broadens the nursing care methods, extends the nursing scope of practice, and provides a new approach for nursing care model in China.

Mediating effect of self-efficacy in the relationship between diabetes related distress and self management in empty nest elderly patients with Type 2 Diabetes

Objective: To explore the mediating effect of self-efficacy in the relationship between diabetes related distress and self management in empty nest elderly patients with type 2 diabetes. Methods: A total of 347 cases of empty-nest elderly patients with type 2 diabetes from two general hospitals were investigated using the Self-Efficacy for Diabetes (SED), the Diabetes Distress Scale (DDS) and the Summary of Diabetes Self-Care Activities (SDSCA). Results: Totally 68.59% of patients suffered from diabetes related distress. There was significant relationship between self-efficacy, diabetes related distress and diabetes self-care activities (P<0.001). Self-efficacy played as the mediator between diabetes related distress and self management. Conclusion: Diabetes related distress is common amongst empty nest elderly patients with type 2 diabetes.The level of their self management and self efficacy is low. Self-efficacy plays a totally mediating effect between them.

Objective: To develop a compliance scale of functional exercise for orthopedic patients and test the reliability and validity of the scale. Methods: Based on the conceptual framework of functional exercise and compliance theory, the initial scale was formed through semi-structured interview and literature review, and the formal scale was developed through expert consultation, pilot testing and large sample testing, and the reliability and validity testing was performed among 1024 postoperative orthopedics patients. Results: The Compliance scale of functional exercise for orthopedics patients included 15 items and 3 dimensions. The cumulative contribution of variance was 69.809%. The Cronbach's α coefficient for the total scale was 0.930, the mean inter-item correlations was 0.494, and the content validity index was 0.936. Conclusions: The Compliance scale of functional exercise for orthopedics patients has been shown good reliability and validity. It can be used as a valid tool to assess the compliance of functional exercise for orthopedics patients.

Reliability and validity of the Chinese version of the Simulation Effectiveness Tool Modified

Objective: To translate the Simulation Effectiveness Tool Modified (SET-M) and test the reliability and validity of the Chinese version. Methods: According to guidelines for cross-culture adaptation process, the Chinese version of SET-M was formed through the translation, back-translation, back-translation review, expert consultation, and pilot study. A convenient sampling method was used to recruit 155 nursing students from two comprehensive universities, who participated in the simulation. The questionnaire included the Chinese version of SET-M and Debriefing Assessment for Simulation in Healthcare Student Version (DASH-SV). Results: The correlation coefficients for each item and total score were between 0.67 and 0.88. The Cronbach's alpha coefficient of the total scale was 0.89, and the Cronbach's alpha coefficient of each dimension was between 0.74 and 0.86. The retest reliability of the total scale was 0.88. Content Validity Index (CVI) of total scale was 0.80. The subscale Confidence, Debriefing and SET-M total score were moderately correlated with DASH-SV total score (P<0.001). Four factors were extracted by exploratory factor analysis, and the cumulative variance contribution rate was 82.29%. All items had a load value of >0.40 on the common factors. Conclusion: The Chinese version of SET-M has good reliability and validity, and can be used in the evaluation of the effectiveness of the simulation teaching in nursing undergraduate education of China.

Objective: To translate and revise the Tampa Scale for Kinesiophobia Heart (TSK-SV Heart), and to verify the reliability and validity of the revised Chinese version of the scale, to provide an effective assessment tool for the kinesiophobia in patients with heart disease. Methods: The Brislin model was used to forward-back translation of the scale. Based on expert discussion and pilot survey, the scale was culturally revised. Totally 550 patients with coronary heart disease completed the questionnaire survey through convenience sampling method from a tertiary hospital in Tianjin. The results of the survey were statistically analyzed using SPSS 21.0 to measure the reliability and validity of the scale. Results: The Chinese version of TSK-SV Heart contained 17 items. Exploratory factor analysis identified 4 principal factors, which explained 61.389% of the total variance. The confirmatory factor analysis showed that the model fit well. The content validity analysis showed that S-CVI/UA was 0.824, I-CVI was 0.800-1.000, S-CVI/Ave was 0.965. The Cronbach's α coefficient of the scale was 0.859, and the Cronbach's α coefficients of each dimension were 0.743-0.824. The split-half reliability of the scale was 0.792, and the split-half reliability of each dimension were 0.810-0.821. The test-retest reliability of the scale was 0.792, and the test-retest reliability of each dimension were 0.674-0.740. Conclusion: The Chinese version of TSK-SV Heart has good reliability and validity, and can be used to evaluate the level of kinesiophobia in Chinese patients with heart disease.

Objective: To investigate the levels and related factors of knowledge, attitude and practice towards donor milk on mothers of infants aged 0-6 months. Methods: An online survey was conducted with the knowledge and attitude questionnaire of human milk donation. Results: Totally 311 valid questionnaires were collected. There were 34.1% of the participants have heard of donor milk or milk banks, 74.0% were willing to donate their milk, but only 2.6% had donated their milk, and 3.5% had used the donor milk. The linear regression analysis demonstrated that the age, education level, living pattern, parity, history of delivering preterm or low-birth-weight infants and infants' age (month) were variables of knowledge towards donor milk. The parity, milk yield, history of delivering preterm or low-birth-weight infants, introduction by health professionals and health of infants were variables of the attitude. Conclusion: The attitude towards donor milk on mothers of infants aged 0-6 months was positive, but the knowledge still needed to be improved, and donating or using milk from breast milk bank was not active. The difference exited in the knowledge and attitude towards donor milk among mothers with different characteristics.

Construction and verification of early warning evaluation model for Postpartum Hemorrhage

Objective: To construct an early warning evaluation model for Postpartum Hemorrhage and verify its validity. Methods: A retrospective analysis of 6790 maternal cases in a tertiary hospital in Beijing was conducted to verify the validity of the Postpartum Hemorrhage warning evaluation model. Results: The area under ROC curve of the evaluation model is 0.632 [95%CI(0.613, 0.651)]. The optimal cutoff value is 5 points, and the corresponding sensitivity is 51.9% [95%CI (48.7%, 55.1%)], the specificity was 66.7% [95%CI (65.5%, 67.9%)]. Conclusion: The early warning model of Postpartum Hemorrhage has certain accuracy, which provides a model for clinical medical staff to predict the occurrence of Postpartum Hemorrhage. Early warning and interventions of Postpartum Hemorrhage should be taken to reduce the occurrence.

Construction and practice of training system on gridding home-based rehabilitation nursing management

Objective: To establish a training system for home-based rehabilitation nursing and improve professional nursing abilities. Methods: Through training center for home-based rehabilitation nursing, integrating the nursing training resources from comprehensive hospital in various regions, and applying flexible and diverse training methods, the training management and follow-up of the nursing staff from the township hospitals and training system on home-based rehabilitation nursing management was established and improved, and then the training mechanism of home-based rehabilitation nursing was established. Results: The skills training center for home-based rehabilitation was carried out three sessions in the city of Taizhou from 2017 to 2018. A total of 366 primary nursing staff from 153 township health centers and community health service centers in the city were educated. The pass rate of theoretical achievement and skill assessment are 100% and 95.4% respectively. The overall satisfaction of the trainees on the skills training center and the training practice units of home-based rehabilitation were 97.8% and 96.2% respectively. Conclusion: The gridding skill training system on home-based rehabilitation nursing can improve professional nursing abilities. It can strengthen the construction for primary medical and health care talents and promote the quality of life for elderly patients in the community.

Objective: To investigate the nurses practice environment in Qinghai Province, analyze the main influencing factors. Methods: the nurses practice environment scale was used. Results: A total of 7578 registered nurses were enrolled, the overall score is (73.90±25.88) points, the highest score of dimension was the medical cooperation (79.17±0.54) points, the lowest score of dimension was nurses involved in hospital affairs (46.47±15.28) points, and the influencing factors were the hospital level, type, gender, age, authorized strength, job title. Conclusion: The overall evaluation of the nurses practicing environment in Qinghai province is acceptable, and the hospital level, type, gender, age, authorized strength, job title are the main influence factors.

The influence of professional quality of life on work engagement among nurses in the Intensive Care Unit

Objective: To investigate the status of professional quality of life and work engagement among nurses in the Intensive Care Unit (ICU), and to analyze the influence of professional quality of life on work engagement. Methods: A convenient sample of 434 ICU nurses recruited from 14 tertiary grade A hospitals in Hunan were evaluated using the general information questionnaire, the Chinese version Professional Quality of Life Scale, and the Chinese version Utrecht Work Engagement Scale. Results: The mean scores of work engagement and the three dimensions of professional quality of life: compassion satisfaction, burnout, and secondary traumatic stress, among ICU nurses were 57.24±20.33, 28.62±7.85, 27.95±6.26, and 24.52±6.27, respectively. Work engagement was significantly and positively correlated with compassion satisfaction (r=0.751, P<0.01) as well as negatively correlated with burnout (r=-0.555, P<0.01), but not correlated with secondary traumatic stress (r=0.038, P>0.05). The overall professional quality of life can independently explain 49.5% variation of work engagement after controlling for demographic variables. Conclusion: In our study, the level of work engagement is moderate in ICU nurses. It is suggested that the overall professional quality of life can partially predict work engagement in this sample. The level of work engagement could be encouraged by improving the professional quality of life among ICU nurses, therefore, to increase quality of care.

Objective: To explore the application effect of a health management APP in improving the quality of life of patients with nasopharyngeal cancer receiving radiotherapy. Methods: 149 cases of nasopharyngeal cancer inpatients receiving concurrent radiotherapy and chemotherapy from December 2017 to August 2018 in the department of radiotherapy of a tertiary grade A hospital in Guangxi were selected as research objects, and were evenly divided into the control group and the observation group according to the random number table method. The control group was given routine health education in the radiotherapy department, while the observation group was given interventions based on the mobile medical APP aside from routine health education in hospital.The quality of life of patients in the two groups before radiotherapy, after radiotherapy for 15 times and after radiotherapy for 30 times were compared respectively using repeated measurement anova. Results : After intervention, there was no statistically significant difference between the observation group and the control group in the symptom scores of tooth problems, mouth opening difficulties, sticky saliva, oral nutrition and weight gain (P>0.05). However, the scores of other symptom fields of the observation group were all lower than those of the control group, and the difference was statistically significant (P<0.05). Conclusion: Health management based on the mobile medical APP can effectively relieve or alleviate adverse reactions of patients and improve their quality of life.

Construction and implementation of hospital-home linkage management model for postoperative lymphedema of breast cancer

Objective: To construct a hospital-home linkage management model for lymphedema after breast cancer surgery and determine its effect. Methods: With the hospital as the center, the core team was set up to train specialized nurses to optimize the work process and system, and the lymphedema clinic was extablished to build an information communication platform which could link patients and their families at home, and finally to construct and implement the hospital-home linkage management model of lymphedema after breast cancer surgery. Indicators including the incidence of lymphedema after breast cancer surgery, the nurses' competency, and the self-management ability of lymphedema of patients in hospital and at home were compared before and after the implementation of this model. Results: After the implementation, the nurses' specialized competency and the patients' self-management ability over lymphedema in hospital and at home after breast cancer surgery were improved (P<0.05). The incidence of lymphedema decreased from 24.6% to 18.2%. Conclusion: Since the establishment and implementation of the hospital-home linkage management model of postoperative lymphedema for breast cancer can effectively improve the competency of breast specialist nurses, help patients with breast cancer to do better in postoperative lymphedema self-management so as to improve the clinical outcome, it is feasible and can be widely used in the future.

The application of standard blood sampling procedure for Clinical Practice Standards for Arterial Blood Gas Analysis

Objective: To explore the practicability and effectiveness of the standard blood sampling procedure that suggested in the Clinical Practice Standards for Arterial Blood Gas Analysis in ICU wards. Methods: A before-and-after historical comparison study design was used, and the cost time, success rate and medical cost of the arterial blood collection before and after the implementation of the standard blood sampling procedure were collected and analyzed. Results: The one-time success rate of puncture for arterial blood collection increased from 90% to 97% (χ2=4.031, P=0.045), the defect rate of specimens was lowered (χ2=5.216, P=0.036), the operation time was shortened (t=3.162, P=0.002), and the medical cost for the operation was reduced (t=-2.196, P=0.029) after the implementation of the standard blood sampling procedure in Clinical Practice Standards for Arterial Blood Gas Analysis. Conclusion: The standard blood sampling procedure in Clinical Practice Standards for Arterial Blood Gas Analysis was applicable for arterial blood collection in ICU ward, and it is benefit to save operating costs.

his article reviews the current status of evaluation tools and non-pharmacological intervention of pain in patients with dementia. It is recommended that medical staff and caregivers should pay full attention to pain-related problems in patients with dementia, in order to improve the quality of life of dementia patients and reduce the burden of family and society. At the same time, it also provides prospects for future research to provide reference for clinical nursing and research.

Dynamic investigation of psychological status of family members in transition from ICU

Objective: To investigate the current status of psychological pressure of ICU patients' family members during transitional period, and to provide reference for preventive strategies. Methods: Totally 61 ICU family members who met the inclusion criteria were recruited according to the convenient sampling method.All the patients were investigated by Family Relocation Stress Scale (FRSS), Hospital Anxiety and Depression Scale (HADS), and The Impact of Event Scale-Revised (IES-R) 24 to 48 h before transferring from ICU, after 24 to 48 h, and 7 d after transferring from ICU. Results: The psychological problems of 61 families were different.The incidence of high-risk or positive symptoms of FRSS, HADS-A, HADS-D, and IES-R (invasion+avoidance) in the transitional period was 100.00%, 94.54%, 98.91%, and 67.76%, respectively. The stress source of family members in the transitional period mainly comes from the worry about the patient's condition and the anxiety about the medical conditions and nursing carein the general ward. The scores of HADS and IES-R of family members in transitional period showed a downward trend with the passage of time and the recovery of patients' condition (P<0.001). Conclusion: The family members in the transitional period of ICU transition have serious psychological problems. Although the psychological pressure reduce over time, we still should pay more attention and take effective measures to reduce the stress.

This article reviews of the epidemiological characteristics, classification, influencing factors of neonatal skin lesion, as well as the content and application status of the risk assessment tools. It is found that scholars have different classification of neonatal skin injury and risk factors. The definition of neonatal skin injury is not clear enough. The incidence of neonatal skin lesions has not been fully documented, and it has been mixed with the incidence of children. The existing risk assessment tools for neonatal skin injury cannot meet clinical needs. A larger database is needed to accurately measure the epidemiological characteristics of neonatal skin injury, and it is necessary to develop a neonatal skin risk assessment tool that meets the clinical needs in China.

Effects of swallowing therapy combined with family rehabilitation training on deglutition disorders in children with cerebral palsy

Objective: To analyze the effects of swallowing therapy combined with family rehabilitation training on deglutition disorders in children with cerebral palsy. Methods: Sixty cases of cerebral palsy children with dysphagia who met the inclusion criteria were selected and divided into observation group and control group by random number table method, 30 cases in each group. Both groups were given swallowing therapy, including neuromuscular electrical stimulation and swallowing training, and the observation group was given individualized family rehabilitation training on this basis. Before treatment and after 3 period of treatment, the two groups used the degree of dysphagia grade and the Watian drinking water test and parental satisfaction to evaluate the degree of deglutition disorder and parental satisfaction, and compared the effects of the two groups after treatment. Results: After treatment, the effective rate of swallowing function improvement in the observation group was significantly better than that in the control group (P<0.01), and parents' satisfaction in the observation group was significantly higher than that in the control group (P<0.05). Conclusion: Swallowing therapy combined with family rehabilitation training is better than swallowing therapy alone in improving the swallowing function of children. Therefore, medical staff should recognize the importance of family rehabilitation and give reasonable and correct guidance.

Obstructive Sleep Apnea (OSA) is a common sleep and breathing disorder which could cause a variety of potential impairments on neurological, cardiovascular, metabolic and cognitive functioning. The OSA seriously affects the Quality of Life of patients with this condition. Through review of the currently available Quality of Life assessment tools, the identified influencing factors and interventions in adults with OSA in existing research, the aim of this article is to provide empirical evidence and reference basis in guiding related research and clinical practice in patients with OSA in China.
