Abstract: Based on the internet and information communication technology, we discussed an "Internet+" hospital-community-family nursing service model which is operated in the form of "applying online and serving offline", and in collaboration with necessary medical and health services, naming as integrated health care organization. In this study, we introduced the organizational framework, service provider, target population, main services, adverse event processing, responsibilities, risk prevention and control, bill payment as well as the platform construction, and explained the operational and collaborative mechanism of this service model. After in-depth analysis of the development needs and suggestions, with adequate preparation and practice, this collaborative nursing service model has been well established and implemented. This "Internet+" hospital-community-family collaborative nursing service model opens up the channel for combining nursing and internet, improves the satisfaction from both patients and nursing staff, broadens the nursing care methods, extends the nursing scope of practice, and provides a new approach for nursing care model in China.
