门诊护士的一般自我效能感在工作环境感知和共情疲劳间的中介效应

The mediating effect of general self-efficacy in the relationship between perceived nursing work environment and compassion fatigue among nurses in outpatient departments

摘要: 目的:研究门诊护士工作环境感知、一般自我效能感和共情疲劳之间的关系,为降低护士的职业倦怠水平,进一步提高门诊护士的工作满意度提供依据。方法:采用便利抽样法,于2022年4月15日—30日分别对来自4所三级甲等医院的门诊护士进行问卷调查,调查工具包括一般资料调查表、中文版共情疲劳量表、中文版护士工作环境感知量表、一般自我效能感量表。结果:在纳入研究的290名门诊护士中,职业倦怠得分为中等的人占51.4%,继发性创伤应激发生率为72.8%。工作环境感知中的人力与物力得分最低,为(3.18±0.71)分,一般自我效能感得分为(2.82±0.62)分。结构方程图显示,门诊护士的工作环境感知与一般自我效能感、同情满足呈正相关,与职业疲倦、继发性创伤应激呈负相关,一般自我效能感在工作环境感知和共情疲劳之间起部分中介作用。工作环境感知通过一般自我效能感进一步减少共情疲劳的产生。结论:门诊护士的共情疲劳有待提高,管理者在创建良好的工作环境时,应提高门诊护士的人员配置和薪资水平,并重视对护士一般自我效能感的培养。

Abstract: Objective: To explore the mediating role of general self-efficacy in the relationship between perceived nursing work environment and compassion fatigue among nurses in outpatient departments. Methods: A questionnaire survey was conducted among 290 nurses in outpatient departments of four tertiary grade A hospitals from April 15 to 30, 2022. The instruments included the general information sheet, the Chinese version of the Compassion Fatigue Scale, Chinese Version of Perceived Nursing Work Environment, and General Self-Efficacy Scale. Results: More than half (51.4%) of the participants experienced moderate level of burnout, and the incidence of secondary traumatic stress was 72.8%. In the perception of work environment, the score of staffing and resource adequacy was the lowest (3.18±0.71), and the general self-efficacy score was (2.82±0.62). Perceived nursing work environment had a positive effect on general self-efficacy and compassion satisfaction, and general self-efficacy was a mediator in the relationship between perceived nursing work environment and nurses' compassion fatigue. Conclusion: The compassion fatigue among outpatient nurses needs to be further improved. Hospital managers should pay more attention to increasing staff number and their salaries, and use effective strategies to increase nurses' general self-efficacy.
