精准化指导干预模式对妊娠糖尿病患者孕期运动促进的效果

Application of precision-guided intervention model on exercise promotion in patients with Gestational Diabetes Mellitus

摘要: 目的:评价精准化指导干预模式对妊娠糖尿病患者孕期运动的促进作用。方法:采用随机数字表法将2020年5月—9月期间在杭州市某妇产科医院定期接受产检的82例妊娠糖尿病患者随机分成对照组(n=42)和实验组(n=40)。对照组患者采用常规的孕期运动健康宣教模式,实验组患者在对照组的基础上采用精准化指导干预模式管理5周。两组患者持续佩戴运动手环。比较两组患者干预前后孕妇运动自我效能感得分、孕期身体活动问卷调查结果及运动手环监测身体活动水平情况。结果:干预后,实验组患者的孕期运动自我效能感得分、孕期身体活动问卷中等强度活动时间量、运动锻炼时间量、家务活动时间量均高于对照组,久坐时间量少于对照组,差异均具有统计学意义(P<0.05);干预后,实验组患者运动手环监测中等强度活动时间量高于对照组,差异具有统计学意义(P<0.05)。结论:精准化指导干预模式有助于妊娠糖尿病患者在孕期提高运动自我效能感水平,增加孕期中等强度活动时间量,促进妊娠糖尿病患者开展孕期多元化运动。

Abstract: Objective: To investigate the application of precision-guided intervention model on exercise promotion in patients with Gestational Diabetes Mellitus (GDM). Methods: A table of random numbers was used to randomly divide 82 patients with GDM who received regular prenatal care in a obstetrics and gynecology hospital in Hangzhou from May to September 2020 into control group (n=42) and experiment group (n=40). Patients in the control group received standard care while the experiment group received precise guidance intervention model for five weeks in addition to standard care. Patients in both groups wore the fitness tracker continuously. Scores of the Pregnancy Exercise Self-Efficacy Scale, Pregnancy Physical Activity Questionnaire, and physical activity level monitoring by AX3 fitness tracker were compared before and after the intervention for both groups. Results: The score of Pregnancy Exercise Self-Efficacy Scale, the amount of time doing Moderate-to-Vigorous intensity Physical Activity (MVPA), the amount of time exercising, and the amount of time doing house chores during pregnancy were all higher in the experiment group than those in the control group, while the amount of time of prolonged sitting was less in the control group than in the experiment group, and the differences were statistically significant (P<0.05); the amount of time doing MVPA monitored by AX3 fitness tracker was higher in the experiment group than that in the control group, and the difference was statistically significant (P<0.05). Conclusion: The precise-guided intervention model can help patients with GDM improve their exercise self-efficacy during pregnancy, increase the amount of time doing MVPA during pregnancy, and promote the diversities of exercise during pregnancy in patients with GDM.
