北京某肿瘤医院优化开放手术器械包配置的实践与效果分析

Practice and the effect of optimizing the allocation of surgical instrument kits for open surgery in a cancer hospital in Beijing

摘要: 目的:通过优化肿瘤专科开放手术器械包配置,提高手术室运营效率,降低手术室护士及清洗消毒灭菌人员每台的操作频次。方法:根据各外科专业现开展的肿瘤手术实际需求,精简其开放手术器械基础包、增加手术器械单包(特殊情况下使用)。统计优化配置前后各专业开放手术器械基础包及单包的手术器械总数量、手术器械的清洗消毒灭菌重量、节省的相关时间及医生对手术室护士工作的满意度。结果:优化配置后,我院各外科专业开放手术器械总数由8?519件减至7?133件,共减少1?386件,减幅为16.27%,平均每个开放手术器械基础包减少手术器械17.33件;减轻手术器械的清洗消毒灭菌总重量20?958?kg;分别节省手术室护士术中器械处置时间和术后器械清洗消毒灭菌时间各416秒。外科医生对手术室护士物品准备的满意度由优化配置前的93.22%提高到优化配置后的100%。结论:优化配置肿瘤开放手术器械包可缩短术中清点和整理手术器械时间、术后手术器械清洗消毒灭菌时间;手术器械基础包重量减轻,可降低手术室护士及灭菌人员搬运负重;提高外科医生对手术室护士工作的满意度。

Abstract: Objective: By optimizing the allocation of open surgical instrument kits of tumor specialty, improve the operation efficiency of operating room, reduce the operation frequency of each set of operating room nurses and decontamination personnel. Methods: According to the actual needs of tumor surgery currently carried out by each surgical specialty, simplify the basic package of open surgical instruments of its professional tumor specialty, increase the single package of surgical instruments (used under special circumstances). Before and after optimization, the total number of each professional open surgical instrument basic package and single package, the weight of surgical instruments, surgeons' satisfaction with operating room nurses were counted, and calculated the time was saved. Results: After optimization, the total number of open surgical instruments in each surgical specialty of the hospital was reduced from 8519 to 7133, a total decrease of 1386, a decrease of 16.27%; 17.33 surgical instruments were reduced for each open surgical instrument basic package on average; reduced the total weight of sterilization of surgical instruments by 20958 kg; 416 seconds for instruments disposal during operation and 416 seconds for cleaning instruments after operation of operating room nurses were saved. Surgeons' satisfaction with the preparation of nurses in the operating room increased from 93.22% before simplification to 100% after simplification. Conclusion: Optimizing the allocation of tumor open surgical instrument package and removing the redundant surgical instruments in the original surgical package can shorten the time of counting and sorting surgical instruments during operation, shorten the washing time of postoperative surgical instruments, and significantly shorten the operation duration; reduce the weight of the basic surgical instrument package of each surgical specialty, which reduces the labor intensity of nurses and decontamination personnel in the operating room; improve the satisfaction of surgeons with nurses in the operating room.
