癌症患者参与临床试验体验的Meta整合

Experience of cancer patients participating in clinical trials: a Meta-synthesis

摘要: 目的:系统评价和整合癌症患者临床试验参与体验,为改善患者参与临床试验的体验,提高患者临床试验的保留率提供建议。方法:计算机检索PubMed、CINAHL、知网、万方等8个数据库,检索建库至2025年3月31日有关癌症患者参与临床试验体验的质性研究。结果:共纳入13篇文献,包含44个主要结果。最终将其归纳为6个类别,综合为2个整合结果,分别为参加临床试验时患者知情同意不足,决策片面、仓促;患者参与临床试验中存在多维的负性与正性双向体验。结论:试验中的癌症患者分析与决策能力不足且存在多重体验。因此,医务工作者应通过提供多项根本支持措施,满足患者多元需求,有效地将患者试验参与的多维负性体验转化为正性体验,最终提高患者的试验保留率。

Abstract: Objective: To systematically evaluate and integrate the experience of cancer patients' participation in clinical trials, so as to realize "patient-centered" clinical trials to improve their participation experience and patient retention. Methods: The study group searched 8 databases including PubMed, CINAHL, CNKI, Wanfang and so on from their inception to March 31, 2025, to collect qualitative studies on cancer patients' participating experience in clinical trials. Results: A total of 13 qualitative studies were included, covering 44 research outcomes, which were summarized into 6 categories, and integrated into 2 synthesized outcomes. The first finding showed that the patient's clinical decision was insufficiently considered causing the decision to be one-sided and hasty. According to the second finding, the number of negative experiences for patients in clinical trials was almost equal to that of positive experiences. Conclusion: Cancer patients in clinical trials had insufficient analytical and decision-making abilities and multiple experiences. Therefore, healthcare professionals should provide multiple fundamental measures to meet patients' diverse needs, effectively transform the multidimensional negative experiences of trial participation into positive ones, and ultimately improve patient retention rates in trials.
