孕期体质量管理行为及其影响因素的网络分析

Network analysis of gestational weight management behaviors and their influencing factors

摘要: 目的:探索孕期体质量管理行为各维度的内部关联及其与影响因素间的关联,为孕期体质量管理干预提供参考。方法:采用便利抽样法,于2025年3月—6月选取武汉市某三级甲等医院产科门诊的578例孕妇为调查对象,采用一般资料调查表、抑郁-焦虑-压力量表、领悟社会支持量表及体质量管理行为量表进行调查。采用多元线性回归分析体质量管理行为的影响因素,采用网络分析探索体质量管理行为的网络结构,以及体质量管理行为及其影响因素间的混合网络结构。结果:调查对象体质量管理行为得分为(55.75±13.47)分,处于中等水平。体质量管理行为网络分析显示,主动坚持运动为核心节点,制定运动计划并核对执行情况、控制饮食为桥梁节点。混合网络分析显示,较高文化程度与每周测量体质量并记录、减少摄入油炸食品/高脂肪食物及含糖饮料、不在家中储存高卡路里食物或喜爱的食物呈直接正向关联,非在职工作状态与孕期继续工作及正常做轻体力家务劳动呈直接负向关联,抑郁症状与减少摄入油炸食品/高脂肪食物及含糖饮料呈直接负向关联,高社会支持水平与没有体育馆或运动设备仍坚持锻炼呈直接正向关联。结论:孕期体质量管理行为及相关因素呈结构性关联,主动坚持运动、制定运动计划并核对执行情况、控制饮食等关键节点可为干预靶点确定提供参考。

Abstract: Objective: To explore the internal association among gestational weight management behaviors and their influencing factors, and to provide evidence for interventions targeting weight management during pregnancy. Methods: A total of 578 pregnant women attending an obstetric outpatient clinic in a tertiary grade A hospital in Wuhan from March to June 2025 were recruited by convenience sampling. Data were collected through questionnaires including the demographic information form, Depression Anxiety Stress Scale-21, the Perceived Social Support Scale, and the Weight Management Behavior Scale. Multiple linear regression analysis were used to analyze the influencing factors of gestational weight management behaviors, and network models were constructed, including network relationship analysis of gestational weight management behaviors and mixed network analysis of behaviors and influencing factors. Results: The mean weight management behavior score was 55.75±13.47, indicating a moderate level. Network analysis of gestational weight management behaviors showed that persistent exercise was the core node, while making exercise plans and checking implementation status as well as dietary control were bridge nodes. Mixed network analysis demonstrated that higher educational level was directly and positively associated with measuring and recording body weight once a week, reducing the intake of fried/high-fat foods and sugar-sweetened beverages, and not storing high-calorie or favorite foods at home. Non-employment status was directly and negatively associated with continuing to work and doing light household chores as usual during pregnancy. Depressive symptoms were directly and negatively associated with reducing the intake of fried/high-fat foods and sugar-sweetened beverages, whereas higher social support was directly and positively associated with continuing to exercise even without access to a gym or exercise equipment. Conclusion: Based on the analysis, a structured pattern of associations exists among gestational weight management behaviors and the related factors. Active persistence in exercise, making exercise plans and checking implementation status, and dietary control may serve as potential targets for interventions on gestational weight management.
