护士第二受害者同伴支持方案的设计及应用效果

Design and application effect of the peer support program for nurses as the second victims

摘要: 目的:构建可在医院常态运行的护士第二受害者同伴支持方案,评价其可行性与效果,为改善护士心理健康提供参考。方法:2024年7月—10月,从长春市某三级甲等医院不良事件上报系统中选取发生不良事件并自愿参与研究的53名护士,用随机数字表法分为干预组28例与对照组25例。对照组接受常规管理;干预组在接受常规管理的同时,设计并实施同伴支持方案。比较两组护士干预后的第二受害者经验及支持量表、匹兹堡睡眠质量指数、自我关怀量表、护士安全行为量表得分。结果:在不良事件发生后第30天,干预组的第二受害者经验及支持量表得分为(66.25±10.50)分,低于对照组的(83.60±12.10)分,差异具有统计学意义(P<0.05);在不良事件发生后第90天,干预组的自我关怀量表得分、护士安全行为量表得分高于对照组,差异均具有统计学意义(P<0.05)。结论:该同伴支持方案可有效减轻护士第二受害者的心理困扰,提升护士自我关怀与安全行为水平。

Abstract: Objective: To develop a peer support program for nurse second victims that can be routinely implemented in hospitals, evaluate its feasibility and effectiveness, aiming to provide a reference for improving nurses' mental health. Methods: From July to October 2024 at a tertiary grade A hospital in Changchun. Fifty-three nurses who experienced adverse events and voluntarily participated were randomly assigned to an intervention group (n=28) and a control group (n=25) using a random number table. Those in the control group received routine management only, while those in the intervention group received both routine management and the peer support program. The scores of the Second Victim Experience and Support Tool (SVEST), the Pittsburgh Sleep Quality Index, the Self-Compassion Scale, and the Nurse Safety Behavior Scale were compared after intervention between the two groups. Results: At 30th day after the adverse event, the score of SVEST in the intervention group was 66.25±10.50, which was significantly lower than that of the control group (83.60±12.10), with a statistically significant difference (P<0.05). At the 90th day after the adverse event, the scores of the Self-Compassion Scale and the Nurse Safety Behavior Scale in the intervention group were significantly higher than those in the control group, with statistically significant differences (P<0.05). Conclusion: The peer support program can effectively alleviate psychological distress among nurse second victims and enhance their self-compassion and safety behavior.
