乳腺癌患者社交回避及苦恼的分型及影响因素分析

Classification and influencing factors of social avoidance and distress in breast cancer patients

摘要: 目的:分析乳腺癌患者社交回避及苦恼的潜在类别及其影响因素。方法:采用便利抽样法,选取湖南省2所三级甲等医院乳腺外科169例乳腺癌患者为研究对象,采用一般资料调查表、社交回避及苦恼量表、灵性态度及参与列表、接纳与行动问卷、认知融合问卷对其进行调查,针对乳腺癌患者社交回避及苦恼进行潜在剖面分析,采用稳健三步法进行多项式Logistic回归分析研究乳腺癌患者社交回避及苦恼水平的影响因素。结果:潜在剖面分析结果支持2个潜在类别的模型,分别定义为“轻度回避与苦恼型”(59.17%)和“重度回避与苦恼型”(40.83%);年龄、学历、在职情况、居住地、家庭人均月收入、生活自理情况、疾病了解程度、医疗费用支付方式、精神心理状态、心理灵活性对乳腺癌患者社交回避及苦恼潜在剖面分类有预测作用。结论:乳腺癌患者社交回避及苦恼存在2个潜在类别,不同类别的乳腺癌患者社交回避及苦恼水平具有个体差异性,建议医护人员采取有针对性的心理干预措施,促进乳腺癌患者社交功能恢复。

Abstract: Objective: To investigate the potential categories and influencing factors of social avoidance and distress in breast cancer patients. Methods: Using convenience sampling method to select 169 cases of breast cancer patients from two tertiary grade A hospitals in Hunan province. The general data questionnaire, Social Avoidance and Distress Scale, Spiritual Attitude and Involvement List, Acceptance and Action Questionnaire-2nd Edition, and Cognitive Fusion Questionnaire were used in this study. Latent Profile Analysis was used to analyze the classification of breast cancer patients' social avoidance and distress. A robust three-step polynomial Logistic regression analysis was used to explore the influencing factors of social avoidance and distress in breast cancer patients. Results: The results of Latent Profile Analysis supported two potential models, which were defined as "mild avoidance and distress type" (59.17%) and "severe avoidance and distress type" (40.83%). Age, education background, employment status, residence, family per capita monthly income, self-care status, knowledge of disease, payment method of medical expenses, spiritual level and psychological flexibility had predictive effects on the potential profile classification of social avoidance and distress in breast cancer patients. Conclusion: There are two potential categories of social avoidance and distress in breast cancer patients, and the levels of social avoidance and distress in breast cancer patients of different categories have individual differences, which suggesting that medical practitioners should take targeted psychological intervention measures to promote the recovery of social function in breast cancer patients.
