Feasibility analysis of the data-driven nursing quality index network system

Objective: To construct a nursing quality index network system and an electronic quality control platform. Methods: In view of the characteristics and bottleneck problems of nursing quality control as well as the development need of nursing as a discipline, our hospital has constructed a nursing quality index network system and an electronic quality control platform based on the "three-dimensional quality structure" model. Results: The system realized accurate data-driven supervision and control of nursing quality including automatic data acquisition, storage, management, analysis and reuse. Conclusion: The system has so far made remarkable achievements, which will provide some reference for researches of new nursing management paths in the future.

Construction of the rapid response rescue mode in emergency departments

Objective: To establish an emergency medical rescue mode to ensure rapid and effective response and treatment to medical emergencies. Methods: The rapid response rescue mode was developed according to the theory of emergency management with the injuried (patients) being the main focus, emergency departments the key center and a multidisciplinary team including the emergency response office, medical department and nursing department for support. The service-and-efficiency-oriented mode was consisted of three channels and two responding systems to promote emergency rescue capacity and patients’ satisfaction. Results: Compared with the data collected before its implementation, the emergency room retention time, the time for pretriage decreased. An obvious decreasing trend was also noticed when analyzing the response time and the transfer and hand of patients. The score of emergency response ability [(78.20±3.60) vs (96.70±1.70)] as well as the satisfaction degree of both medical staff (87.5% vs 98.8%) and patients (87.5% vs 96.7%) had significantly increased (P<0.05). Conclusion: Implication of the rapid response rescue mode in emergency departments can effectively shorten the treatment time of the injured (patients), enhance the medical response capacity, and improve nursing quality.

Exploration and implementation of Kangaroo Mother Care for critically ill premature infants

Objective: In order to enhance nursing quality and promote better and faster recovery of critically ill premature infants, a standardized Kangaroo Mother Care (KMC) was explored and constructed. Methods: We established a multi-disciplinary team which was led by the Neonatal Intensive Care Unit and assisted by the Obstetrics Department, the Nosocomial Infection Management Department and the Medical Social Work Department. The team then together set the work regulations of KMC and relevant training strategies to make sure its application in clinical practice. Results: The construction and implementation of KMC has not only effectively promoted the growth and neurological development of premature infants but also strengthened parent-infant bounding through skin-to-skin contact. Besides, it has also enabled open communication between medical staff and parents to harmonize the doctor-patient relationship. Conclusion: The KMC turns out to be a successful attempt of cross-cultural adaptation. It has so far been carried out in 47 hospitals within a medical treatment partnership system in Hunan Province and promoted to many other cities across the country.

Application of SEPPV Care Model in cancer-related fatigue of patients after breast cancer surgery

Objective: To analyze the effects of SEPPV Care Model on patients with cancer-related fatigue after breast cancer surgery. Methods: We selected 139 breast cancer patients one month after surgery admitted in our hospital from August 2017 to May 2018 as the research objects. They were then divided into two groups according to the time they were admitted to the hospital. Finally, 69 people were in the control group and the rest in the intervention group. Patients in the control group received conventional nursing while the intervention group received the SEPPV Care Model besides conventional nursing. Indicators including cancer-related fatigue and quality of life were compared between the two groups. Results: The degree of cancer-related fatigue in the intervention group was significantly lower than that in the control group, and the difference was statistically significant (P<0.001); the quality of life of the intervention group was significantly higher than that in the control group, and the difference was statistically significant (P<0.001). Conclusion: The SEPPV Care Model can effectively reduce cancer-related fatigue in patients after breast cancer surgery, thereby improving their life quality.

Construction of preoperative prehabilitation program for surgical patients with lumbar degenerative diseases

Objective: To construct the prehabilitation program for surgical patients with lumbar degenerative diseases based on the concept of Enhanced Recovery After Surgery. Methods: The prehabilitation program was designed by the researchers on the basis of literature review and group discussion. Two rounds of experts consultation were conducted with 13 experts using the Delphi expert consultation method. Results: The active coefficient of experts in both rounds of expert consultation was 100.0% respectively. The ratio of experts who provided opinions was 76.9%. The authority coefficient of experts was 0.87. The final preoperative prehabilitation program included 3 primary items (exercise, nutritional support, and psychological intervention), 9 secondary items, and 25 tertiary items. Conclusion: The preoperative prehabilitation program is reliable and practical. It is expected to provide theoretical basis for better clinical practice to accelerate rehabilitation for surgical patients with lumbar degenerative diseases.

Effects of phase II cardiac rehabilitation among post-revascularization patients with coronary artery disease

Objective: To evaluate the effects of phase II cardiac rehabilitation among post-revascularization patients with coronary artery disease. Methods: Sixty-three eligible participants were recruited to participate in phase II cardiac rehabilitation program which included exercise sessions and patient education lectures. Exercise sessions were conducted three times a week for 12 weeks, with a total of 36 exercise sessions. Educational lectures were provided once a week for 12 weeks. The exercise capacity, anxiety, and depression of participants were measured before and after phase II cardiac rehabilitation. Results: Patient adherence to phase II cardiac rehabilitation was 42.9%. After the phase II cardiac rehabilitation, the exercise capacity value was significantly higher than that in the baseline (P<0.01). Moreover, the scores of anxiety and depression were significantly lower than those in the baseline (P<0.01). Conclusion: The medical professionals should pay attention to patients' adherence to phase II cardiac rehabilitation, as it is found to be relatively low. Phase II cardiac rehabilitation has a significant and positive impact on exercise capacity, anxiety, and depression among post-revascularization patients with coronary artery disease.

Objective: To explore the true psychological experience of rehabilitation exercise for patients with Chronic Low Back Pain (CLBP) and kinesiophobia. Methods: We conducted a semi-structured interview with 11 CLBP patients with kinesiophobia by purposive sampling method, and used the Colaizzi analysis method to analyze, induce, and refine themes of interview data. Results: Totally three themes were extracted, including ineffective self-coping and management (change of self-consciousness, negative coping strategies, negating the effect of rehabilitation exercise), excessive burden of disease (psychological burden, economic burden), and weak support systems (insufficient professional rehabilitation supports, lack of psychological support, insufficient family supports). Conclusion: Medical institutions should pay full attention to the self-management of CLBP patients with kinesiophobia, strengthen the whole process of specialized rehabilitation guidance in and out of the hospital, care for patients' psychological problems, and cooperate with family caregivers to enhance patients' psychological and family support, so as to relieve the patients' pain experience and promote the recovery of the disease.

The current status and influencing factors of hypotension in Plasma Exchange for patients with neuroimmune diseases

Objective: To investigate the current status and influencing factors of hypotension in Plasma Exchange for patients with neuroimmune diseases, so as to provide evidence for clinical interventions. Methods: A total of 69 patients with neuroimmune diseases who underwent Plasma Exchange in the intensive care unit of neurology from a tertiary grade A hospital in Beijing were selected by convenience sampling. The incidence of hypotension during Plasma Exchange was investigated by using a self-designed Plasma Exchange data scale. Results: Among 69 patients, 36 cases (52.17%) had no hypotension, 9 cases (13.04%) had mild hypotension, and 24 cases (34.78%) had moderate hypotension. No severe hypotension or death occurred. Multivariate ordered logistic regression analysis showed that the use of sedative drugs and systolic blood pressure before Plasma Exchange were the influencing factors of hypotension in Plasma Exchange for patients with neuroimmune diseases (P<0.05). Conclusion: The incidence of hypotension in Plasma Exchange for patients with neuroimmune diseases is relatively high, which is related to the use of sedative drugs and the systolic blood pressure before Plasma Exchange. Interventions should be taken for the influencing factors of hypotension to reduce the incidence of hypotension in Plasma Exchange and ensure the safety of Plasma Exchange.

Effect of resourcefulness training on depressed patients after Percutaneous Coronary Intervention

Objective: To analyze the effect of resourcefulness training on resourcefulness level and depressive symptoms in depressed patients after Percutaneous Coronary Intervention (PCI). Methods: According to the time order of PCI, 73 patients with depression were divided into control group (n=37) and intervention group (n=36), and the intervention time was 3 months. The control group was given routine nursing care, and the intervention group was given routine nursing care combined with resourcefulness training. Resourcefulness Scale (RS), Chinese version of Cardiac Depression Scale (C-CDS) were assessed before intervention (T1), 3 months after the intervention (T2), and 6 months after the intervention (T3); the frequency of angina attacks, cardiogenic outpatient visits were counted at T2 and T3 to evaluate the efficacy of the intervention. Results: Before intervention, there was no statistical significance in the level of resourcefulness and depression symptoms (P>0.05). At T2, the RS score of the intervention group was higher than that of the control group, the C-CDS score, the frequency of angina attacks and cardiogenic outpatient visits were lower than the control group, the differences were statistically significant (P<0.05). At T3, the C-CDS score and angina attack frequency in the intervention group were lower than those in the control group (P<0.05). The repeated analysis of variance found that the differences of C-CDS and RS scores between the two groups at different time points were statistically significant (P<0.05). Covariance analysis found that after excluding the time effect, patient's C-CDS scores and RS scores were statistically different between the two groups (P<0.05). Conclusion: Resourcefulness training can improve the resourcefulness level and relieve their depressive symptoms of depressed patients after PCI.

To summarize the transfer work participated by the nursing team of a hospital in the severe rainstorm on July 20 in Zhengzhou, mainly including the establishment of management team, rapid formulation of plan and unified command of action. Combined with this practice and the current situation of disaster nursing development, it is suggested to establish a hierarchical training mechanism for different practice objectives, select and focus on training a group of disaster nursing specialist nurses and reserve specialist talents; strengthen exchanges outside the province, build or introduce disaster nursing curriculum framework and professional teaching materials, so as to improve the training system; and explore the construction of disaster nursing model suitable for local characteristics and improve disaster nursing competence.

Objective: To investigate the core competence of midwives in Yunnan province and analyze the influencing factors. Methods: A cross-sectional survey of 535 midwives from 29 hospitals in Yunnan province was conducted by midwife core competency scale. Results: The total core competence of midwives was 3.99±0.58, and the scores of postpartum and pregnancy health care were 4.09±0.66 and 4.05±0.61, respectively. The lowest score was pre-pregnancy health care 3.76±0.66. Multivariate regression analysis showed that the duration of midwifery practice, nationality, training methods and training duration of midwifery were influencing factors of core competency of midwives (P<0.05). Conclusion: The core competence of midwives in Yunnan province needs to be improved. The national characteristics should be fully considered. The core competence of midwives in Yunnan province can be improved comprehensively with training programs.

Post setting of lung cancer specialist disease manager based on the integrated care management model

Objective: To explore the feasibility and effects of setting up a post of lung cancer specialist disease manager in the management of lung cancer patients. Methods: Through the establishment of a integrated care management center and the specialist disease manager positions, the integrated care management model was implemented for patients with lung cancer. The biopsy screening for malignancy, follow-up compliance rate, diagnosis and treatment completion rate, self-management ability and satisfaction of the enrolled patients were investigated and analyzed. Results: Since the establishment of the post 1 year ago, a total of 5576 patients were enrolled. Among the 186 patients who underwent fiberotic bronchoscopy, the rate of biopsy screening for malignancy was 91.9%, which was significantly higher than the rate before the implementation (35.3%, P<0.001). The follow-up compliance rate in each time period was 100.0%. The medical care service completion rate was 95.3%. Patient self-management score increased from 7.98±2.34 to 9.42±1.63 (P<0.001) and the overall satisfaction rate was 96.3%. Conclusion: The integrated care management model for patients with lung cancer can identify lung nodules early and accurately, assess the lung cancer risk and standardize management of lung cancer treatment, improve patients' self-management ability, and increase patients' satisfaction. The post setting of lung cancer specialist manager provides new ideas for standardizing the management of patients with chronic disease and enriches the connotation of nursing disciplines.

The mediating effect of psychological capital in the relationship between perceived stress and professional identity among psychiatric nurses

Objective: To explore the role of psychological capital between perceived stress and professional identity among psychiatric nurses, in order to improve the professional identity of psychiatric nurses. Methods: Totally 927 psychiatric nurses were recruited from Psychiatric Hospital in Liaoning Province. The questionnaires used in this study included Perceived Stress Scale-10 (PSS-10), Psychological Capital Questionnaire (PCQ), and Occupational Identity Scale (OIS), as well as a demographic data sheet. Hierarchical linear regression was performed to analyze the associations among perceived stress, psychological capital and professional identity. Bootstrap were used to explore the mediating role of psychological capital. Results: The perceived stress score of psychiatric nurses was (17.11±5.75), the psychological capital score was (96.16±18.66), and the professional identity score was (28.81±7.23).The mediating role of psychological capital in the association between perceived stress and professional identity were -0.148 (95%CI: -0.187, -0.113). Conclusion: The perceived stress and psychological capital of psychiatric nurses are associated with professional identity. Psychological capital has some intermediary effects between perceived stress and professional identity among psychiatric nurses. Implementing measures focusing on both perceived stress and psychological capital may provide success in improving the professional identity of psychiatric nurses.

Non-pharmacological intervention of Lower Limb Lymphedema in patients with gynecological cancer after surgery: a network Meta-analysis

Objective: A network Meta-analysis was conducted to compare the clinical effects of 7 non-pharmacological interventions on the relief of Lower Limb Lymphedema (LLL) in patients with gynecological cancer after surgery. Methods: Computer search was conducted in PubMed, Web of Science, Embase, Cochrane Library, CNKI, WanFang Database, CBM and VIP database. The retrieval time was limited from the establishment of the database to January 2021. The "network" and "mvMeta" commands in Stata 13.0 software and Revman 5.4 software were used for statistical analysis. Results: Totally 14 randomized controlled trials were included, including 1090 patients, involving 7 non-drug interventions. Comprehensive clinical application effect, the LLL incidence of the exercise intervention + routine care group was the lowest, while the stress intervention + routine care group had the best effect in reducing the circumference of thigh edema. Conclusion: Exercise intervention + pressure intervention + routine nursing is recommended to alleviate LLL in patients with gynecological cancer after surgery.

Objective: To systematically evaluate the effectiveness of virtual reality technology in reducing pain and anxiety among children undergoing needle-related procedures. Methods: We searched CNKI, Wanfang Data, VIP database, CBM, PubMed, Web of Science and other databases from the inception to September 8, 2020. All the studies on virtual reality technology in reducing pain and anxiety among children undergoing needle-related procedures with randomized controlled design and quasi-experimental design were included. The, and meta-analysis was performed using RevMan 5.3 software after the evaluation of the quality of studies. Results: A total of 8 studies encompassing 729 participants were included. Virtual reality technology had statistical significance on reliving children's self-reported nursing needle-related pain (SMD=-0.46, P<0.001) and needle-related anxiety (SMD=-0.45, P<0.001). Conclusion: Virtual reality technology can relieve pain and anxiety among children undergoing needle-related procedures. Due to the limited number of included studies, more high-quality studies with large sample are needed in the future.

Objective: To construct the prediction model of Back Propagation (BP) neural network-based Post-Intensive Care Syndrome (PICS). The prediction model was constructed based on general characteristics, clinical history data and the evaluation of PICS, 80% of the participants were randomly selected as the training set, the remaining 20% participants were used as the test set, and BP algorithm was used to construct prediction model. Results: The classification accuracy of the model reached 88%, the predicted output and the expected output showed a good degree of coincidence, the overall accuracy of the model was high and the fitting effect was satisfied. Conclusion: The BP neural network-based prediction model can be used to predict the occurrence of PICS.

Objective: To develop and use the intelligent multi-terminal nursing quality management system, so as to strengthen the construction of nursing information, improve the quality of nursing and work efficiency. Methods: A project team was set up to design and develop a multi-terminal nursing quality management system and apply it to the clinical practice. Functional modules included standard resource database, inspection management, report summary, basic setting, etc. Results: After the application of the intelligent multi-terminal nursing quality management system, the detection rate of clinical nursing problems was increased (P<0.05); the time for making nursing quality inspection plan, entering information and conducting statistical analysis was shortened (P<0.05); the execution rate of the head nurse's ward inspection and the satisfaction of nurses to the quality control method were increased (P<0.05). Conclusion: The establishment and application of the intelligent multi-terminal nursing quality management system can promote the scientific nursing management, improve the efficiency and quality of nursing management, help to reduce the management cost, promote the continuous improvement of nursing quality, and speed up the nursing management informatization.

Objective: To develop a program with patients' participation and to evaluate the effects of the program on prevention of perioperative pressre injurty. Methods: The literature review and Delphi method were used to develop the program with patients' participation. A total of 110 patients undergoing hip fracture surgery from May 2020 to January 2021 were recruited and randomly divided into the control group and the interventional group with 55 cases in each group. Patients in the control group received routine care. Patients in the interventional group received the program intervention besides routine care. Results: The incidence of Perioperative Pressure Injury in the interventional group was lower than that in the control group (P<0.05). The comfortable level in the surgical area and the actual program participation of the patients in the interventional group were higher than those in the control group (P<0.05). Conclusion: The program with patients' participation can effectively prevent the occurrence of perioperative pressure injuries in patients undergoing hip fracture surgery, improve the comfort of the patients and the actual level of patient participation, and provide a new model for care in operating rooms.

Objective: To evaluate the effect of complication early warning model in breast cancer patients undergoing day chemotherapy for the first time. Methods: A total of 86 patients with breast cancer who received chemotherapy for the first time in the oncology internal medicine day ward from January to December 2020 were selected, and divided into control group (n=42) and experimental group (n=44) according to the admission time. In the experimental group, the early warning model of chemotherapy complications nursing information platform was used from the second day to the fourteenth day after discharge, while the control group was given routine telephone follow-up and consultation guidance. The intervention effect of the two groups was compared with the Common Terminology Criteria for Adverse Events 4.0 and the quality of life of breast cancer patients. Results: The grade of complications during intermission of chemotherapy in the experimental group were lower than that of the control group, and the grade of leucopenia, thrombocytopenia, vomiting were significantly lower than that of the control group (P<0.05). The grade of constipation in the experimental group was significantly lower after the intervention (P<0.05). The quality of life of breast cancer patients in the experimental group (112.73±4.33) was higher than the control group (P<0.05). Conclusion: The chemotherapy complications warning model can lower the grade of chemotherapy complications in breast cancer patients during intermission of chemotherapy, improve the quality of life, and worthy of clinical promotion.

Objective: To evaluate the effect of integrated care model for children undergoing peritoneal dialysis with End Stage Renal Disease. Methods: A multidisciplinary, integrated care model was applied among children undergoing peritoneal dialysis aged 0 to 18 years old in Children' s Hospital of Fudan University from January 2012 to December 2020. The model included prescribing treatment modalities based on an expectation of dialysis and quality of life of children and their families; optimizing the treatment based on families' preference; dynamically assessing families' financial conditions and caregivers' psychological status; coordinating the resources, and assisting in applying for fundings. During the dialysis initiation, the model included comprehensive management of children and their families during hospitalization, including symptoms management, dialysis care, training and performance assessment peritoneal dialysis, and the continuous care and routine follow-ups of families with children undergoing peritoneal dialysis after discharge. Results: From 2018 to 2020, the peritonitis rate was 0.10-0.12 per patient-year. The optimized outcomes of peritoneal dialysis were achieved through continuous quality improvement with the one-year survival rate of 97% and the 5-year survival rate of 95%. Conclusion: The implementation of the integrated care model lowered the incidence of peritoneal dialysis related complications, optimized the peritoneal dialysis treatment, and improved children's quality of life.

Objective: To explore the effects of National Metabolic Management Center (MMC) in management of blood glucose and dietary behavior in patients with type 2 diabetes. Methods: Records of 1594 patients who entered the MMC of a Grade III Level A hospital in Shanghai from June 2017 to April 2020 were reviewed. These patients were regularly followed by the center for a year. The blood glucose level after 1 year was observed. The effects on dietary behavior were also observed. Results: After 1 year of MMC management, the blood glucose, blood pressure, triglycerides and cholesterol level of patients decreased significantly compared with baseline (P<0.05). Patients significantly changed their daily vegetable and fruit intake behavior and daily vegetable intake behavior than baseline (P<0.05). Conclusion: MMC management can effectively improve the blood sugar levels in patients with type 2 diabetes, and the Center can help patients better establish healthy diabetes eating behaviors.

Objective: To establish a nursing consultation outpatient clinic based on the Internet cloud platform for prevention and reduction of the incidence of neonatal diseases. Methods: The hospital's WeChat official account was connected with a mobile health platform of a third organization to build a nursing consultation cloud platform. Targets of the services, contents of the consultation, service process, and system feedback were clearly defined. The effects of the consultation clinic based on Internet cloud platform were evaluated. Results: From May 2017 to March 31, 2021, there were 1393 consultations in the cloud outpatient clinic and 797 in the offline outpatient clinic. During the same period, the number of consultations in the cloud outpatient clinic was significantly higher than that in the offline outpatient clinic (P<0.001). The number of newborn feeding care, skin care, and sleep care was significantly higher than those in the offline outpatient clinic (P<0.001). Conclusion: Neonatal nursing cloud clinic can effectively meet the needs of neonatal continuity care, solve the problems of parents, prevent some neonates from going to the hospital due to inappropriate care, and save social resources. Clinical nurses can obtain new space for personal development. Excellent nurses can also gain new value for themselves.

Objective: To establish a "hospital-community-family" ternary linkage cardio-cerebrovascular disease health education model, and to evaluate the effects of this model. Methods: The "hospital-community-family" ternary linkage health education program was developed based on the cardio-cerebrovascular health education base of a tertiary hospital.The high-quality resources were shared including cardiovascular disease knowledge training, CPR first aid skill training, and self-management skills training. Results: After the health education, knowledge of cardiovascular and cerebrovascular diseases and skills of cardiopulmonary resuscitation techniques were significantly improved. The self-management ability of patients with cardiovascular and cerebrovascular diseases at home was significantly enhanced (P<0.001). Conclusion: The "hospital-community-family" ternary linkage of health education on cardiovascular and cerebrovascular diseases can facilitate the high-quality resources sharing. The program can improve public's ability to prevent cardiovascular and cerebrovascular diseases, increase the success rate of rescue of cardiovascular accidents outside the hospital, and improve the prognosis of patients. The program is worthy of being promoted in the future.

Objective: To explore the effect of Information-Motivation-Behavior Skill (IMB) –based "Internet +" transitional care in patients with Acute Myocardial Infarction (AMI) after Percutaneous Coronary Intervention (PCI). Methods: From July, 2020 to October, 2020, a total of 74 patients with AMI after PCI were divided into two groups randomly. The participants in the control group were with traditional transitional care, and those in the experimental group were with IMB-based "Internet +" transitional care. Results: One and three months after discharge, the quality of life, treatment and lifestyle compliance, anxiety and depression of the participants in the experimental group were significantly better than those in the control group (P<0.05). Conclusions: IMB–based "Internet +" transitional care model is benefit to improve quality of life, compliance, anxiety and depression of patient with AMI after PCI, which is worthy of further exploring.
