Abstract: Objective: To establish a "hospital-community-family" ternary linkage cardio-cerebrovascular disease health education model, and to evaluate the effects of this model. Methods: The "hospital-community-family" ternary linkage health education program was developed based on the cardio-cerebrovascular health education base of a tertiary hospital. The high-quality resources were shared including cardiovascular disease knowledge training, CPR first aid skill training, and self-management skills training. Results: After the health education, knowledge of cardiovascular and cerebrovascular diseases and skills of cardiopulmonary resuscitation techniques were significantly improved. The self-management ability of patients with cardiovascular and cerebrovascular diseases at home was significantly enhanced (P<0.001). Conclusion: The "hospital-community-family" ternary linkage of health education on cardiovascular and cerebrovascular diseases can facilitate the high-quality resources sharing. The program can improve public's ability to prevent cardiovascular and cerebrovascular diseases, increase the success rate of rescue of cardiovascular accidents outside the hospital, and improve the prognosis of patients. The program is worthy of being promoted in the future.
