Abstract: Objective: To synthesize the evidence on exercise-based weight loss interventions for childhood obesity globally, aiming to provide a scientific basis for clinical practice. Methods: Based on the "5S" evidence hierarchy model, a systematic search was conducted in Chinese and English databases, guideline platforms, and academic platforms to identify clinical decisions, guidelines, evidence summaries, expert consensus statements, and systematic reviews. The retrieval period spanned from October 31, 2015 to January 31, 2026. Reviewers assessed the quality of the included studies, extracted and classified the evidence. Results: A total of 4 clinical decisions, 6 guidelines, 1 expert consensus, and 5 systematic reviews were included. Twenty-three evidence items were synthesized across six domains: exercise assessment, exercise program formulation, exercise intervention implementation strategies, exercise intervention promotion strategies, exercise fatigue and injury management, and exercise intervention effectiveness evaluation. Conclusion: The evidence summarized in this study for exercise-based weight loss interventions in childhood obesity is relatively comprehensive, enabling healthcare institutions and clinical practitioners at all levels to translate and apply the evidence according to their local medical resources and clinical setting.
