Abstract: Objective: To compile a questionnaire suitable for investigating the actuality of puerperae's knowledge, attitude, and practice of rehabilitation for postpartum diastasis recti abdominis, and to examine its reliability and validity. Methods: Based on the knowledge-attitude-practice theory, the primal questionnaire was compiled through literature review, Delphi expert letter inquiries and pre-survey. A questionnaire survey was conducted on 532 puerperae to test the reliability and validity of the questionnaire to form the final questionnaire. Results: The final questionnaire was made up of 3 dimensions with 32 items. Five common factors were extracted by exploratory factor analysis extracts, and the cumulative variance contribution of the factors was 73.7%. Confirmatory factor analysis showed that the model structure fits well. The content validity of each item in the questionnaire ranged from 0.824 to 1.000, and the scale-level content validity index of whole questionnaire was 0.980. The Cronbach's α coefficient of the whole questionnaire was 0.965, the split-half reliability was 0.876, and the retest reliability was 0.884. Conclusion: The questionnaire has good reliability and validity. It is suitable for evaluating the level of puerperae's knowledge, attitude and practice of rehabilitation for postpartum diastasis recti abdominis.
