Abstract: Objective: To evaluate the effect of post-ICU clinic service. Methods: We established a post-ICU clinic, adopted the medical-nursing collaborative diagnosis and treatment mode, comprehensively evaluated patients discharged from the ICU, developed rehabilitation plans and guide implementation, specialist referral when necessary, and provided dynamic continuous nursing services. Results: Totally 146 patients were followed up, including 346 cases of health assessment, 334 cases of exercise program guidance and daily living ability training, 214 cases of respiratory training and 379 cases of health guidance since the clinic was opened. The rehospitalization rate was lower than the telephone follow-up patients (P<0.05). The rate of returning to work was higher than the telephone follow-up group (P<0.05). Patients' satisfaction was 99.07%. The visiting nurses believed that the post-ICU clinic was helpful to improve their ability to deal with professional problems. Conclusion: Post-ICU clinic can effectively solve the rehabilitation problems of discharged patients from ICU, which is conducive to the recovery of patients' physical and social functions, improves patients' satisfaction and nurses' professional ability.
