Abstract: Objective: To establish and manage the heart failure care clinic with doctor-nurse collaboration. Methods: To establish the heart failure care clinic with doctor-nurse collaboration through selecting and training nursing experts, preparing outpatient equipment and defining the workflow and content of specialized nursing outpatient service; controlling the quality of the heart failure care clinic with doctor-nurse collaboration. Results: Compared with the controlled group, the rate of mortality in 1 year decreased in outpatients with heart failure (P<0.05), the readmission frequency of heart failure patients was significantly reduced (P<0.05) and the quality of life of patients with heart failure was improved (P<0.05). Conclusion: It is helpful to improve the self-management ability of patients with heart failure and improve the quality of life of patients by establishing and managing the heart failure care clinic with doctor-nurse collaboration.
