可穿戴设备测量急性期脑卒中患者血压的准确性研究

Accuracy study of wearable devices in blood pressure monitoring in the acute stroke patients

摘要: 目的:验证基于光电原理的可穿戴设备对急性期脑卒中住院患者血压监测的准确性。方法:采用诊断性试验评价研究设计,采用方便抽样法,于2021年9月—12月选择在北京市某三级甲等医院神经内科住院的急性期脑卒中患者35例为研究对象,在相同时间分别使用可穿戴设备和床旁心电监护仪采集血压数值,以床旁心电监护仪测量的血压数据为金标准,对比可穿戴设备血压监测的准确性。结果:测得414组血压值,可穿戴设备与床旁心电监护仪测得的收缩压(SBP)kappa值为0.768,灵敏度为89.1%,特异度为87.7%;两种方式测得舒张压(DBP)的kappa值为0.648,灵敏度为94.6%,特异度为67.9%。使用两种方法测量SBP与DBP的结果,差异均无统计学意义(P>0.05)。结论:使用可穿戴设备对急性期脑卒中住院患者进行血压测量具有较好的准确性,可用于急性期脑卒中患者的动态血压监测。

Abstract: Objective: To verify the accuracy of wearable wrist blood pressure monitoring watch (hereinafter referred to as wearable wrist watch) based on photoelectric principle in blood pressure monitoring of hospitalized patients with acute stroke. Methods: Diagnostic test evaluation study design was used. By using convenient sampling method, the neurology patients with acute stroke of a tertiary grade A hospital from September to December 2021 were selected as the research participants, and at the same time, the wearable wrist watches and bedside ECG monitor instrument were used to collect blood pressure values. Blood pressure data measured at bedside ECG monitor was used as the gold standard to compare the wearable wrist watches the accuracy of blood pressure monitoring. Results: A total of 35 patients with acute stroke were enrolled, and 414 groups of blood pressure values were measured. The SBP kappa value measured by wearable wrist watch and bedside ECG monitor was 0.768, with 89.1% sensitivity and 87.7% specificity. The kappa value of diastolic blood pressure (DBP) was 0.648, the sensitivity was 94.6%, and the specificity was 67.9%. There was no significant difference in SBP and DBP measured by the two methods (P>0.05). Conclusion: Wearable wrist watch measuring blood pressure in patients with acute stroke in the hospital has a good accuracy, and can be used in the acute stroke patients ambulatory blood pressure monitoring.
